[Utility of the surgical treatment for severe epistaxis by endoscopic approach of sphenopalatine and ethmoidal arteries].

INTRODUCTION: To evaluate the efficacy and cost-effectiveness of the treatments used for controlling epistaxis, particularly compared with the surgical endoscopic ligation or cauterization of sphenopalatine (SP) and anterior ethmoid (AE) arteries, a intervention prospective-retrospective study is presented with the aim of assessing the feasibility of these surgical techniques as an alternative to conventional treatments. PATIENTS AND METHODS: 184 consecutive patients admitted in the ENT ward between the years 1997 and 2005 were included in the study, and distributed into three groups depending on the last treatment applied to control their bleeding: AP group.- anterior packing (n=98); PP group.- posterior packing (n=66), and ES group.- endoscopic ligation and/or cauterization of SP or AE (n=20). RESULTS: Highly significant differences were found between the groups, not only regarding the efficacy (90% for surgical treatment compared with 41.3% for AP and 63.1% for PP), but also the length of postreatment hospital stay (AP.- 4.92 days; PP.- 6.3 days; ES.- 2.15 days). DISCUSSION AND CONCLUSIONS: The lack of efficacy of conventional treatment and the increasing risk factors that condition nasal bleeding, together with the advance of endoscopic and anesthesic procedures have brought along the possibility of a surgical solution for these patients. Our results demonstrate the feasibility of these techniques as alternative for nasal packing in the treatment of posterior epistaxis, and even its ethical and economical convenience as a substitutive to posterior packing.

[Metastases of pulmonary small cell carcinoma on cervical skin noted before the primary tumor].

We are reporting the case of a patient with final diagnosis of bronchopulmonar carcinoma of small cells who at first place assisted to E.N.T. Department due to a cervical node (adhered to the dermis) and hemoptysis of more than 2 months. We resected the neck mass which confirmed the presence ofa cutaneous metastases of an oat cell carcinome, being the patient after word ea sstudy by the Neumology Service, who found a righth lung tumor, of the same histological type. In spite of the treatment the patient die.

[Melkersson-Rosenthal syndrome. Report of a case with bilateral facial palsy].

Melkersson-Rosenthal syndrome is a rare illness characterized by secondary facial edema mainly located on the upper lip, recurrent facial palsy and plicated or fissured tongue. We report the clinical case of a 30-year-old female with the classic triad but severe bilateral affectation of the facial nerve. We describe the results of all the tests and complementary explorations we asked, besides the associated pathologies, that carried us to the definitive diagnosisjoint to her symptoms. Finally we have performed a short review of the literature at respect of this entity.

[Postraumatic massive epistaxis. An indication for selective arterial embolization].

We report a 38 years old male admitted to our hospital with massive epistaxis secondary to nasal trauma which required to perform a posterior packing, transfusion and embolization of the internal maxillary artery for stopping the nosebleed. Then we made a FENS (Functional Endoscopic Nasal Surgery) by cauterization of several bleeding points on nasal septum following an anterior packing that was removed after 48 hours without new epistaxis. In a patient like this we think that an angiography with selective embolization is the main choice when other options have failed.

[Recurrent and severe epistaxis secondary to nasal trauma. Report of an atypical case in a 22-year-old male].

We report the case of a 22-year-old male who presented a diferred, recurrent and severe epistaxis after nasal trauma which required several admissions in our hospital and different treatments. Concretely by chronological order it has been practiced anterior tamponades, endoscopic ligature-cauterization of the sphenopalatine artery, classic posterior packing and finally a new FENS with cauterization of anterior ethmoidal artery. Besides it was necessary a transfusion to go up the anemia suffered by the patient whose global stay was 16 days. Recurrent postraumatic epistaxis are very rare but can be represent a difficult problem to solve.

[Maxillary sinusitis by Fusarium sp. Report of a case and literature review].

We report the clinical case of a 54 year old male who was diagnosed as left maxillary sinusitis with poor evolution in spite of several treatments. All the tests performed on this patient are shown and they allowed the final anatomopathologic diagnosis of infection by Fusarium sp., an opportunist fungal pathogen very rare as cause of an ENT infection. It was necessary to practice a Caldwell-Luc technique or extern antrotomy and an antifungal therapy (itraconazol) to obtain a significant improvement. Finally a bibliographic review has been made at respect of this pathology.

[Ethmoidal epidermoid carcinoma showing as cluster headache. Report of a case and literature review].

We report the case of a 54 years old male diagnosed as ethmoidal epidermoid carcinoma which, after almost 1 year--asymptomatic period, debuted with facial pain (cluster headache) without another associated clinic. Sino-nasal malignant tumours represent about 5%-6% in all ENT carcinomas and they use to be of later diagnosis, with high probability of local recurrence and invasiveness on structures like orbit and skull base. The main treatment for ethmoidal squamous carcinomas is surgery and depending on staging can require radiotherapy and/or chemotherapy. A bibliographic review is done at this respect.

[Nasopharyngeal angiofibroma. Our experience (1990-2001)].

We present our experience in the treatment of nasopharyngeal angiofibromas, tumours not very common, with a benign histology but with the risk of severe epistaxis or possible involvement of vital structures. In a period of 12 years (1990-2001) we have treated 6 males with this pathology, operating 5 of them. We describe the clinical manifestations, the tumour stages following the Fisch classification, the diagnostic and surgical techniques used, the evolution of the patients and their postoperatory. We review also the literature about this subject.

[Treatment of thyroid cancer. Retrospective study and review of the literature].

We present a retrospective study about our experience in the treatment of thyroid carcinoma. For that, between 1992 and 1997, we have collected 37 malignant thyroid tumours (15%) over 250 operations on thyroid gland by our ENT Service. We analysed several variables: Age, sex, personal antecedents, clinical symptoms, results of the complementary tests (thyroid hormones, ultrasonography, scintigraphy and FNAB), type of surgery complications and number of recurrences. The thyroid carcinomas, specially differentiated tumors, are an increasing pathology with tendency to affect females. In our serie usually they are solitary thyroid nodes, scintigraphically cold ones, in which FNAB as the best test. We think that a total thyroidectomy is the best treatment and it must be made by an ENT.

[Papillary carcinoma on thyroglossal duct cyst].

Papillary carcinoma ansing on a thryroglosal duct cyst is a rare tumor. Since Ucherman described first case in 1915, only 150 cases have been reported. We present clinic evolution and treatment of a new case and review literature and discussion about tumor origin, adequate diagnosis test and treatment. This should include Sistrunk procedure but further surgery on thyroid gland is not accepted by all authors.

[Primary extramedullary plasmocytoma of the tongue base].

Primary extramedullary plasmacytoma (PEP) are malignant neoplasm of plasma cells. They are very rare, however 80% of the cases appear in head and neck region. We reported a rare case of PEP of the base of the tongue, in a 44-year-old male with oral bleeding.

[Laryngeal paralysis and olivopontocerebellar atrophy. Apropos of a case].

We display the study performed to a female patient affected of laryngeal paralysis to become, based in clinical and radiologic criteria, to diagnose her cerebellar atrophy. We justify our work because of how infrequently this illness heredodegenerative of the central nervous system begins with cranial pairs paralysis. We emphasize the importance that the new methods of explorations specially TAC and IRM, have to guess the possible etiologies of central originated paralysis.
