Angel's trumpet (Brugmansia arborea) and mydriasis in a child--a case report.

We report a case of anisocoria in a 6-year old child who felt out of an inflatable swimming pool in the garden. After instillation of pilocarpine 1% the dilated pupil constricted, which is an important clinical sign associated with intracranial trauma. There was evidence of exposure to angel's trumpet (Brugmansia arborea), a plant known to contain anticholinergic substances. As the results of further neurological examination turned out to be normal, we believe the mydriasis was due to an accidental exposure to angel's trumpet. The time lag between exposure and presentation at the emergency room, the probably low volume of the plant extract in the eye and the unpredictable concentration of alkaloids in the plant, can influence and question the reliability of the pilocarpine test in this case.

Acute manifestation of LHON and coincidental finding of a pituitary adenoma: a case report.

A patient with Leber Hereditary Opticus Neuropathy (LHON) and a pituitary adenoma is presented. The different ophthalmological signs of LHON are described and illustrated. A definite diagnosis is made by detecting a mitochondrial mutation. A radiological examination revealed a pituitary adenoma. The pituitary adenoma may be a trigger factor in the penetrance of LHON.

Branch retinal artery occlusion in systemic diseases: a case report.

Although branch retinal artery occlusion (BRAO) is a relatively benign disease in terms of permanent visual impairment, the associated systemic diseases confer significant morbidity and mortality. The following systemic disorders can be found among patients presenting with a retinal artery occlusion: hypertension (59%), significant atherosclerotic cardiovascular disease (21%), diabetes mellitus (15% to 21%), left-sided valvular heart disease (5%), and cerebrovascular accidents (5%). These underlying systemic diseases are often responsible for a significant reduction in life expectancy and are in many cases potentially treatable conditions. A thorough cardiovascular examination (including a carotid duplex ultrasonography) may identify these occult diseases, and a prompt and effective treatment may improve the quantity and quality of patients' lives by reducing the risk of further arterial occlusive events.

Lasik learning curve: clinical study of 300 myopic eyes.

We present a retrospective analysis of the short term results of our first 300 myopic eyes which have been corrected by LASIK refractive surgery using the Automated corneal shaper and the NIDEK EC 5000 excimer. We will discuss our technique and outcome based on the evaluation of the safety of the procedure and the predictability of the postoperative result. We discuss all the complications we have encountered during this learning curve.

Ocular toxocariasis in a 36-year-old patient: a case report.

A 36-year-old female presented wit unilateral papillitis, followed by the development of a posterior pole granuloma. A positive ELISA-test for toxocara canis with a titer of 1/200 established the diagnosis. The patient was consecutively treated by pars plana vitrectomy and oral corticosteroids.
