[Gammagraphy of somatostatin receptors in an ACTH secreting thymic carcinoid].

Ectopic ACTH secretion represent 10 to 15% in the causes of Cushing's syndrome. Carcinoid tumor of the thymus is a rare neoplasm and is currently believed to arise from population of thymic cells of neural crest origin. We present the case of a 43-year-old man with a corticotropin hormone (ACTH) secreting thymus carcinoid. 111In-octreotide scintigraphy demonstrated an intense uptaken on the lesion. The surgical specimen measured 17 x 18 x 18 cm and weighed 1. 25 kg. Postoperatively the patient was treated by chemotherapy and radiotherapy. A follow-up 111In-octreotide scintigraphy was normal. Very few scintigraphic images of these lesions have been published in the literature. Somatostatin receptor scintigraphy can be successful in the detection and follow-up of ACTH-secreting neuroendocrine tumors.

[131I-treatment of the autonomous toxic nodule. Assessment of a fixed dose protocol].

Ninety six patients diagnosed of toxic thyroid adenomas received fixed doses of 131I (555MBq) for treatment of hyperthyroidism. The inclusion and exclusion criteria, the patient's characteristics and the advantages of the fixed doses are described. The hypothyroidism rate found at the end of the mean follow-up period (2.46 +/- 1.72 years) which was 27.08% significantly lower to that observed in patients treated for Graves-Basedow disease, is pointed out. No differences were found in the hypothyroidism rate when the patients were differentiated by sex or when this situation was related to pretreatment with the antithyroid drugs. The extensive differences in the hypothyroidism rates observed between the different series that have been published and the variability in the doses administered leads us to think that factors other than the dose of 131I may be responsible for this post-therapeutic hypothyroidism.

[Thoracic SPECT using (111 In-DTPA-D-Phe1)-octreotide in bronchial carcinoid syndrome].

We describe two cases of bronchial carcinoid tumour detected by (111In-DTPA-D-Phe1)-octreotide scintigraphy. The SPECT of the thoratic region provided a better images of the primary tumour than the planar images. In one of the patients, the bronchial carcinoid tumour was associated with sarcoidosis, which led to an erroneous evaluation of the extension of the tumour.

[Gammography using Tc 99m MIBI in patients with suspected breast cancer. Proposed diagnostic protocol].

The aim of this work has been to determine whether the joint use of mammography and scintimammography is capable of reducing the number of biopsies required in patients with suspected breast cancer. We have performed scintimammography in 90 patients, 97 lesions, with breast cancer suspicion. In the mammography was evaluated the degree of malignancy suspicion and the size of the lesion. Only 41 of the biopsies revealed the presence of cancer. The sensitivity, specificity, PPV and NPV of scintimammography were 85%, 79%, 74% and 88%. According to the mammography findings 20 lesions (1 breast cancer) were included as low, 31 lesions (4 breast cancer) as indeterminate and 46 lesions (36 breast cancer) as high malignancy suspicion. 14 lesions (2 low, 2 indeterminate and 10 high suspicion) were smaller than 1 cm. The scintimammography was positive in all breast cancer of low and indeterminate suspicion of malignancy and in 30 of high probability. On the basis of this results we propose that a biopsy must be carried out to those lesions with a high suspicion of malignancy, and to lesions with low or indeterminate suspicion that are smaller than 1 cm or that present a positive scintimammography. Following this protocol, only 64 of the 97 biopsies would have been necessary, with a reduction of the 34% in the total number of biopsies and, more important, a 65% of reduction in the number of biopsies carried out in the groups of low and indeterminate suspicion of malignancy. At the same time we would indicate biopsy in all cases of breast cancer.

[A Spanish multicenter scintigraphic study of the breast using Tc 99m MIBI. Report of results].

UNLABELLED: The aim of this work has been to study, with a homogenous method, a wide sample of patients with breast cancer suspicion, in order to evaluate the real clinical usefulness of the scintimammography with MIBI-99mTc. METHODS AND MATERIAL: We have studied by way of prone scintimammography 388 patients (418 lesions) with breast cancer suspicion. In the mammography the size and degree of breast cancer suspicion was evaluated. Diagnosis was established by biopsy. RESULTS: 247 lesions were diagnosed as breast cancer and 171 as benign diseases. She results of the scintimammography were sensitivity 90%, specificity 70%, PPV 81% and NPV 83%. In palpable lesions sensitivity and specificity were 94% and 61%, and in non-palpable lesions 75% and 81% respectively. In 264 lesions mammographic breast cancer suspicion was establish. 13 of the 14 FN results were obtained in lesions with a high malignancy suspicion. In lesions with a low or indeterminate suspicion, sensitivity and NPV of the scintimammography were 97% and 98%. CONCLUSIONS: Scintimammography has a high sensitivity but a relatively low specificity in the study of lesions with breast cancer suspicion. Scintimammography could be a good complement of the mammography in lesions with a low or indeterminate suspicion of malignancy.

[Scintigraphic assessment of the severity of inflammatory bowel disease using Tc 99m exametazime-labeled leukocytes].

The aim of the work was to value the utility of semiquantitative scintigraphics indexes (GI) as predictive factor of activity grade of intestinal inflammatory process in patients with confirmed diagnosis of inflammatory bowel disease (IBD). 64 patients have been studied (39 males and 25 females), mean age was 40,2 years (range: 18-88 years) with diagnosis of IBD in acute exacerbation confirmed by endoscopy and biopsy. In all patients was obtained abdominal scintigraphy with 99mTc-exametazine labelled leukocytes. The severity of intestinal inflammatory process was quantified using clinical indexes -Truelove-Witts index (TI) for ulcerative colitis (UC) and Crohn's disease activity index (CDAI), Harvey index (HI) for Crohn disease (CD)-, endoscopic index (EI) and haematological parameters: erythrocyte sedimentation rate (ESR), hematocrit, haemoglobin, leukocyte and platelet count, fibrinogen and albumin level. In all the scans regions of interest were drawn over liver, spleen, bone, abdominal background and positive intestinal segments. Subsequently three GI were calculated: GI1, GI2 and GI3 for the scans obtained at 30 min (P) and 3 h p.i. (T). The evaluation of UC severity showed significant correlation between: 1) The GI1 P and T and TI, EI, ESR, leukocyte and platelet count; 2) The GI2 P and TI and EI; 3) The IG2 T and EI, hematocrit and platelet count; 4) The GI3 P and TI, EI, ESR and platelet count; 5) The GI3 T and TI, EI, leukocytes and platelet count. In the evaluation of CD was observed significant correlation between the GI1 P and GI3 P with the CDAI and EI, between the GI1 T and the GI3 T with the CDAI and ESR; the GI2 did not show significant correlation with any of the valorised parameters. In conclusion, the scintigraphic activity indexes based in the semiquantitative evaluation of scans obtained with 99mTc-exametazine labelled leukocytes showed usefulness in the evaluation of the IBD severity, especially in the UC. Among the studied indexes, IG1 seems the more accurate.

The diagnostic value of Tc-99m human polyclonal immunoglobulin imaging compared to Tc-99m HMPAO labeled leukocytes in inflammatory bowel disease.

The aim of this study was to establish the usefulness of Tc-99m human polyclonal immunoglobulin (HIG) imaging in a group of patients who were suspected of having inflammatory bowel disease (IBD). The authors performed 30 scans (15 with Tc-99m HMPAO leukocytes and 15 with Tc-99m HIG) on 15 patients with IBD. Ten patients had Crohn's disease and five had ulcerative colitis. The sensitivity of Tc-99m HIG scintigraphy for detecting IBD was 33%, while the sensitivity of Tc-99m HMPAO leukocyte imaging was 100%. The Tc-99m HMPAO leukocyte imaging also detected a larger number of affected segments and provided better image quality of the extent of disease than Tc-99m HIG. On the basis of these results, the authors believe that Tc-99m HIG imaging is not a useful technique in the evaluation of patients with IBD, in the identification of location, or the extension or degree of disease activity.

[Somatostatin receptors in meningioma: diagnostic and therapeutic value].

OBJECTIVE: We discuss the diagnostic and therapeutic usefulness of the presence of somatostatin receptors (SR) in meningiomas. DEVELOPMENT: The SR are membrane receptors, may be saturated, have great affinity and pharmacological specificity for somatostatin (SS) and its analogues. SR have been found in 100% of the meningiomas studied. The presence of a large number of SR in different tumours permits gammagraphic detection in vivo by marking SS analogues with 1-231-DTPA and subsequent visualization using a gamma camera. This study may help to differentiate between meningiomias and other tumour lines. Experiments using different SS analogues have shown different degrees of affinity of these SS analogues for the different receptors. In various animal models a potent inhibitor effect on tumour growth has been seen following SS and its analogues. Unlike in hypophyseal adenomas and endocrine gastro-entero-pancreatic tumours, neither SS nor its analogues have shown any direct inhibitory action on the growth of meningioma cell cultures. Quite the opposite occurs. A small but significant stimulation of growth was seen in the presence of SS. CONCLUSIONS: We consider the detection of SR by isotopic marking of SS analogues is useful in the differential diagnosis of meningiomas. The therapeutic usefulness of the analogues of SS for treatment of meningiomas remains controversial. Further study of the different types of receptors and analogues is necessary.

[The use of SPECT with 201Tl in the evaluation of brain tumors].

There is increasing interest in the literature regarding the use of SPECT (single photon emission computed tomography) with 201Tl in the evaluation of cerebral tumours. Thallium (201Tl) is one of the isotopes most commonly used in studies of the myocardium and in the diagnosis of various tumours. Normal brain takes up very little 201Tl, but in viable tumour tissue the 201Tl becomes intracellular and is rapidly cleared from the blood stream. Such cerebral tumour uptake is related to changes in the permeability of the blood-brain barrier, regional blood flow and transport by means of the Na(+)-K+ adenosine triphosphate pump. Once the SPECT images have been obtained following administration of the radioisotope, one can calculate the indices of early and of late uptake together with the retention index. The usefulness of SPECT in the preoperative detection of malignant expansive pathology, choice of tumour region during stereotaxic biopsy, control of tumour reproduction and their differentiation from post-radiotherapy necrotic tissue, and evaluation of chemotherapy in treatment of cerebral tumours are analyzed. Finally the application of SPECT in metastatic conditions and the differential diagnosis of expansive lesions is considered.

Diagnosis of prosthetic vascular graft infection by technetium-99m-HMPAO-labeled leukocytes.

UNLABELLED: The purpose of this study was to determine the usefulness of 99mTc-HMPAO-labeled leukocyte scans in the diagnosis of prosthetic vascular graft infection. METHODS: We performed 75 scans in 61 patients with vascular grafts. Thirty-six patients were evaluated for suspected infection and 25 were control patients. Scintigraphic images were performed at 5 min, 30 min, 3 hr and, occasionally, 24 hr. Persistent increased uptake at 3 hr along the suspected area of the graft was considered evidence of graft infection. RESULTS: All 20 infected grafts were detected with 99mTc-HMPAO leukocyte scan. The sensitivity and specificity of the scan in the detection of infected graft were 100%. We also detected two pelvic abscesses, two infected fistulae, two soft-tissue infections, three cases of ischemic colitis, one acute diverticulitis, one infected hematoma, one septic arthritis and one noninfected hematoma. One patient with a superficial groin infection had a negative scan. The eight pseudoaneurysms did not show scintigraphic evidence of graft infection. Correlative CT studies were performed in 12 cases. CONCLUSION: Technetium-99m-HMPAO-labeled leukocyte scan is an accurate and valuable diagnostic method for evaluation of suspected prosthetic vascular graft infection.

[Gastric emptying of solids, acid secretion and tobacco in duodenal ulcer].

We study in a group of patients with endoscopically diagnosed duodenal ulcer (19; 17 males) and controls (11; 7 males) the gastric emptying of solids through scintigraphy and gastric acid secretion by standard tests. In the same way we investigated prospectively some clinical data, specially smoking habits. As a whole, patients with duodenal ulcer showed an emptying of solids slightly faster than controls (T 1/2-minutes-: 85.4 +/- 28.6 in patients with duodenal ulcer versus 116.9 +/- 46.5 in controls, p less than 0.03). However, most of our patients (15 of 19 or 79%) were found to have a normal emptying rate. No correlation was found between secretory outputs and gastric emptying. Smokers with duodenal ulcer had a faster emptying that non-smokers with duodenal ulcer (T 1/2 74.8 +/- 30.05 vs. 99.91 +/- 19.86; p = 0.05).

[Gammagraphy with labeled leukocytes as an activity and extension index of Crohn disease].

The knowledge of the location and extent of bowel involment and disease activity can be an important aspect on the evaluation of inflammatory bowel disease. Thirty five patients of Crohn's disease (CD) have been studied in this sense. We performed a total of 44 scintigraphic examinations using 99mTc-HMPAO labeled leukocytes and the results were compared with several clinical-biological indexes and with radiology and endoscopy. The Tc scan was the most sensitive (84.3%) and specific (100%) activity index and showed an excellent correlation with radiology and/or endoscopy in the assessment of extent of disease. Leukocyte scanning brought useful information for the management of stenosis in CD.

[Gammagraphy with sucralfate labelled with technetium in esophagitis].

In previous studies it has been reported that, after being labeled with technetium, sucralfate, an useful drug in peptic diseases, can be used to detect peptic lesions of the digestive tract. In this work we report our experience with this technique in the diagnosis of esophagitis. 25 studies (11 controls and 14 patients) were undertaken. Sucralfate scintigraphy was normal in the 11 control studies, and abnormal in 10 out of 14 patients. Scintigraphy was abnormal in peptic as well as caustic lesions.
