[Unilateral testicular tumor in adrenogenital syndrome].

In male patients with congenital adrenogenital syndrome (AGS), in addition to the well known adrenal changes, tumor-like testicular proliferations may be observed. They have been known from literature since decades, but have become rare due to modern therapy. Especially in the mild and inapparent cases of AGS, the "tumors" cause clinical and morphological problems. The case of a 34 year old male AGS patient is presented, who underwent resection of a large adrenal cortical adenoma at age 28. Six years later, orchiectomy followed because of a testicular tumor. In frozen sections, the "Leydig cell-like" tumor caused diagnostic difficulties. After usual embedding and examination, the diagnosis of testicular "tumor" in AGS was confirmed. By review of the literature, pathomorphogenesis and morphology of these testicular tumors as well as possible histogenesis of their cells are discussed. Because of the close similarity to Leydig cell tumors, this most important differential diagnosis is especially considered. As a rule, testicular tumors in AGS are no indication for orchiectomy.

[Disorders of bladder emptying following Burch colposuspension].

Forty patients with recurrence urinary stress incontinence (RUSI) were evaluated before and after Burch-colposuspension for voiding difficulties. Thirty-two (80%) patients were cured. Seven females developed an urge incontinence in the postoperative course, one female an urge plus stress incontinence. Residual urine was found in 6 of 7 patients with urge incontinence, in 5 patients a urinary tract infection. Four patients with urge incontinence after colposuspension were insulin dependent diabetics. In 18 patients residual urine was found in the postoperative course, only 6 patients were symptomatic with urge and urinary tract infection. We see a limited indication for Burch colposuspension in RUSI with reduction in vaginal mobility, detrusor hypocontractility, detrusor instability and in diseases predisposing to neurogenic bladder disorders.

[Operative treatment of fractures of the penis (author's transl)].

5 cases of penile fracture are described. 2 patients were referred directly and received immediate surgical treatment. The corpus cavernosum was exposed and the tunica albuginea repaired with absorbable sutures. The other 3 patients were referred from 10 to 19 days after the initial injury. On admission 2 of the were experiencing deviation of the erect penis, one also had painful erection. They too were explored and the tunica albuginea sutured to correct the deformity. All 5 patients were treated post-operatively with a broadspectrum antibiotic, an anti-inflammatory agent and tranquilizers to prevent erections. Except for one patient with delayed wound healing there were no complications of the operation. The long-term results were good, all patients experiencing satisfactory straight erections.

[Measurement of urethral closure pressure profiles with stress performed by CO2-infusion in stressincontinent women: sources of error in methodology and modes of clinical application (author's transl)].

The measurement of urethral closure pressure with stress is limited by the compressibility of gasiform mediums, which causes damping of intraurethral pressure spikes. In this study damping could be reduced by alterations of the catheter and the connecting tubes of an aircystometer. With this modified technique 55 incontinent women were examined. The differentiation between stress- and urgeincontinence was clinically sufficient.

[Clinical problems in urinary obstruction (author's transl)].

It is difficult to define and diagnose urinary obstruction in a meaningful way only by radiographic methods and without involving a urodynamic concept. Urinary infection, urolithiasis and elevated arterial blood pressure are the most common complications found with urinary obstruction. In 70 patients with obstruction of the pelviureteral junction we found infection in 15.7 percent, lithiasis in 27 percent, and arterial hypertension in 27 percent.

Analysis of bladder configuration and pressure from pressoreceptor stimulation in rabbits.

In a previous study (Bödeker, J., Vogt, W., Kölln, C.-P., and Nagel, R.: Invest. Urol., 12: 461, 1975.) pressoreceptor stimulation induced micturition at less than normal bladder filling. Furthermore, urethral pressure profile decreased in both maximal and mean values. In the present investigation the effects of pressoreceptor stimulation on bladder configuration, intravesical pressure, and vesicoureteral reflux were studied. With pressoreceptor stimulation, the bladder outlet became more funnel-shaped; the change in bladder configuration occurred without alteration in intravesical pressure; and vesicoureteral reflux could not be demonstrated.

[Intravesical and intraurethral pressure studies: significance for the prognosis and the treatment of children with myelomeningocele (author's transl)].

By statistical analysis of premicturition bladder pressure and urethral pressure in 37 children with myelomeningocele we were able to find four different types of neuropathic bladder dysfunction: 1. Group A (N = 19): No upper urinary tract dilatation and deterioration was seen with normal intravesical and low to normal intraurethral pressure. A conservative approach was warranted. 2. Group B (n = 6): Overflow incontinence due to low intravesical and high intraurethral pressure was found in this group. Upper urinary tract deterioration could be avoided and continence achieved by partial sphincterotomy. 3. Group C (n = 6): Progressive impairment in upper urinary tract dilatation was observed in four out of six children with high intravesical and low intraurethral pressures. In these children we prefer the urinary diversion. 4. Group D (n = 6): A dangerous impairment in upper urinary tract dilatation was found in four out of six children with high intravesical und high intraurethral pressures. When phenoxybenzamin therapy fails urinary diversion is the treatment of choice in this group.

Effects of pressoreceptor stimulation on micturition relfex and urethral pressure profile in rabbits.

The influence of pressoreceptor stimulation on the micturition reflex and urethral pressure profile was studied. Stimulation of pressoreceptors of the aortic arch and carotid sinus causes an increase of parasympathetic tone and a decrease of sympathetic tone. Striated muscular tone was also decreased. Pressoreceptor stimulation was simulated by electrical stimulation of the depressor nerve in the rabbit. Our studies show that with pressoreceptor stimulation micturition occurred with 24 per cent less bladder filling and 32 per cent less intravesical pressure than before stimulation, and urethral pressure profile showed a 17 per cent decrease of the maximum and mean urethral pressure. The interpretation of the action of parasympathicomimetic and sympathicomimetic drugs on bladder and bladder neck must take into consideration the cardiovascular effects of these drugs and therefore the effect on the pressoreceptors.

[The congenital urethral diverticulum of the Male Urethra of adults].

On 4 male adults we demonstrated a congenital urethral diverticulum. The clinical symptomatology mainly showed the inflammatory micturition difficulties, while the obstructive micturtition symptomatology of the childhood was hardly registered by the patients. Injection and micturition urethrogram led to the diagnosis. The mostly additional obstructive effect of a diverticulum can be well diagnosed by urodynamic investigations (cystourethromanometry and uroflowmetry). We used the following treatment: removal of the membrane between diverticulum and urethra and resection of the urethral diverticulum with primary urethral obstruction. The advantages of an open approach in comparison to endoscopical therapy were pointed out.
