[Risks of pulmonary vein isolation by radiofrequency for the treatment of paroxysmal atrial fibrillation: a single centre experience of the first 200 patients].

The authors report the initial experience of an electrophysiological laboratory starting ablation for atrial fibrillation, a promising technique which is not yet widely practiced because of the risks related to the procedure. The incidence of severe complications (tamponade, pulmonary vein stenosis, ischaemic events) did not appear to be different in the first 100 procedures compared with the next 100 procedures: 3% in the two groups. The selection of patients, strict perioperative management and the initial support by confirmed operators seem to be the factors which minimise the complications rate of the procedure.

[Predicting prostate cancer with dynamic endorectal coil MR and proton spectroscopic MR imaging].

PURPOSE: Determine the feasibility of dynamic gadolinium enhanced MRI and spectroscopic imaging in routine clinical practice using standard equipment and its usefulness for patients with negative biopsies and high degree of suspicion of prostate cancer. PATIENTS AND METHODS: Fifty five patients underwent endorectal MRI using T2W spin echo (SE) imaging, dynamic gadolinium enhanced imaging and proton spectroscopic imaging before repeat US-guided transrectal biopsies. The statistical analysis consisted in the correlation of the results obtained with each of the two MRI techniques and the results of the biopsies in the corresponding prostate lobe. RESULTS: 32 patients were included in the analysis. Biopsies revealed cancer for 15 patients. The statistical analysis showed a lack of significant correlation between T2W-SE imaging and biopsy results. A correlation with statistical significance was found between dynamic gadolinium enhanced imaging and biopsies (p=0,0018) and between spectroscopic imaging results and biopsies in the corresponding lobe (p=0,0001). CONCLUSION: Endorectal MRI with a standard clinical equipment using dynamic gadolinium enhanced imaging and spectroscopic imaging may be used in clinical routine to improve detection and localization in prostate cancer compared to T2 weighted spin echo imaging.

Colloidal Protein Solutions as a New Standard Sensor for Adhesive Wettability Measurements.

Wetting experiments are carried out for the investigation of the interfacial interactions that take place when various solid materials are in contact with protein colloidal solutions. These materials are chosen because they are widely used in the field of orthopedic surgery. Contact angles (sessile drop) and surface tension (du Nouy's ring method) are measured in standardized conditions which take into account the various parameters disturbing wetting phenomena. An original cumulative deposits method (CDM) described here permits observation of the evolution of the wettability and allows estimation of the protein adhesion rate. Two families of materials emerge according to their behavior when they are in contact with the protein solution. One set (a chromium cobalt alloy and two polyethylenes) presents no detectable adhesion. The other set (stainless steel, zirconia, alumina, virgin and nitrogen implanted titanium alloy) shows a cumulative adhesion of proteins. We propose to adopt a BSA buffered solution as a referential sensor to characterize solid surfaces. From the CDM, the practical parameter P*, called the adhesive wettability power, can be used in order to analyze and classify biomaterials. Copyright 1999 Academic Press.

Long-term study of mortality and vascular complications in juvenile-onset (type I) diabetes.

A cohort of 372 insulin-dependent diabetic children, diagnosed between October 1949 and December 1960, were followed-up until December 1976 by the same team of physicians. At the time of diagnosis all patients were under 16 yr of age and were given standardized treatment which did not change from 1949 to 1976. The therapy consisted of daily insulin adjustment based on clinical assessment, the degree of physical activity, and the results of semi-quantitative urine tests for sugar and ketone bodies. These tests were systematically performed before breakfast, lunch, and dinner. Diet was normal, unmeasured, rich in carbohydrates (approximately 60%), and quantitatively unrestricted unless the patient was overweight. Rates for mortality and for the principal complications among this cohort were computed by the actuarial method. During the 26 yr of study, 26 deaths occurred, 16 of which were directly connected with diabetes. After 16 yr of follow-up, rates of proteinuria and hypertension were 4% and 2.1% respectively. The incidence of retinopathy reached 27%, including 1.5% proliferative retinopathy. After 26 yr, the rates rose to 14% for proteinuria, 16% for hypertension, and 85% for retinopathy, including 18% in the proliferative phase.

[The prevalence and incidence of juvenile onset insulin-dependent diabetes mellitus in France (author's transl)].

From notification of cases the prevalence and incidence of juvenile diabetes has been estimated in the population of France. Only diabetes treated with insulin has been considered. The prevalence of diabetes in children at the end of 1975 was one per 4,200 children who had not yet reached their 15th birthday. The number of cases increased regularly with each year of age. There are more boys than girls with diabetes but the sex ratio does not, in fact, differ significantly from that of the normal population. The incidence for 1975 was one per 27,000 children aged 0 to 14 years, i.e. one new juvenile diabetic per year can be expected in a population of 100,000 inhabitants of all ages. Diabetes was diagnosed more often in the children in October, November, December, January and February regardless of age (0 to 4; 0 to 19). Comparison of the french data with information collected from published literature and from other personal sources shows some similarities but also many marked differences between countries. Some of these differences may be due to different methods of ascertainment. Standardisation of methods is a prerequisite for comparability of data.
