Aldosterone-producing adenoma associated with foci of myelolipoma.

The simultaneous occurrence of adrenal myelolipoma and endocrine disorders is rare. Myelolipomas have occasionally been found in patients with Cushing's syndrome, pheochromocytoma and hyperthyroidism. A recently published study described one well-documented case of adrenal adenoma and myelolipoma in a patient with Conn's syndrome. In this report the patient had a one-year history of treated hyperthyroidism and diagnosed aldosterone-producing adenoma. A disseminated adrenal myelolipoma was found within the adrenocortical adenoma. To our knowledge this histological finding has not been previously reported in association with both Conn's syndrome and hyperthyroidism.

Adenoma

[Diagnosis and treatment of cystic echinococcosis].

New data are given on the life cycle of tapeworms of the genus Echinococcus, agents of a serious helmintozoonotic disease. The tapeworm Echinococcus granulosus, the agent of cystic echinococcosis, circulates also in Slovakia. Cystic echinococcosis most frequently affects the liver and a radical surgical treatment supplemented with the application of antiparasitics are the commonly used methods. The authors present the WHO proposal for classification of echinococcal cysts into 6 types, according to ultrasound imaging. They also report on a new technique known as PAIR (puncture--aspiration--injection--reaspiration), successfully used in many patients in endemic countries. They warn of a rapid distribution of the tapeworm E. multilocularis, the agent of alveolar echinococcosis, in central Europe. (Ref. 13.)

Echinococcosis

Iatrogenic injuries of large vessels.

At the beginning of the article the authors emphasized that the frequency of iatrogenic injuries of great vessels is very high now. They draw attention to the damage caused in coincidence with invasive diagnostic or therapeutic methods. The iatrogenic injuries are brought about during extensive operations due to advanced tumors, during transplantations and extensive reconstructive or corrective operations. But even some simple and common operations are not protected from these complications. The authors present their series of 10 patients with iatrogenic injuries treated during the years 1989-1991. The submitted case reports indicate interesting aspects of these accidents. The conclusions show the importance and danger of these accidents, and also the possibility of incorrigible results being caused by incorrect steps. (Tab. 1, Ref. 14.)

Vascular complications in reconstructive and replantation surgery.

Authors deal with the causes of complications in reconstructive and replantation surgery of the hand. The most common complications are of vascular origin. Early revision, removal of all devitalized tissue and autoplastics of arteries and veins combined with ATB treatment contribute to their successful management. (Fig. 7, Ref. 6.)

[Simultaneous treatment of an anal abscess and fistula].

At the First Surgical Clinic of the Faculty Hospital and Policlinic in Kosice during the period between Jan. 1, 1993 and June 30, 1995 99 patients with anal abscesses were operated (70 patients with a first attack and 29 with a relapsing abscess). Surgical treatment involved incision, evacuation, drainage and possibly treatment of an anal fistula. During operation an internal orifice and course of the fistula was detected in 28 patients, i.e. in 28.2%. The anal fistula was treated by decision in 14 patients or by an elastic ligature (multiple seton) in 14 patients. During the postoperative period of 3-32 months in patients subjected to one-stage treatment of the abscess and fistula no relapse was recorded nor incontinence due to hard or loose faeces. The presented form of treatment performed by an experienced surgeon with adequate knowledge of the anatomy and surgery of the anus is in the authors opinion an asset to the prevention of relapses of anal infections.

Abscess

[Laparoscopic peroperative cholangiography--indications and significance].

The authors have performed 35 peroperative laparoscopic cholangiographies by means of transcystic cannylation of the choledochus in 410 patients subdued to laparoscopic cholecystectomy. Choledocholelithiasis was found in 8 patients (22.85%). The authors advocate for the application of peroperative cholangiography in indicated cases since the method is simple, safe and sufficiently successful. (Tab. 5, Ref. 9.)

[Laparostomy in the treatment of severe hemorrhagic-necrotic pancreatitis].

The authors present their experience with the therapy of severe necrotic pancreatitis which was applied in the period from 1986 to 1993. Alcoholic etiology was stated in 52.56% patients. The authors analyse in detail the cases treated with "open packing" laparostomy. They emphasise its advantages and advocate to its more frequent application. Laparostomy in the frame of complex therapy of acute pancreatitis can contribute to mortality reduction. The mortality in the group of patients yielded 17.75%. Complications and multiorgan failure with a high mortality rate and severe forms of necrotic pancreatitis still represent a sad component of abdominal surgery. (Tab. 6, Ref. 13).

[Reconstructive operations in our clinical data].

The authors present the results of reconstructive vascular operations performed during the period from 1990 to 1994. According to the criteria of the European Vascular Surgery Council the lowest patency was recorded in the distal type of femoropopliteal bypasses in the lower extremities. They emphasize the necessity to apply the "Graft Surveillance Programme" in vascular consultation centre. (Tab. 2, Ref. 7.).

[Treatment of cholecysto-choledocholithiasis in the present era of minimally invasive laparoscopic surgery].

The authors try to contribute to the solution of the problem of cholecystolithiasis associated with choledocholithiasis. Nowadays in the era when choledochilithiasis is treated by low-invasion methods the solution has a different character. The majority of gallstones is symptomatic, their verification and removal by the endoscopic route is not associated with major problems. Concrements not detected during the perioperative period and not detected by diagnostic methods may cause various postoperative difficulties. The authors' experience is based on 302 laparoscopic cholecystectomies performed in the course of 14 months. They made cholangiographic examinations in 54 of the operated patients. This number comprised 21 where the examination was made before the operation, 18 during the operation and in 5 it was made during the postoperative period. In 20 patients (37.03%) the cholangiographic finding was positive. Due to intensive collaboration with the endoscopic centre, when using the method of therapeutic splitting und per operative cholangiography in indicated cases, they were able to reduce the risk of missed concrements to 0.33%.

[Importance of close interdisciplinary cooperation in the treatment of diseases of the thyroid gland].

The authors reflect on the importance of interdisciplinary collaboration when resolving problems of the diagnosis and treatment of diseases of the thyroid gland. In 1989-1993 at the First Surgical Clinic of the Kosice Hospital 189 patients were operated on account of thyroid disease. During the last year the number of operations increased substantially due to better cooperation. In their group eufunctional nodular goitre was indicated most frequently for surgery. In the diagnosis they preferred ultrasonography and needle aspiration cytology. Peroperative biopsy is used in every operation of the thyroid gland. If the USG and cytological finding are not clear, they perform hemothyroidectomy and send the material to a pathologist for evaluation. By intensive interdisciplinary collaboration they achieved a very low number of two-stage operations in carcinoma, 1.05% paralyses of the recurrent nerve and a 1.58% postoperative mortality.

[Increased activity of erythrocyte superoxide dismutase as the result of adaptation to long-term oxygen therapy].

Activity of erythrocyte superoxide dismutase (SOD) was assessed in 17 patients with decompensated chronic cor pulmonale and in 15 healthy volunteers. Also the serum levels of uric acid and hematological parameters were analyzed. Recordings were made in ten day intervals during long term oxygen therapy. The authors have found the increase of SOD activity in the treated patients which can be interpreted as a generalized adaptation of the organism to long term oxygen therapy.

[Superoxide dismutase activity of erythrocytes and their osmotic fragility].

Blood samples were obtained from 15 diabetic patients (type I) and 10 healthy subjects. Erythrocyte superoxide dismutase activity, Heinz bodies and osmotic fragility were determined. Our results suggest that decreased activity of erythrocyte SOD predisposes to denaturation of haemoglobin (Heinz bodies) and haemolysis (increased osmotic fragility).
