Early experience in lung transplantation.

Lung transplantation is becoming an acceptable mode of therapy worldwide for the end-stage lung disease. We present our initial experience with the first 10 consecutive lung transplant patients at Hadassah University Hospital. There were 5 males and 5 females with an age range 27 to 59 years. Eight patients underwent single lung transplantation, one patient had double lung transplantation and one had heart-lung transplantation. Indications were: pulmonary fibrosis in 4, emphysema in 4, cystic fibrosis in 1, and cystic bronchiectasis in 1. Two patients had primary graft failure (1 death). Nine patients had a serious infection after transplantation (1 death). Four patients developed airway complications including dehiscence of bronchial anastomosis (1 death), bronchial stenosis requiring placement of a stent in 2 patients, and pneumothorax in 1 patient. One patient required tracheostomy. One patient died of massive brain infarction secondary to pulmonary venous thrombosis and embolization. Six patients are intermediate-term survivors, with a follow-up period of 4-24 months. Four of them had at least one episode of rejection each. In all survivors pulmonary functions and quality of life improved and they do not need supplemental oxygen. We conclude that lung transplantation is a viable option for end-stage lung disease. Better selection of patients and perhaps improved immunosuppression agents will further improve outcome in lung transplantation. Shortage of donor supply is currently the limiting factor in successful lung transplantation in Israel.

Surface hardness of light-cured composite resin cured directly or through a transparent matrix using three different light guides.

The purpose of this study was to compare the Vickers Hardness (HV) of composite resin which was light-cured by 3 different light guides, either through or without a transparent polyester matrix band. The 3 light guides, standard Demetron (SG), Turbo Tip (TT) and a standard Demetron, with obvious accidental damage (DL) were tested with both Model 100 and Cure Rite radiometers. HV tests were done on all samples both immediately and one hour after light-curing. Results were analysed using the Anova and Student t-test. The light intensity readings in mW/cm2 were SG 600 and TT 850 with both radiometers and DL tested 350 and 380. Composite resin surfaces undergo significant surface hardening in the first hour after light-curing. A 350-380 mW/cm2 intensity gives significantly lower HV than 600 + mW/cm2 but 800 mW/cm2 does not produce significantly greater HV than 600 mW/cm2. Curing through a transparent polyester matrix gives marginally greater HV than curing directly. The output of light guides is a significant factor in the surface hardness of cured composite resin.

Importance of Arg-219 for correct biogenesis of alpha 1 homooligomeric glycine receptors.

The inhibitory glycine receptor is characterized by a pentameric arrangement of subunits with four predicted transmembrane segments (M1-M4) each. Here, we have mutagenized arginine residues located at both termini of the alpha 1 subunit segment, M2, which lines the receptor's anion channel. No glycine-gated channel formation could be detected in the plasma membrane of expressing cells for any of the mutants. In addition, mutating the arginine at the cytoplasmic terminus of M2 (R219) generated proteins which were only core-glycosylated, retained within intracellular compartments, and aggregated to high molecular weight complexes. Thus, residue R219, which corresponds to an arginine/lysine conserved in other ligand-gated ion channel polypeptides, is essential for correct biogenesis of the receptor.

Female nurse executives. Finally, at an advantage?

Discrimination, subtle and overt, from socialization and institutional forces, continues to keep nurse executives, most of whom are women, out of chief executive officer (CEO) positions. However, because of the women's movement, changing organizational forces, and shifting demographics, more nurse executives are likely to move into the ranks of CEOs. The authors explore the issues of managerial discrimination and indicate strategies that are beginning to open doors to female nurse executives who desire CEO positions.

Pseudoaneurysm of the left ventricle after myocardial infarction: A curable form of myocardial rupture.

Rupture of the myocardium following an acute infarction is occasionally compatible with survival. In such cases, pre-existent pericardial adhesions confine the bleeding to a limited space within the pericardial sac and produce a chamber which communicates with the left ventricular cavity. The chamber gradually enlarges and forms a pseudoaneurysm. Two cases are described in which the diagnosis was made preoperatively and the pseudoaneurysm was surgically resected. Twenty-one previously reported cases are reviewed, as well. The condition's progressive nature leading to death from heart failure or rupture of the pseudoaneurysm, and its amenability to surgical cure make it imperative that the diagnostic features be well known. The diagnosis should be suspected in cases of progressive heart failure developing shortly after the acute phase of myocardial infarction, in which chest roentgenograms show an enlarging heart shadow characterized by a distinct posterior or lateral bulge. The diagnosis can be confirmed by left ventriculography, which should be performed immediately in all suspected cases.
