Allergic bronchopulmonary candidiasis.

A patient had an illness consistent with allergic bronchopulmonary candidiasis. She had asthma, fleeting pulmonary infiltrate, immediate skin reactivity and precipitating antibody against Candida albicans, elevated total serum IgE concentration, elevated IgE and IgG antibody activity against C albicans, and two positive sputum cultures for C albicans. Serial serologic studies showed a significant decrease of serum IgE levels and IgE antibody activity after corticosteroid treatment.

Staged bilateral thoracotomies for multiple pulmonary arteriovenous malformations complicating hereditary hemorrhagic telangiectasia.

A patient with hereditary hemorrhagic telangiectasia (Osler-Weber-Rendu syndrome) with multiple bilateral pulmonary arteriovenous malformations was hypoxic, cyanotic, polycythemic, and severely limited by dyspnea on exertion. Following staged bilateral thoracotomies, with removal of 23 major fistulas (12 from the right lung and 11 from the left), marked improvement in symptoms, blood gases, and objective measurements of exercise tolerance occurred.

Double-blind comparison of metaproterenol and isoetharine-phenylephrine solutions in intermittent positive pressure breathing in bronchospastic conditions.

A 5 percent solution of metaproterenol sulfate (Alupent) and a fixed-combination solution of isoetharine and phenylephrine were compared in a single-dose double-blind study in a total of 27 patients with reversible bronchospastic disease. The patients were assigned to parallel groups for treatment and received the usual dose of 0.3 ml of metaproterenol and 0.5 ml of the isoetharine-phenylephrine solution via equipment for intermittent positive-pressure breathing. Tests of pulmonary function, blood pressure, and pulse rate were performed before the treatment and at intervals of 30 minutes to six hours after administration. The duration of effect (defined as an increase over baseline in the forced expiratory volume in one second [FEV1] of at least 15 percent) of metaproterenol averaged four hours, as compared with one hour for the reference solution. The overall response of FEV1 to metaproterenol was significantly (P = 0.01) superior to the response to the isoetharine-phenylephrine solution. Metaproterenol also surpassed the reference drug in terms of increases in the mean forced expiratory flow during the middle half of the forced vital capacity (FEF-25-75%) to a degree approaching statistical significance. Changes in blood pressure and pulse rate were clinically insignificant with both drugs, and the total number of adverse experiences was substantially the same with both solutions.

Expired gas monitoring by mass spectrometry in a respiratory intensive care unit.

The application of a medical mass spectrometer for the monitoring of respired gases in the respiratory intensive care unit of a community hospital is reviewed. This monitoring system is routinely used with intubated patients for periodic monitoring of end-tidal CO2 tensions (PETCO2), FIO2, and PETO2 dead space to tidal volume ratios, and the determination of AaDO2; the value of these measurements is discussed. It is especially useful for continuous monitoring at critical points in the patient's course such as weaning from the ventilator, determining optimal ventilator settings, monitoring, unstable nonintubated patients, and in better defining the pathophysiological disturbances impeding patient progress, examples of which are presented. Preliminary observations suggest it may also provide a simple technique for determining optimal expiratory retard settings. The initial cost of such a system is justified by the benefit to the patient, i.e., reduction in the frequency of nonessential arterial blood gas determinations, shortened weaning period, and early detection of potentially dangerous trends. Technical problems encountered with this system and potential future uses are also discussed.
