Dispersed and acinar forms of pancreatic metaplasia in human gastric mucosa.

Pancreatic acinar cell metaplasia of the gastric mucosa is a newly recognized entity. Its physiological relevance and association with other gastric pathological conditions remains to be elucidated. We studied morphology of pancreatic metaplasia of the gastric mucosa in 15 cases. Two morphological forms of pancreatic metaplasia were noticed: an acinar form and a diffuse form. The diffuse form is characterized by small foci of pancreatic metaplasia dispersed in the gastric mucosa without any separation from the surrounding gastric glands. The acinar form is characterized by acini of pancreatic metaplasia separated from the surrounding gastric mucosa by strands of connective tissue and single smooth muscle cells. Acinar form of pancreatic metaplasia seems to be spared by the chronic atrophic gastritis involving the neighboring gastric mucosa.

[Secondary prevention of colon cancer].

Each year more than 6000 patients in Poland die from cancer of the colon. Morbidity due to colonic cancer increases rapidly in our country. From 1973 to 1982 there was a three-fold increase in the morbidity due to colonic cancer as compared to the years 1952-1955. Five-year relative survival in the same periods did not improve, either being 28% in men and 27% in women. To compare, in the USA with high morbidity rates due to colonic cancer where each year there are 145,000 new cases of cancer, 5-year relative survival is 42% for men and 54% for women. This raises a question whether our strategy of treatment of colonic cancer is correct. This question is even more justified now when secondary prevention of colonic cancer is of practical value. Total mortality due to colonic cancer is 60%. However, diagnosis and treatment of early forms of colonic cancer reduces mortality to 20% and below. Therefore, effective management colonic cancer should include not only the so-called primary prevention (interfering with etiological factors for instance through high fibers diet) but also secondary prevention through monitoring of precancerous changes in the colon, removal of potentially malignant lesions and early forms of colonic cancer [19]. Secondary prevention interfering with the pathogenesis of colonic cancer is the subject of the present paper. We would like to emphasize the need for a better management program for colonic cancer, especially that the data concerning colonic cancer in Poland may be underestimated due to diagnostic neglect and faulty cancer register.(ABSTRACT TRUNCATED AT 250 WORDS)

Close contacts between mast cells and nerve fibres in the human oxyntic mucosa.

The study of mast cells and nerve fibres in routine biopsies of human oxyntic mucosa was performed with the use of microscopic and ultrastructural methods. Simultaneous visualization of mast cells with Alcian blue and nerves with anti S-100 antibody allowed to study contacts between these two elements of lamina propria. Approximately 17% of mast cells appose nerves in gastric mucosa, which is less than the number of such contacts in the gut reported by other authors. We have found no differences between histologically normal mucosa and gastritis in the aspect of the total number of mast cells per 1 mm2 of lamina propria, number of their contacts with nerves and the ratio of mast cells apposing nerves to the total amount of mast cells. The ultrastructural study revealed significant polymorphism of mast cell-neuron contacts as well as the absence of any specialized structures at the site of adhesion between these two types of cells. The mode of degranulation of mast cells suggests that they are actively engaged in the reaction to noxious stimuli challenging the oxyntic mucosa.

[Studies on the effect of Krynica mineral water from the "Zuber" spring on alcohol-damaged gastric mucosa in rats].

The study was performed to investigate the protective effect of mineral water "Zuber", "Zuber" with "Jan" and an antacid on the alcohol induced gastric mucosal injury in rats. It was shown that Alugastrin, a hyperosmotic liquid antacid, protects rat gastric mucosa against alcohol. In opposite, a hyperosmotic and alkaline mineral water "Zuber" increases a vulnerability of the gastric mucosa and worsen the damaging effect of alcohol on it. Izoosmotic mixture of mineral waters "Zuber" and "Jan", similarly to a physiologic saline, do not protect rat gastric mucosa. The study show that mineral waters "Zuber" and "Zuber" with "Jan" have a short-lasting neutralizing effect on the gastric HCl and in opposite to antacids have not protective properties on gastric mucosa.

Ultrastructure of colonic endocrine cells in ulcerative colitis.

Endoscopic specimens of colonic mucosa from 40 ulcerative colitis patients were investigated. Specimens were taken from both sexes patients, aged 16-74 (means = 41 years). An average number of 5 biopsies was examined in each patient taken 10, 20, 40, 50 and 80 cm from rectum. Material was divided into 3 groups: 1. histologically confirmed ulcerative colitis (CU), 2. clinical diagnosis of CU in remission-histologically non specific inflammation, 3. functional disorders of the large bowel-histologically normal colonic mucosa. The latter was a control group. Biopsies were fixed in glutaraldehyde, postfixed in osmium tetroxide. Ultrastructural examination was performed on Epon sections in Tesla BS-500 electron microscope operating at 60 kV. A Lausanne classification of endocrine cells was used in the study. The most common were EC cells, followed by PP, D1 and L respectively. In the first group of patients damaged endocrine cells were more commonly observed. These destructive changes were: vacuolisation of cytoplasm, degranulation and discharge of endocrine granules into intercellular space and degradation of the entire cells.

Cytoprotective effect of 16, 16' dimethyl prostaglandin E2 and some drugs on an acute galactosamine induced liver damage in rat.

Present experiment was aimed to study whether 16, 16' dimethylprostaglandin E2 (dmPGE2), Hepatofalk (HF), or Orotofalk (OF) may prevent an acute liver damage induced in rats with D-galactosamine (GalN). Fifty male rats were divided into 5 groups: 1. controls, 2. rats receiving GalN 750 mg/kg b. w. intraperitoneally, 3. animals pretreated with 5 microgram/kg dmPGE2 given subcutaneously 24 hours prior to, 30 min prior to and 6 hours after GalN. Rats of group 4 received HF 0,8 ml/kg intramuscularly and group 5 OF 0,3 caps/kg intragastrically 24 hours prior to, 30 min prior to and 6 hours after GalN. Animals were sacrificed 24 hours after GalN injection. Histological, histochemical and ultrastructural studies of the liver were performed. In rats receiving GalN alone (group 2) typical severe liver damage consisting of acidophilic necrosis of hepatocytes, periportal and intralobular inflammatory infiltration, hypertrophy and hyperplasia of Browicz-Kupffer cells has been observed. Histochemical investigations showed in this group fatty degeneration and a decrease in glycogen content in hepatocytes, irregular distribution of lysosomes, numerous cytolysosomes and uneven decrease in lysosomal enzymes activity (acid phosphatase and beta-glucuronidase). Ultrastructural studies revealed depletion in glycogen, fat droplets, hypertrophy of smooth endoplasmic reticulum and numerous autophagic vacuoles. Some of these vacuoles or residual bodies were dropping out into intercellular space. Focal accumulation of lamellar cytomembranes as well as condensation of heterochromatin in nuclei were also observed. Pretreatment of animals with dmPGE2 (group 3), HF (group 4) or OF (group 5) prior to GalN prevented liver cell necrosis. Histological, histochemical and ultrastructural picture of the liver was in these groups close to normal. Only very slight hypertrophy and hyperplasia of Browicz-Kupffer cells, was seen as well as depletion of glycogen and hypertrophy of smooth endoplasmic reticulum in hepatocytes. We conclude dmPGE2, HF and OF offered impressive cytoprotection against GalN induced liver damage in rat.
