Optimizing professional education in public health.

Traditional public health education has lacked required practicums such as are the rule in other health professions. Described is a model professional education program that is built around required course work in all areas of public health and a series of mandatory internships at sites of public health practice, utilizing state and local health departments, other federal and state agencies, and public and private sites where students are supervised in public health rotations similar to those a medical, dental, nursing, or veterinary student would experience in a clinic or ward.

Anaesthesia for caesarean section in a patient with Watson's syndrome.

Watson's syndrome is an uncommon genetic disorder whose features include mental retardation and pulmonary valvular stenosis. The purpose of this report is to describe one management strategy used successfully to anaesthetize a woman with this disorder who presented for Caesarean section. Epidural anaesthesia using 0.5% bupivacaine without epinephrine in 2-3 ml boluses to a total dose of 15 ml was administered over 30 min. Invasive haemodynamic monitoring in the form of arterial and central venous catheters were used to guide therapy and help ensure maternal and fetal well-being. Intravenous ketamine in doses of 10-20 mg every five minutes to a total dose of 245 mg (4.5 mg.kg-1) was used to overcome the patient's uncooperative nature and facilitate invasive procedures. Postoperative analgesia was provided using 3 mg epidural morphine. The patient was observed in the intensive care unit for the first postoperative day and experienced an uncomplicated intra- and postoperative course. We conclude that this technique represents a safe and effective method for anaesthetizing patients with this complicated problem for Caesarean section.

Preadmission anaesthesia consultation clinic.

Many surgical procedures are delayed or cancelled due to inadequate preoperative assessment and preparation. We describe the case referral pattern and efficiency of our preadmission anaesthesia consultation clinic, which was designed to minimize this problem. Data were collected prospectively on the first 400 patients referred to the clinic. Sixty per cent of referrals were related to the cardiovascular system, in particular coronary artery disease (27%). Eighty-one per cent of referrals were considered appropriate. With ideal functioning of the clinic, delays and cancellations could potentially be reduced to 0.75% and 1.5% of cases respectively. The clinic was well accepted and appreciated by patients. We conclude that this represents a potential important reduction in hospital costs and improvement in operating room efficiency.

[A contemporary model of environmental health sciences: its application in the recruitment of faculty and in academic development].

The authors describe the development and application of an intellectual model fitted to the environmental health sciences. The model became the basis for the design of a new study plan in addition to providing the strategy for recruitment of the faculty for the new School of Public Health of the San Diego State University in San Diego, California. They give an account of the circumstances leading up to the establishment of this institution, the gradual consolidation of the different health science programs into a single school with graduate studies leading to masters degrees in six main fields: environmental health, occupational health, epidemiology, maternal and child health, health services administration, and health development. The authors see the beginning of a redefinition of the function of the basic sciences represented in environmental health in the light of new knowledge and technology, and believe that all fields of knowledge (microbiology, parasitology and health development) will contribute as a unified whole to increase the capacity of public health to place both biological and environmental health problems in a context that society can control.

Water and health.

The systems protecting the quality of drinking water in the United States are deteriorating. Many urban water and sewer systems are obsolete. Industrial wastes are imposing a burden on wastewater treatment facilities. In the U.S., outbreaks of water-borne disease are primarily limited to enteritis associated with viruses or Giardia. Issues in the contamination of drinking water by toxic substances include disposal practices, accidental contamination, agricultural runoff and chlorination byproducts. Physicians must support stringent enforcement of water quality standards.
