The art and science of hair transplantation have come a long way since its introduction in 1959. Both the technique and instruments have been greatly improved, thus offering the patient an excellent cosmetic result that will last a lifetime.
Occlusive dressings reduce healing time by up to 50% over air-exposed wounds. Biobrane (Woodroof, Santa Ana, CA), a biosynthetic wound dressing which is maintenance free, shortens wound healing, and results in less erythema and milia formation, appears to be the wound dressing best suited for dermabrasion.
Improvements in the technique and instrumentation used in hair transplantation have led to the production of better grafts, a more natural hairline, a greater number of grafts from a donor site, the effective control of postoperative bleeding, and the reduction of large areas of baldness prior to hair transplantation.
One of our many roles as dermatologic surgeons is to provide wounds with a biologically favorable environment in which healing can proceed most efficiently. Over the past twenty years research in superficial wound healing has accelerated the introduction of new dressing materials. Various topical agents and occlusive inert dressings, which limit tissue death and maintain hydration, enhance epithelialization. Occlusive dressings will hasten regeneration up to twice that found in air-exposed sites. This characteristic is described in patients who have undergone dermabrasion.
A group of nine patients with severe nodulocystic acne vulgaris were treated with oral 13-cis-retinoic acid and their acne cleared. During or after retinoid therapy, all patients underwent full-face dermabrasion. Postoperative healing was normal, and no significant complications were seen.
Six cases of giant lipomas were treated by liposuction surgical removal. The method involves only a 1-cm incision and insertion of a liposuction cannula. The fibrous stroma of the lipoma may be more difficult to remove. Liposuction is a safe and easy method of removing giant lipomas.
In order to obtain the "perfect" plug, distortion of the skin produced by the punch must be eliminated by reducing the pressure exerted. This can be accomplished by using a power-driven, exquisitely sharp punch and by proper saline infiltration. Other factors are also reviewed.
