Risk assessment of domestic and industrial effluents unloaded into a freshwater environment.

An ecotoxicologic study was performed to assess the environmental status of the Lujan River. It is an important freshwater system in the northeast of Buenos Aires Province, Argentina. Surface waters (SWs) and liquids effluents (LEs), before they reached the river, and sediments were assessed via acute toxicity screening using a battery of tests with native species. Additionally, the presence, in each LE and SW sample, of bioaccumulatable compounds was checked by SPME extraction and gas chromatograph-MS determination. An environmental risk assessment of each LE was carried out via toxic units and assessment factors approach and through extrapolation methods. Hazardous concentrations for each LE were compared with their river effluent concentrations. Ninety-one percent (91%) of the total toxic load of the river was due to 4 of 11 LEs (37%) evaluated. Although SW samples were not toxic, a real environmental risk was found for this freshwater environment. Sediment toxicity was found to be related to the proximity to pipe discharges. Bioaccumulatable compounds were found in SWs and in LEs. Esters of phthalic acids, morpholine, hydroquinone, and nonylphenol were found throughout the river at different sample sites and in different months during the 1-year sampling program.

Design of a PC controlled test device for the study of patient motion in X-ray radiology: first applications and results.

Simulation and study of patient motion should be undertaken for different imaging techniques since all new radiological techniques or equipment must pass through evaluation processes to verify their functional capability and assess their performance compared with other equipment. A device to study the effects of motion on X-ray image quality was designed and built. The main goal of the PC controlled device developed by our group is to allow the adaptation of different phantoms and test objects. Preliminary experiments for the evaluation of different thorax screen-film systems were carried out by simulating respiratory motion. It is concluded that moving objects should be included for the comparison of screen-film systems since predictions based only on findings in stationary test conditions could lead to false conclusions.

[Primary T-cell lymphoma in the central nervous system of immunocompetent patients. Clinical and pathological study of two cases].

Primary T-cell lymphoma in the central nervous system has rarely been described in the literature. We report 2 such cases in immunocompetent patients. The first presented with subacute encephalopathy affecting both memory and speech. A CT-scan of the head showed a contrast-enhanced mass around the third ventricle. The second presented with 2 partial seizures with secondary generalization; the CT-scan in this case showed a right temporal lesion. Both underwent surgery for total removal of the masses followed by cranial irradiation. Tissue examination confirmed the diagnosis. The first patient died 14 months after diagnosis. In the second case lymphoma recurred 8 months after surgery.

[Laser CO2 in the surgery of acoustic neuroma].

The advantages derived from the use of the CO2 Laser in the treatment of tumours of the cranial base have been widely reported. The authors present their results after the resection of 12 cases of giant acoustic neurinomas operated on with microsurgical technique and CO2 Laser, comparing these results with a similar series, already published (Neurochirurgie, 1984, 30, 17-24), of 12 cases operated on only with microsurgical technique. The present methodology includes the evaluation with magnetic resonance imaging and with pre and intra-operative evoked potentials. A 60 Watt CO2 Laser adapted to the surgical microscope was used. The following parameters are compared: mortality, facial nerve preservation, duration of surgery, and hospitalization and and recovery. The only advantage of the conventional microsurgical techniques over the laser technique was the duration of surgery (5.5 hours vs. 6.1 hours). The other parameters improved with the use of the laser. This work, although retrospective, shows less surgical trauma when using the CO2 Laser associated with the conventional microsurgical technique.

Medulloblastomas: a review of 11 cases.

The authors review 11 cases of medulloblastomas, 9 of which were diagnosed and treated in the last 6 years. All were uniformly treated by surgery, radiotherapy and chemotherapy, according to the SIOP protocol. Complications and results are described.

Surgical experience with arteriovenous malformations in children.

The authors report their surgical experience with 15 cases of arteriovenous malformations in children operated on in the last decade. Subarachnoid hemorrhage was the main clinical manifestation in 12 patients. Epilepsy was a secondary feature in 3 of these. Cortical areas were involved in 7 cases while the other 8 had the malformation in medial or deep structures of the brain. No mortality occurred from surgery. A radical excision was achieved in 12 cases, while in 3 cases remnants of the lesion were shown by postoperative angiography. Nine patients recovered from surgery without neurological impairment. Two children with left-sided arteriovenous malformations showed mild speech disturbances and 2 others with midline lesions suffered from hypotonus and ataxia.

[Abdominal metastasis of a pineal germinoma in a child with a ventriculo-peritoneal shunt].

We present a case of germinal tumor of the pineal gland, treated with a ventriculo-atrial shunt, cytostatics and radiotherapy. Two and a half years later, in Spite of the clinical remission of the cerebral tumor a metastasis appeared in the abdominal cavity. After reviewing the current literature, the necessity of using millipore filters in the CSF derivation systems when dealing with malignant cerebral tumors to avoid the spreading of malignant cells through the CSF.

[Sudden tetraplegia caused by hemorrhage in a central-medullary astrocytoma].

A case is reported of central-medullary cervical astrocytoma, which without any previous history is shown by a sudden pattern of tetraplegia and breathing insufficiency. The myelography suggested the existence of an expansive intramedullary process, carrying out an urgent surgical decompression and evacuating a central-medullary hematoma in relation to a glial benign type. Various aspects of the sudden tetraplegia are discussed.

[Syringomyelia due to arachnoiditis. Clinical-radiological description of 5 patients].

Five patients (mean age 44 years--range 21-67 years) with arachnoiditis of different origin who posteriorly developed syringomyelia are presented. The etiopathogenic mechanism of the arachnoiditis was post injury in one case, 2 patients had history of meningitis (tuberculous and pneumococcic) and in the other two no related factor was found. The period of latency among the causes which originated arachnoiditis and the diagnosis of syringomyelia oscillated between 10 months and 16 years. The localization of the cavity was dorsal in 3 cases, cervical in another and in the last it was extended along the whole spine. No patient demonstrated the Arnold-Chiari malformation nor basilar impression. Analyzing the clinical history, radiologic studies and surgical findings the most probable etiopathogenic mechanism involved in each case is discussed.
