Osteoradionecrosis: predisposing factors and outcomes of therapy.

Eighty-three episodes of osteoradionecrosis are reported. Of those episodes affecting the mandible (78), 23 (29.5%) required radical resection. The most common precipitating factors were postradiation extractions (22/83), periodontal disease (19/83), and preradiation extractions (17/83). Those episodes initially located within the zone of attached mucosa fared well with conservative measures while those initially located beyond the zone of attached mucosa fared poorly. In bone necroses where the external radiation dose to the affected bone exceeded 7,000 rad, the mandibular resection rate was high (44%). The most effective way of resolving advanced bone necroses was achieved with a course of hyperbaric oxygen therapy combined with a surgical sequestrectomy.

Advances in osseointegrated implants for dental and facial rehabilitation following major head and neck surgery.

Osseointegrated implants can be used to facilitate retention stability and support for facial and intraoral prostheses used to restore head and neck defects. Preliminary studies indicate that in nonirradiated maxillectomy patients the success rates are about 75%. In the reconstructed mandible the results appear to be more favorable--over 90% for implants placed in free nonvascularized bone grafts and over 90% for free revascularized bone grafts. Similar high success rates have been observed for most sites used to support facial prostheses. Success rates for auricular sites exceed 95% and for floor of nose sites success rates exceed 90%. Success rates have been lower (77%) for implants placed in the frontal bone for retention of orbital prostheses. Success rates for irradiated bone sites have been lower and range from 60.4% in the maxilla to 68.6% in facial bone sites. Of greater concern is that most implants placed in irradiated sites are beginning to show signs of impending failure.

A smoking cessation intervention for head and neck cancer patients: trial design, patient accrual, and characteristics.

This paper describes the aims, study design, and patient accrual and characteristics from an ongoing randomized controlled trial evaluating a surgeon- and dentist-delivered smoking cessation intervention for head and neck cancer patients. Subjects (n = 186) accrued into the trial are profiled in terms of demographics, medical and treatment descriptors, smoking history and behavior at enrollment, and related psychosocial variables (mood, adherence indicators, and alcohol consumption). These patients have long histories of tobacco use, moderate to high levels of nicotine dependence, and a lack of overt mood disturbance and are receptive to behavioral change.

A prosthodontic overview.

Osseointegrated implants have been in widespread use for more than 10 years. During this time, the design and use of implant-supported and implant-retained dental restorations have undergone steady change. This paper discusses this evolution and defines current trends in this exciting new discipline.
