Seroprevalence of measles, rubella, and mumps antibodies in Catalonia, Spain: results of a cross-sectional study.

Determination of antibody levels against vaccine-preventable diseases is of great value to assess immunization programmes. The objective of this study was to determine the prevalence of measles, rubella, and mumps antibodies in representative samples of the child and adult population of Catalonia and compare the findings to those obtained in 1996. A representative sample of the child and adult (>or=15 years) population of Catalonia was studied. Enzyme-linked immunosorbent assay techniques were used to determine the presence of antibodies. Equivocal results for antibodies against measles and rubella were tested using an immunofluorescence technique. To compare proportions, the chi-square test and the Fisher's exact test were used. Statistical significance was set at 0.05. Adjusted odds ratios were calculated using multiple logistic regression analysis. Samples from 2,619 people were analyzed. The global prevalence of antibodies was 98.3% for measles, 91.1% for mumps, and 98.1% for rubella. The prevalence of rubella antibodies was higher in women than in men (98.8 vs. 97.2%, respectively). Compared with the results obtained in the 1996 seroprevalence study, only the prevalence of rubella antibodies showed a statistically significant increase in men (97.2 vs. 94.6%; p=0.002) and, in particular, in women (98.8 vs. 95.3%; p<0.001). The low prevalence of susceptible subjects has already led to the elimination of indigenous measles in Catalonia and should allow the elimination of indigenous rubella by 2005. The level of antibodies necessary to interrupt the transmission of mumps has still not been reached in all age groups.

Dramatic decline in acute hepatitis B infection and disease incidence rates among adolescents and young people after 12 years of a mass hepatitis B vaccination programme of pre-adolescents in the schools of Catalonia (Spain).

The aim of the study was to describe the impact of hepatitis B vaccination and disease incidence in adolescents and young people 12 years after the launching of a mass hepatitis B vaccination of pre-adolescents in schools. Vaccination coverage was assessed using administrative and serological data. Infection trends were evaluated by means of seroepidemiological surveys. High levels of vaccination coverage and vaccine-induced immunity were achieved. The resulting low proportions of susceptible adolescents and young people have undoubtedly contributed to the substantial reduction in the prevalence of hepatitis B infection in the 15-24 years age group (0.9 per 100 in 2001 versus 9.3 per 100 in 1986) and in the reported incidence of hepatitis B cases (80% reduction). Over the last 3 years, the declining trend seems to have been halted, although 35% of cases reported during this period corresponded to immigrants.

Immunogenicity and reactogenicity of the adult tetanus-diphtheria vaccine. How many doses are necessary?

The immunogenicity and reactogenicity of the tetanus-diphtheria adult type vaccine was compared in two groups: group I (n = 201, 18-30 years old, presumably vaccinated with the DTP vaccine) and group II (n = 147, > or = 45 years old, without vaccination antecedents). Before vaccination, the seroprotection levels for tetanus were 90.5% (group I) and 30.6% (group II). These rose to 99.5% and 81.7%, respectively, after administration of one vaccine dose. For diphtheria, prevaccination seroprotection levels were 38.3% (group I) and 19.0% (group II). These rose to 85.8% and 65.7%, respectively, after vaccination. The logistic regression analysis showed an association between antibody titre and age. In group II, 3 doses of Td vaccine were needed to reach titres similar to those achieved in group I with a single dose. Stated reactogenicity was greater in: young subjects, women, those with higher titres of tetanus antibodies and those receiving other vaccines simultaneously. These results confirm the need for vaccination schedules adapted to the characteristics of each population age-group.

[Changes in seroepidemiology of hepatitis A virus infection in Catalonia in the period 1989-1996. Implications for new vaccination strategy].

BACKGROUND: The aim of this study was to investigate the prevalence of the risk factors of hepatitis A virus infection (HAV) in a representative sample of a Catalonian population obtained from 1995 to 1996 and the changes in the prevalence of this infection over the period of 1989-1996. SUBJECTS AND METHODS: The prevalence of anti-HAV was determined by an ELISA test in a randomized sample of 2,142 individuals, 884 from 6 to 14 years of age and 1,248 over the age of 15 years. The results were related to sociodemographic variables and multiple logistic regression analysis was performed to establish which variables were related to the risk of infection. RESULTS: The global prevalence of HAV infection was 67.8%. The prevalence of HAV infection increased from 3.5% in the group from 5-14 years of age to 99% in that over the age of 64 years (p < 0.001). A higher prevalence was observed in those born outside of Catalonia (odds ratio [OR] = 3.97; 95% CI, 2.4-6.4) and in those with a lower level of education (OR = 2.60; 95% CI, 1.9-3.5). In the period 1989-1996 the prevalence of the infection has decreased in the population under the age of 45, the differences being statistically significant in the age groups 10-14 (p < 0.0001) and 25-34 (p < 0.0001). CONCLUSIONS: The prevalence of HAV infection has progressively decreased in Catalonia while it proportionally increases the susceptible population under the age of 45 years. These findings may be important in the design of strategies for the prevention of HAV infection with universal vaccination programs against this disease.

Measles immunity and vaccination policy in Catalonia.

The prevalence of antibodies against measles in a representative sample of the school population in Catalonia was established and the results compared with previous studies. The study was carried out in 1996 using blood samples obtained from schoolchildren aged 6-7 years, 10-11 years, 13 14 years and 15-16 years. 1231 schoolchildren were studied. The global prevalence of antibodies was 96.3%, and a considerable increase was observed with respect to the prevalence in 1986 (89.4%). The level of antibodies was 94% at 6-7 years and 10-11 years, 99.7% at 13-14 years and 98.3% at 15 16 years. These data would suggest the advancement of the administration of the second dose of measles-mumps rubella vaccine to 4 6 years instead of at 11 years.

[Comparative study of human cutaneous sensitivity to 2 and 5 international units of the PPD RT 23 tuberculin with Tween 80].

BACKGROUND: The aim of this study was to know whether the human reaction to the Mantoux test with 5 UT of PPD RT 23 is similar to that achieved with the same test but with 2 UT of the same substance. METHODS: A simultaneous double Mantoux test was used at doses of 2 UT and another of 5 UT of the same trade mark of tuberculin PPD RT 23 with Tween 80 in 2,575 individuals. The subjects were distributed into two different groups of different ages, taking into consideration the possible presence of BCG vaccination. RESULTS: The doses of 5 UT produced a greater number of tuberculin positive individuals and a greater diameter of induration of reaction than that of the doses of 2 UT in both groups and with significant statistical differences. One hundred ninety-six (7%) subjects were tuberculin positive with 5 UT and negative with 2 UT. CONCLUSIONS: The reactions produced by the Mantoux test with 5 and 5 UT PPD RT were different since the two doses are not bioequivalent.

Seroepidemiological survey of hepatitis B infection in a cohort of military recruits in Barcelona, Spain.

The prevalence of HBsAg and anti-HBs in a representative sample of 839 recruits from a cohort of 17-18 year-old males living in Barcelona has been determined by radioimmunoassay of eluates of finger-prick dried blood. It was 0.48% for HBsAg, and 3.22% for anti-HBs. All carriers of HBsAg were anti-HBe positive and had normal liver function tests. Half of them had antecedents suggestive of a materno-fetal acquisition of the infection. A relationship between educational level and the risk of infection was also demonstrated.

[Lipomatous hypertrophy of the interatrial septum. Report of 2 cases diagnosed in vivo].

Lipomatous hypertrophy of intervenous tubercle is a clinicopathological entity characterized by an accumulation of not capsulated fatty tissue, with its thickness exceeding 15 mm, in the atrial septum, above the fossa ovalis, at the intervenous level and protruding into the right atrium. It can be clinically associated with atrial electric abnormalities such as disorders of the atrial conduction and supraventricular arrhythmias, difficulty of the venous return and sudden death. Nowadays its diagnosis is not difficult using non invasive image techniques. We present two cases of lipomatous hypertrophy of intervenous tubercle which were suspected by echocardiography. In one of them we used transthoracic echocardiography and in the other one transesophageal echocardiography. The diagnostic was latter confirmed by magnetic nuclear resonance and axial computerized tomography.

Influence of treatment with zidovudine (ZDV) on the long-term survival of AIDS patients.

The influence of treatment with zidovudine (ZDV) and other factors on long-term survival of AIDS patients was analyzed in a cohort of 629 adults. A total of 434 (69%) were diagnosed before ZDV became routinely available in Spain (December 1987) or refused the drug, while the remaining 195 (31%) received ZDV (starting at a dose of 750-1,200 mg/day). A total of 412 (65.5%) were parenteral drug addicts and 217 (34.5%) male homosexuals. Two hundred thirty-two (36.9%) presented with a tuberculosis, 303 (48.2%) with other opportunistic infections, 69 (11%) with Kaposi's sarcoma, and the remaining 25 (4%) with a lymphoma. By December 1990, 251 (39.9%) of the 629 have already died with a cumulative survival probability of 50.6% after 2 years (45.3-55.9%; 95% confidence interval). When patients receiving ZDV were compared with those untreated, the estimated survival probability was significantly (p less than 0.0001) higher (89% vs. 59% after 1 year, 69% vs. 48% after 2 years, and 55% vs. 40% after 3 years). Moreover, treatment with ZDV (p less than 0.0001) together with being less than 45 years old (p less than 0.0001), being a parenteral drug addict (p = 0.016), and presenting with tuberculosis (p less than 0.0001) were the factors selected by the multivariate analysis as independently improving the prognosis. In conclusion, adult AIDS patients (homosexual or drug addicts) may benefit from treatment with ZDV, at least during 3 years.

Impact of tuberculosis on the new AIDS definition in Barcelona [corrected].

The impact of the new AIDS definition on tuberculosis in Barcelona is evaluated. In 1988 active epidemiological surveillance demonstrated a tuberculosis incidence rate of 58.31/100,000 and an AIDS incidence rate of 9.93/100,000 according to the old definition, and 14.16/100,000 according to the CDCs' new definition (42.5% increase). The majority of these new cases are HIV-positive drug abusers with extrapulmonary tuberculosis. Pulmonary or extra-pulmonary tuberculosis is also observed more frequently in drug abusers than in homosexuals (odds ratio = 7.4; CL 95%; 3.5-15.5). Up until 1986, 39% of AIDS cases were homosexuals and 36% drug abusers. In 1988, according to the old AIDS definition, 54.2% of AIDS cases were drug abusers and 31.9% were male homosexuals but with the new definition it is even more striking that the high risk group is still drug abusers. Today we must add to the old social problem of tuberculosis the problem of drug abusers, and all the difficulties of compliance with chemotherapy or antituberculous chemoprophylaxis. New strategies will have to be devised to deal with this new problem.

[Malaria. Analysis of 149 cases (1981-1987)].

The epidemiological features of the 149 cases of malaria reported to the Institut Municipal de la Salut in Barcelona from 1981 to 1987 are reported. The yearly evolution of the number of cases showed a tendency to increase predominating in the summer months. The incidence rate for 1977 was 1.99/100,000. The districts with the highest rate of involvement were districts I (Ciutat Vella), owing to the immigration, and V (Sarrià-Sant Gervasi), probably owing to overseas visitors. There was a remarkable predominance of males (63.8%), patients aged 20-40 years, and cases of African origin (81.81%); 50.42% of these came from Equatorial Guinea. The most commonly isolated plasmodium was P. falciparum (52.53%), with a progressive increase throughout the years. Only 13.43% of patients had followed a correct chemoprophylaxis. Malaria is still the most important protozoan infection in the world. In the present study its possible reintroduction in Spain is discussed. The malarial endemic used to be very important in this country, and its eradication was not achieved until 1964.

[Isolated congenital diverticulum of the left ventricle].

Congenital diverticulum of the left ventricle is an uncommon cardiac malformation. We present a 45 years old woman, previously asymptomatic. The symptoms and electrocardiogramas suggested ischemic heart disease. The left ventriculogram demonstrated an isolated apical diverticulum, although the coronary arteries were normal. We will consider the etiopathogeny, classification, clinical history, diagnosis and treatment of the isolated apical diverticulum.

[A community outbreak of Legionnaires' disease in Barcelona: epidemiologic and environmental study].

The investigation of the outbreak of legionellosis which developed in Barcelona in February 1988 included: 1) a case-control study, with the patients with Legionella pneumophila pneumonia (n = 56) and those with pneumonia by other organisms (n = 109) in the Hospital Clínic i Provincial (HCP); 2) a review of refrigeration towers and the water supply system; 3) a meteorological study, and 4) a survey of the construction works being carried out in the HCP area. The cases lived in the urban district 2 more often than the controls (odds ratio = 3.73, 95% confidence intervals 1.8-7.6), and were not associated with hospitals of hotels. L. pneumophila was not recovered from the environmental samples. The temperatures were unusually high, with a low air humidity. In the area near the HCP demolition materials were frequently removed. A causative focus was not detected; it cannot be ruled out, however, that the source of the outbreak was the removal of demolition materials in the affected area on the days preceding the epidemic outbreak.

Predictors of the survival of AIDS cases in Barcelona, Spain.

Factors influencing the outcome of disease were analysed in 289 adults presenting with AIDS in Barcelona, Spain from January 1986 (31 cases) to December 1987 (258 cases). One hundred and fifty-four (53.3%) were parenteral drug addicts and 100 (34.6%) were male homosexuals. Ninety-six (33.2%) presented with tuberculosis, 148 (51.2%), with other opportunistic infections, 34 (11.7%) with Kaposi's sarcoma, and the remaining 11 with a lymphoma. By February 1988, 144 (49.8%) of the 289 had died, with an actuarial survival probability of 46.7% at 2 years (40.7%-52.7%, 95% confidence interval). The factors selected by the multivariate analysis as independently worsening the prognosis were: having been diagnosed as having AIDS before 1986, being more than 45 years old, not being a parenteral drug addict and presenting with an opportunistic infection other than a tuberculosis or with a malignancy. In conclusion, some factors influencing the prognosis for AIDS patients are very dependent upon the geographical area of the series.
