Long-term follow-up of elderly patients with severe aortic stenosis treated by balloon aortic valvuloplasty. Importance of haemodynamic parameters before and after dilatation.

Early and late prognosis after percutaneous balloon aortic valvuloplasty (PBAV) was assessed in 38 consecutive elderly patients (mean age, 78.5 +/- 6.1 years). Significant valve opening was achieved in 35 patients. The hospital mortality was 8% (three patients); two other patients died within the first month and three underwent aortic valve replacement. At 2 years follow-up, there were 10 additional deaths (seven cardiac deaths) and five patients had symptom recurrence managed by aortic valve replacement (3) or repeat PBAV (2). Overall, six patients underwent surgery without untoward events and six had repeat PBAV. Only two out of six patients with repeat PBAV had sustained improvement; one was referred to surgery and the remaining three died soon after the second PBAV. One- and 2-year survival were respectively 72 and 62% and percentage of survivors with persistent improvement 68 and 41%. Although aortic valve area after PBAV was associated with outcome, predictors of poor long-term prognosis were primarily related to the pre-operative haemodynamic status. Patients with pulmonary resistances greater than 400 dynes cm-1 s-5 had the poorest outcome (chi 2 = 18.4-P less than 0.0001). Overall, signs of heart failure were predictors of poor long term follow-up. These data indicate that long-term success of PBAV is mainly related to the left ventricular dysfunction noted prior to intervention.

HLA complex and hypertrophic cardiomyopathy in a European population.

HLA tissue typing was performed in 43 unrelated white persons with hypertrophic cardiomyopathy (HCM) living in the northern part of Belgium. The frequency distribution of the 14-HLA-A, 18 HLA-B, 5 HLA-C and 10 HLA-D antigens, which were tested, was similar in patients with obstructive, provocable obstructive or non-obstructive HCM to that of a control group of 800 normal persons.

Artificial circus movement tachycardias: incidence, mechanisms, and prevention.

In a group of 45 patients treated with Medtronic 7000 and 7100 pulse generators for sick sinus syndrome or second or third degree atrioventricular block, an atrial synchronous mode of pacing was programmed in 34 cases and spontaneously occurring artificial circus movement tachycardias (ACMTs) were observed in nine. An analysis of conditions of occurrence, triggering mechanisms and patterns of ACMT, is presented. Various modalities of prevention are discussed. They resulted in suppression of ACMT in five patients and decrease of incidence in a sixth; the three remaining subjects were managed by definitive reprogramming in the DVI mode. Our conclusion is that correct prevention of ACMT requires the use of dual chamber pulse generators with programmable atrial refractory periods. For patients in whom a unit has already been implanted, careful observation of the triggering mechanism and pattern of ACMT may help in determining the most suitable way to prevent and suppress the arrhythmia.

[Do residential facilities for mentally retarded people exert an influence on the capacity for autonomy and social integration of their residents?].

The study relates to the lodging establishments situated in the Province of Liège (190 persons in 10 establishments). The independent variables are: the seriousness of each resident's handicap, and the belonging to an establishment. The dependent variables are composed of the three self-reliance scores and of the three integration scores of an instrument perfected for sake of the research: the E.C.A.I. (Evaluation des Comportements d'Autonomie et d'Intégration = Evaluation of the self-reliance and integration behaviors). The statistical data processing (especially the stepwise multiple regression analysis) points out that, provided the effect to the seriousness of the residents' handicap has been neutralized, the establishments have a specific influence on the level of their self-reliance and integration. An increase in the number of the establishments investigated should allow, beyond this global influence, to identify the working and structural characteristics of the establishments propitious to an increasing self-reliance and integration of the residents.

[Relation between immunogenicity and differentiation of metastasis in an experimental murine melanoma].

The graft into the footpad of Jackson mice of B16 melanoma cells which are only slightly pigmented, induces the simultaneous appearance of achromatic and well-pigmented metastatics. We have analysed histologically both types of metastases from the 8th to the 20th day after the resection of the primary tumor. The percentages of mitoses are higher in the achromatic metastases than in the pigmented ones. Both are infiltrated by lymphocytes and by a small number of macrophages and polymorphonuclears. A statistical analysis demonstrates however that the percentage of lymphocytes is not related to the importance of the metastase pigmentation. The discussion is devoted to the relations between the infiltration by lymphocytes, the immunogenicity and the differentiation of the tumor.

Pulmonary function in moderate neuromuscular disease without respiratory complaints.

Pulmonary function studies were carried out in 29 patients with moderate neuromuscular disease who had virtually no respiratory complaints. The earliest and most pronounced abnormalities were decreased transrespiratory pressures. Next a restrictive pulmonary defect with increased residual volume and a proportional decrease of maximal expiratory flows occurred, together with a decrease in static lung compliance. Because of a decrease in transpulmonary pressure at total lung capacity and often an increase in diffusing capacity per unit lung volume the above mentioned restrictive defects were attributed to muscular weakness and interstitial lung disease was excluded. Finally, some ventilation inequality could be found, probably originating in the poorly ventilated supradiaphragmatic regions.
