HIV-associated lymphoma: histopathology and association with Epstein-Barr virus genome related to clinical, immunological and prognostic features.

All 51 cases of HIV-related malignant lymphoma in Denmark diagnosed from 1983 to 1989 were reviewed. There were 12 Burkitt-type lymphomas, 30 immunoblast-rich lymphomas and 9 other lymphomas. Patients with immunoblast-rich lymphomas had significantly lower CD4 cell counts (median 60 vs. 188 x 10(6)/l, P less than 0.05), and more often a history of previous AIDS-defining illnesses (50% vs. 0%, P less than 0.005), compared with patients with Burkitt-type lymphomas. Epstein-Barr virus (EBV) DNA was demonstrated in 14 of 19 immunoblast-rich tumours, and in 2 of 7 Burkitt-type lymphomas (P = 0.10). Compared with EBV DNA-negative tumours EBV DNA-positive tumours were associated with lower CD4 cell counts (median 39 vs. 188 x 10(6)/l, P = 0.01). It is concluded that two main types of HIV-related malignant lymphoma exist. One is associated with severe immunosuppression, is often of immunoblast-rich morphology, and may be linked to EBV, whereas the other may occur in the absence of immunosuppression, is often of Burkitt-type morphology, and is probably not linked to EBV. In addition to these two main types, other non-Hodgkin lymphomas and Hodgkin's disease do occur.

[Knowledge of AIDS and of transmission risks of HIV infection in a group of homosexual people. A questionnaire study].

A questionnaire investigation among 430 homosexual persons all of whom were members of an organization for homosexuals was completed by 144 (33%). These replies showed that they knew about HIV infection, risk behaviour and "safe sex". Their knowledge about the signs of HIV infection and AIDS was, however, more limited. The replies were compared with an age-matched group of the male population. The results of the investigation provide reasons to suppose that the information campaigns have been of value but information to prevent spread of infection is still necessary. In addition, further information is necessary about the subjective signs of HIV-infection so that persons in whom these signs develop may be offered the best possible treatment as early as possible.

[A group of homosexuals change their sex habits after information about AIDS. A questionnaire survey].

A questionnaire investigation to which 144 homosexual persons replied anonomously revealed alterations in the sexual habits after information about AIDS. Significant reduction in the annual number of partners and significantly fewer employ the more dangerous sexual practices which now occur particularly in more permanent partnerships are among the alterations started. An unchanged number still employ active anal and orogenital coitus. Employment of condoms has increased significantly, particularly in anal coitus and "casual" partners (from 3% to 82%). The majority accept the use of condoms and state that they employ more "safe sex" than prior to information about AIDS. 12% stated that they had sex with both sexes and the possibility of spread of infection from homosexual to heterosexual groups is present. The intensive informative work from the homosexuals own organisation and from public health authorities appear to have had some effect but further information and influencing are necessary if the spread of HIV infection is to be stopped.

Prevalence of vasospastic ischaemia induced by the cold pressor test or hyperventilation in patients with severe angina.

We performed coronary artery spasm provocation, using the cold pressor test (CPT) and hyperventilation (HVT) in 105 patients (87 males, 18 females) with mean age 51.9 years (range 25-69) consecutively admitted for coronary angiography due to attacks of chest pain. A positive response to spasm provocation (ST segment elevation or depression greater than or equal to 0.1 mV or pseudonormalization of negative T-waves) was seen in 25 patients (23.9%) (group A), with 8 patients responding to CPT and 23 to HVT. In the remaining patients (group B) a negative response was found. Of 80 patients with coronary artery stenosis (diameter reduction greater than 50%), 22 (27.5%) showed a positive response. During CPT and HVT the rate pressure product increased 25% and 16%, respectively in group A versus an increase of 139% during exercise testing in the same patients. This suggests that ischaemia induced by CPT or HVT is not caused by increased myocardial oxygen demand. Repeat spasm provocation was performed during coronary angiography in 14 patients from group A and 14 from group B. The induced reduction in the diameters of the ischaemia related vessels was on average 48.3% in group A versus a 9.9% reduction of the left anterior descending artery in the patients from group B (P less than 0.01). In group A 88% had a history of nocturnal angina versus 38.8% in group B (P less than 0.001). A positive exercise test was found in 87% and 35.6% in group's A and B respectively (P less than 0.0005).

Pulmonary embolectomy using extracorporeal circulation.

Experience with 23 patients treated surgically, with the use of cardiopulmonary bypass, for massive pulmonary embolism is presented. Seventy-four percent survived surgery. Of the survivors all but 3 were fully recovered after observation times ranging from 3 months up to 7 years. The patient category presents the basis for our contemporary indications for surgery, i.e. patients in fulminant shock, in deteriorating clinical conditions or with contraindications for thrombolysis. In these patients surgical intervention is recommended with the use of the heart lung machine.
