[Is there still a place for surgery in the treatment of communicating pseudocyst in chronic pancreatitis?].

The purpose of this study was to evaluate the results of long term percutaneous catheter drainage in the treatment of chronic pancreatic pseudocysts communicating with pancreatic ducts. Since 1986, 13 consecutive patients with symptomatic pseudocysts were treated prospectively. All pseudocysts were associated with chronic pancreatitis. The location was head (n = 8), body (n = 3) or tail (n = 2). The procedure included fine needle aspiration under ultrasonic control, percutaneous drainage along a guide wire and closed suction aspiration. A radiological control was performed weekly and the catheter was left in place until total resolution of pseudocyst. Immediate relief of pain and resolution of pseudocyst were obtained in all 13 patients. The average length of hospital stay was 12.2 +/- 8 days (1 to 23). The average duration of drainage was 36 days. There was no mortality of morbidity related to the drainage. There were two asymptomatic recurrences, but long-term follow-up (mean 3 years) showed that all 13 patients were symptom-free. The clinical results of such a long-term catheter drainage seem better in our study than in other previous series. It is a simple and innocuous procedure, and could be carried out in an outpatient unit. Such drainage represents an efficient alternative to endoscopic or surgical treatment of chronic pancreatic pseudocyst with ductal communication. In our experience surgery is no longer indicated.

[Value and limitation of ultrasound-guided percutaneous drainage in acute lithiasic cholecystitis].

The aim of this study was to evaluate the results of ultrasound guided percutaneous cholecystostomy in the treatment of acute calculous cholecystitis. From July 1987 to December 1980, 32 patients (mean age = 80) were treated prospectively by this procedure. There was only one death (3%). Successful treatment was achieved in 70% of the patients and was related with mobilization of the gallstone impacted in the infundibulum of the gallbladder. When mobilization of the impacted stone cannot be obtained, surgery is indicated within one week, as later surgery is much more difficult. Twenty-three percent of the patients successfully cured later died from non biliary disease; this indicates the very poor condition of most of the patients. In our experience, percutaneous cholecystostomy is a safe and minimally invasive procedure in the treatment of elderly high-risk patients presenting with acute calculous cholecystitis.

Intraoperative radionuclear 125I-labeled metaiodobenzylguanidine scanning of pheochromocytomas and metastases.

We developed a technique to assess the feasibility of intraoperative radionuclear detection of pheochromocytomas and their metastases. Thirteen patients were entered into the study: five control subjects with nonchromaffin adrenal tumors, eight with pheochromocytomas, and one of these patients showing bone metastasis. Each subject received thyroid blockade and an intravenous injection of 500 microCi (37 megabecquerels) 125I-labeled metaiodobenzylguanidine (MIBG) 3 days before surgery. In the five control subjects, adrenal tumor uptake never exceeded the liver or spleen uptake. One patient with a negative preoperative MIBG scan demonstrated no intraoperative uptake. Five patients with pheochromocytoma had positive preoperative scans and in one other patient preoperative scanning was not done. In each of these six patients intraoperative count ratio of pheochromocytoma/liver from 14:2 to 250:16 and pheochromocytoma/contralateral adrenal ratio from 60:1.5 to 60:16 was demonstrated. An intraoperative scan in one of these patients detected two small metastatic tumor deposits previously overlooked by the surgeon after removing a larger mass that had been localized by a preoperative 131I-MIBG scan. A negative preoperative scan in one patient was followed by an intraoperative scan demonstrating a bone metastasis with a ratio of metastasis/normal bone of 10:0.5. Specimen studies demonstrated a significant MIBG uptake ratio of tumor/plasma ranging from 95 to 667 (average 404 +/- 242) greater than in control subjects (average 25 +/- 41); in the patient with metastasis the uptake ratio of metastasis/normal bone reached 98.4. We conclude that intraoperative 125I-MIBG scanning might detect pheochromocytoma deposits overlooked by preoperative 131I-MIBG scans.

[Anesthesia for cesarean section and excision of pheochromocytoma caused by Sipple's syndrome].

A case of a 23-year-old primigravida with a tumour of the left adrenal gland and a medullary thyroid carcinoma is reported. Her blood pressure remained at about 100/60 mmHg throughout pregnancy. She was scheduled for elective Caesarean section combined with removal of both adrenal glands. Anaesthesia was carried out using 10 micrograms.kg-1 alfentanil, 5 mg.kg-1 thiopentone, 1.5 mg.kg-1 succinylcholine and 0.5 vol % enflurane. A single hypertensive crisis (190/100 mmHg) occurred intraoperatively, during dissection of the left adrenal gland. This responded well to 1 mg.min-1 phentolamine. The postoperative course was uneventful for both the mother and the child. Total thyroidectomy with block dissection of the lymph nodes was to be carried out within three weeks after the Caesarean section. Only two similar cases of multiple endocrine neoplasia associated with pregnancy have previously been published.

[Cardiopexy using the hepatic ligament in the treatment of gastroesophageal reflux. Apropos of 200 cases].

The authors report 200 cases of cardiopexy with the ligamentum teres (Rampal-Marchal's procedure) associated with a 180 degree posterior fundoplication, in the surgical treatment of gastroesophageal reflux. 200 patients with severe reflux (76% oesophagitis) were operated on with this procedure over a 10 year period. Symptoms of reflux disappeared immediately in 99% cases, with healing of oesophagitis in 124 out of the 127 patients controlled with endoscopy, and a significant increase of inferior sphincteric pressure (from 11 cm H2O to 25 cm H2O). Objective controls by post-prandial pHmetry evidenced persistant reflux with 10 patients, but 9 of them are totally free of symptoms. Operative mortality was 1.5%. Transient dysphagia was observed in 32% cases. All the patients were reviewed with a mean follow up of 23 months. 4 clinical recurrences of reflux were observed (2%) but no oesophagitis was found on endoscopic controls with these 4 patients, and only one had to be reoperated on. Actuarial chance to remain free of recurrence was estimated at 97.8% up to 48 months according to the Kaplan-Maier's method. Cardiopexy with the ligamentum teres ensures the lengthening of the abdominal portion of the oesophagus and anchors the antireflux assembly within the pressure environment of the abdomen in a strong and flexible way. It seems to be the best procedure for the treatment of GE reflux.

[Ultrasound-guided percutaneous drainage of intraperitoneal fluid collections. An experience of a surgical team with 205 patients].

Percutaneous drainage has become, during the last ten years, the treatment of choice of abdominal abscesses. From January 1986 to January 1990, 205 patients have been treated in our department for an abdominal collection. In this prospective work we have realised percutaneous drainage in every patients who would have been operated, according to the classical indications. In 28 patients (acute cholecystitis), the indication has been elective because of the surgical risk, depending to the age (mean 80 years). We have treated 67 intra abdominal collections (34 post operative), 39 pseudocysts of the pancreas, 40 liver collections (18 abscesses); and we have done 37 biliary drainages. 15% of the patients showed communication of the collection with the digestive tract. Mortality was less than 5%. Half of these deaths occurred during the evolution of necrotic pancreatitis. 78% of the patients have been cured by the percutaneous drain. The other patients have been operated upon but the drainage facilitated the operation. In our experience percutaneous drainage is a secure technique and every abdominal surgeon would be able to practice this technique.

Improvement of parathyroid Tl-Tc scintigraphy by using a new image subtraction method.

Forty five thallium-technetium parathyroid scans were performed preoperatively in patients with a high suspicion of parathyroid adenoma or hyperplasia. The patients were imaged after sequential IV injection of 201Tl-thallous chloride (74 MBq) and 99mTc-pertechnetate (222 MBq) using a gamma camera with a pinhole collimator. According to surgical findings, the prevalence of parathyroid abnormalities was 42/45: single (34 patients) and double (1 patient) adenomas, hyperplasia (7 patients, 16 hyperplastic glands). Three methods of analysis of scintigraphic images were compared. Method one was the visual comparison of the thallium and the technetium images. Method two used the subtraction image obtained after normalisation. Method three used a new image comparison method which performs the geometric and gray level registrations of the two images and generates the image of the significant differences. Three observers were involved in the evaluation procedures. Surgery and histological examinations were taken as the gold standard. Results show that the sensitivity is increased by applying method three. The interobserver reproducibility of the results is also higher for method three. We conclude that an adequate image subtraction technique significantly increases the diagnostic value of parathyroid scintigraphy.

[Thallium-technetium scintigraphy for presurgical localization of parathyroid adenomas and hyperplasia].

The purpose of this study was to assess the value of parathyroid Thallium 201-Technetium 99m scintigraphy in 45 patients with hyperplasia or parathyroid adenomas with or without associated thyroid abnormalities. The prevalence of parathyroid abnormalities found during examination was 42/45: 36 adenomas (35 patients) and 16 hyperplasias (7 patients). Seventeen patients (38%) had thyroid abnormalities. The sensitivity and specificity of the three image analysing methods (visual analysis, simple subtraction, sophisticated subtraction) were determined by assessing answers given by 3 observers. The population was divided into 6 groups: adenomas and hyperplasias with (group I, n = 42) or without (group II, n = 25) thyroid abnormalities, adenomas alone with (group III, n = 35) or without thyroid abnormalities (group IV, n = 22), hyperplasias alone with (group V, n = 7) or without thyroid abnormalities (group VI, n = 5). The highest overall sensitivity was obtained, whichever the group, by applying the sophisticated image subtraction technique; for any given method, the best results were observed in the group comprising adenomas alone, especially without associated thyroid abnormalities. On the other hand, in the groups comprising hyperplasias alone, sensitivity was low. This leads us to conclude that, together with ultrasonography, parathyroid scintigraphy is currently the best preoperative technique for localizing parathyroid adenomas, even in the presence of associated thyroid abnormalities. However this must be carried out in conjunction with a sophisticated image subtraction technique.

The inferior alveolar artery in its bony course.

Based on the dissection of 30 hemi-mandibles, the authors report a study of the inferior alveolar artery in its intraosseous course. On morphologic considerations they propose a classification of the collaterals into two groups: the principal collaterals destined for the teeth and the bony alveolar tissue and the secondary collaterals destined for the sheath and the nerve as well as the bony tissue around the canal. Loss of the teeth and absorption of the alveolar bone modify the caliber of the inferior alveolar arterial axis, the distribution of its collaterals and possibly its mode of termination. These facts suggest a consideration of the vascularization of the mandible in terms of four sectors. They arrive at practical conclusions that may be drawn from this study in stomatology.

Anatomical basis of the surgical approach to the medial wall of the orbit.

The main neurovascular structures related to the medial wall of the orbit were studied in 70 orbits after injection of the orbital arterial bed with latex dye. The position, anatomical relations and variations of the ophthalmic and ethmoidal arteries and trochlear and nasociliary nerves are described. Some differences were noted in comparison to classical anatomical descriptions, notably regarding the position of the ophthalmic artery which was commonly found to lie medial to the obliquus superior muscle distal to the point where it crosses under the muscle. An important surgical landmark, the lateral orifice of the anterior ethmoidal canal which lodges the anterior ethmoidal artery, was noted and its position described.

The crura of the diaphragm and diaphragmatic passage. Applications to gastroesophageal reflux, its investigation and treatment.

The authors report an anatomical, morphological and functional study on the diaphragmatic passage of the esophagus. This study allows to conclude that the anatomical structures of the region play an important role in gastroesophageal continence. The idea of "physiological sphincter" must be broadened and take into account the crura of the diaphragm which from a true extrinsic anatomic sphincter. The authors also insist on the dynamic aspect of this region and on the variations of pressure related to the respiratory movements. From a practical standpoint, the conclusions of this study should permit the surgeon to make a dynamic investigation of gastroesophageal reflux and adapt treatment to a more precise physiopathological mechanism.
