Optical resolution of 6-fluoro-2-methyl-1,2,3,4-tetrahydroquinoline by supercritical fluid extraction.

6-Fluoro-2-methyl-1,2,3,4-tetrahydroquinoline (FTHQ) enantiomers were separated by supercritical fluid extraction using carbon dioxide. Diastereoisomeric salts were formed from the racemic base with less than one equivalent of O,O'-di-(4-toluoyl)-(2R,3R)-tartaric acid (DPTTA). Further purification was achieved by partial salt formation of the enantiomeric mixture with an achiral acid (HCl) followed by the supercritical fluid extraction of the free enantiomers.

Capillary density of tenotomized skeletal muscles. II. Observations on human muscles after spontaneous rupture of tendon.

Alterations in the capillary density of the M. biceps brachii was studied in 20 cases during 2 months after spontaneous rupture of the tendon. The fiber typing was done using succinic dehydrogenase and myofibrillar ATP-ase at pH 9.4 for the staining, the capillaries were visualized with alkaline phosphatase and PAS reactions, and in each case 1,000 muscle fibers with their capillaries were estimated. In the intact muscles the capillary count ranged from 1,500 to 1,910 (mean 1,740) per 1,000 fibers. During the 1st month following the rupture the decrease in the capillary count of the muscle occurred rapidly, being after 4-7, 8-10, and 18-29 days 36%, 52%, and 66%, respectively, as compared to the contralateral muscles. After 2 months a decrease of approx. 70% was found in the amount of capillaries compared with the contralateral muscles. The obliteration of the capillaries was particularly pronounced in the amount of anastomoses. These results indicate that the capillary supply of the human M. biceps brachii decreases enormously during the 1st month after a tenotomy.

[The role of non-progressive myopathies in the spontaneous rupture of tendons (author's transl)].

A series of non-progressive myopathies have been found (central core disease, centronuclear myopathy, myofibrillar dysproportion) when muscles of patients with spontaneous tendon ruptures were examined. Pathological muscle actions, uneven pulling force on the tendons, disturbances in the nervous feed-back between tendons and muscles and the role of myopathies in the alteration of resting and working muscle tone by affecting the type--I. muscle fibers are thought to be pathogenic factors in the spontaneous tendon ruptures associated with non-progressive myopathies.

[Experimental studies and clinical experiences with homologous tendon grafts preserved with beta-propiolactone].

In dog experiments homologous tendon grafts preserved in beta-propiolactone have been examined by means of light-polarisation microscope and electronmicroscope. The histologic data showed that grafts preserved by beta-propiolactone were completely reorganised within 8 weeks after the transplantation similarly to autologous grafts. This process takes place with very little initial inflammatory reaction, followed by fibroblast reaction. The original collagen fibrous framework is decomposed and new tendon is formed by new collagen fibres on this frame. Cicatrisation was not encountered and graft function was excellent. Functional results in 5 patients were investigated 2 years after replacement of the flexor tendons of the hand and were found to be good in 4 cases, satisfactory in 1 case. The results of animal experiments and the excellent experience with preliminary clinical application of the method suggest that homologous tendon grafts preserved in beta-propiolactone may be used for replacement of injured human flexor tendons of the hand.

[Paget's cancer of male breast].

The case of a male patient with Paget's cancer of the breast is reported. Radical mastectomy was performed. Two and a half years later the patient was free of symptoms.

[Spontaneous tendon ruptures of identified etiology (author's transl)].

The authors found in the material of the National Institute of Traumatology among the 650 cases of spontaneous tendon ruptures seven cases of rupture, when an identified disease localized on the tendon was the precipitating cause. Aetiology of the rupture has been as follows: tendon tuberculosis, rheumatic tendinitis xantoma tuberosum, haemangioma and pseudocyst of the tendon. In two other cases muscle biopsy revealed a concealed myopathy (central core disease and centronuclear myopathy) that may responsible for the rupture of the tendon.

Study of the antitumoral activity of S-carbamoyl-L-cysteine derivatives in animal experiments.

The antitumoral activity of three amino acid derivatives, S-carbamoyl-L-cysteine, S-ethyl-carbamoyl-L-cysteine, and S-chloroethyl-carbamoyl-L-cysteine, was studied. The ethyl and chloroethyl derivatives had a pronounced curative effect on certain animal tumors. It is interesting in the case of S-carbamoyl-L-cysteine that N-ethylization of the substance resulted in increased chemotherapeutic effectivity with no simultaneous change of the toxicity. According to various studies this effect is different in many respects from that of the known cytostatics, and a possibility of a specific selective or surface activity is raised.

[Observations of ultrastructural changes on prepared homologous tendon transplants in humans (author's transl)].

Authors have investigated the ultrastructural morphology of transformation of homologous human tendon transplants. Tenoblasts and mast cells were found next to collagenous fibres originating in the transplant and disintegrating in the voluminous ground substance. The formation of new collagen tissue was of different grades; areas of only a few fibres and areas with oriented fibres were both present. Twenty months after implantation parts characterised by apparently mature structure were found as well as abundant new fibre formation and mast cells.

[Surgical outcome in bilateral tuberculotic hydronephrosis].

The case of a patient is reported in whom pre-operational infusion urography indicated severe hydronephrotic lesions of both kidneys. It was attempted to preserve the kidneys by performing pelviplasty on the one side and ureteral implantation on the other side. Surgery combined with prolonged antituberculous drug treatment ensured full success.

[Urologic involvement caused by pancreatic cyst].

Pancreas-cysts seldom occur in surgical practice and rarely cause urological complications. The history of a patient is presented in whom dislocation of the kidney and compression of the ureter together with pyelectasis have been observed. Operation revealed that the urological symptoms were elicited by a cyst on the pancreas. Attention is drawn to this rare disease.

[Uterine and bladder prolapse causing kidney damage].

A case of severe renal disturbance caused by uterine and bladder prolapse was observed and successfully treated by vaginal operation. Among the surgical indications of vaginal prolapse the lesions of the urinary system must also be considered.
