Comparative cytologic findings of in situ and invasive adenocarcinoma of the uterine cervix.

The cytologic findings in 13 cases of cervical adenocarcinoma in situ (mean age 33.9 yr) were compared with those in 9 cases of cervical invasive adenocarcinoma (mean age 45.7 yr). All diagnoses were confirmed by biopsy. The major differences found were that a necrotic background (66% invasive vs. 0% in situ) and macronucleoli (77% invasive vs. 0% in situ) were more common in invasive cases, whereas associated squamous-cell dysplasia (69% in situ vs. 0% invasive) was more common in in situ cases. Other differences included syncytial tissue fragments with irregular cell borders, frequent cell drop-off, and markedly atypical single columnar cells (or markedly atypical naked nuclei) seen more commonly in invasive cases, whereas monolayer sheets and/or syncytial tissue fragments with smooth borders and rare cell drop-off were more common in in situ cases. Although none of these features is diagnostic individually, in combination they are helpful in distinguishing between in situ and invasive adenocarcinoma. Nine of 13 of the in situ cases showed associated squamous-cell dysplasia. In all nine of these cases, the glandular dysplasia was overlooked or underestimated in severity on original cytologic diagnosis. This suggests that the presence of a squamous lesion may lead to decreased detection of glandular lesions, perhaps because the features of squamous lesions are more well known; thus they are more easily recognized, at the expense of glandular lesions. All invasive adenocarcinomas were identified as carcinoma in the original diagnosis with seven specified as invasive adenocarcinomas.

The effect of a high carbohydrate diet on running performance during a 30-km treadmill time trial.

The purpose of the present study was to examine the influence of a high carbohydrate diet on running performances during a 30-km treadmill time trial. Eighteen runners (12 men and 6 women) took part in this study and completed a 30-km time trial on a level treadmill without modifying their food intake (trial 1). The runners were then randomly assigned to a control or a carbohydrate (CHO) group. The CHO group supplemented their normal diets with additional carbohydrate in the form of confectionery products during the 7 days before trial 2; the control group matched the increased energy intake of the CHO group by consuming additional fat and protein. The mean (SEM) carbohydrate intake of both groups was 334 (22) g before trial 1, after which the CHO group consumed 566 (29) g.day-1 for the first 3 days and 452 (26) g.day-1 for the remaining 4 days of recovery. Although there was no overall difference between the performance times for the two groups during trial 2, the CHO group ran faster during the last 5 km of trial 2 than during trial 1 [3.64 (0.24) m.s-1 vs 3.44 (0.26) m.s-1; P less than 0.05]. Furthermore, the 6 men in the CHO group ran the 30 km faster after carbohydrate loading [131.0 (5.4) min vs 127.4 (4.9) min; P less than 0.05], whereas there was no such improvement in times of the men in the control group.(ABSTRACT TRUNCATED AT 250 WORDS)

Malignant transformation of angiomyolipoma.

Angiomyolipomas are well recognized but uncommon tumors that can involve the kidney. To our knowledge a well documented case of malignant change of angiomyolipoma has never been reported. We report a case of renal angiomyolipoma and leiomyosarcoma with a clearly identified transition of benign to malignant elements seen in the resected tissue.

Pre-season physiological characteristics of English first and second division soccer players.

This investigation was undertaken in an effort to establish physiological characteristics of soccer players and to relate them to positional roles. A total of 135 footballers (age 24.4 +/- 4.6 years) were assessed for body mass, % body fat, haemoglobin, maximal oxygen uptake (VO2 max), leg power, anaerobic capacity and speed prior to an English league season. The sample included 13 goalkeepers, 22 full-backs, 24 centre-backs, 35 midfield players and 41 forwards. The goalkeepers were significantly heavier (86.1 +/- 5.5 kg; P < 0.01) than all groups except the centre-backs, had significantly higher estimated body fat percentages than centre-backs, forwards, midfield players (P < 0.01) or full-backs (P < 0.05), significantly lower estimated VO2 max values (56.4 +/- 3.9 ml kg-1 min-1; P < 0.01) and were slowest over 60 m (12.71 +/- 0.42 s). The midfield players had the highest predicted VO2 max values (61.4 +/- 3.4 ml kg-1 min-1), this being significantly greater (P < 0.05) than for the centre-backs. The forwards were the fastest group over 60 m (12.19 +/- 0.30 s), being significantly quicker than goalkeepers or centre-backs (P < 0.01) and full-backs (P < 0.05). Anaerobic power, as well as knee extensor torques (corrected for body mass) and extensor-flexor ratios, were similar between groups. No difference in estimated body fat percentage was observed between any of the outfield players, and haemoglobin concentrations were similar among players of all positions.(ABSTRACT TRUNCATED AT 250 WORDS)

Rabbit esophageal cell cytoplasmic pH regulation: role of-antiport and-dependenttransport systems.

Regulation of cytoplasmic pH (pHi) of esophageal cells assumes importance as these cells can be exposed to mucosally absorbed acid during gastroesophageal reflux episodes. In this study, we examined whether esophageal cells possess pHi transport systems. Esophageal cells were harvested utilizing a gentle trypsin technique that yieldedcells per esophagus. Cells were attached to a glass cover slip that had been pretreated with rat-tail collagen, and pHi was measured continuously in a spectrofluorometer utilizing 2',7'-bis(2-carboxyethyl)-5(-6)- carboxyfluoroscein acetoxymethyl ester as a pH-sensitive fluorescent probe.The basal pHi of cells exposed to a-containing solution averaged 7.52 ± 0.20 (n = 6). The pHi declined slightly but not significantly to 7.46 ± 0.12 with the addition of 5%and 28 mMWhen H2 4,4'-diisothiocyanatostilbene- 2,2'-disulfonic acid (DIDS; 0.5 mM) was added, pHi was unchanged. However, addition ofM amiloride caused pHi to decrease to 7.29 ± 0.18 (P less than 0.01). When cells were acidified (pHi 6.3-7.0) using a(20 mM) pulse technique, pHi was rapidly restored toward neutrality in the presence of a-free externalconcentration ([]o)-containing solution (pH units/min = 0.26 ± 0.12; n = 8). Alkalinization was completely blocked withM amiloride. In the presence ofM amiloride, 28 mM, and 5%, acidified cells also alkalinized, although at a slower rate (0.11 ± 0.04 pH units/min; n = 16).(ABSTRACT TRUNCATED AT 250 WORDS)

Suppression of feeding and body weight by inescapable shock: modulation by quinine adulteration, stress reinstatement, and controllability.

Three experiments examined food intake and body weight in rats after exposure to one session of intermittent, inescapable electric shock. Quinine adulteration and shock both suppressed feeding (Experiment 1); recovery of feeding after shock was impeded when quinine adulteration was combined with a mild daily stress reinstatement (Experiment 2). Body weight also was suppressed by shock (Experiments 1 and 2); control over shock provided some protection against this deficit (Experiment 3). These results suggest roles for "finickiness" and vulnerability to mild stressors in the maintenance of eating disorders associated with stress and depression. The findings also may have implications for interpretation of deficits in appetitively motivated behaviors after stress.

The influence of high carbohydrate diets on endurance running performance.

The purpose of the present study was to examine the influence of high carbohydrate (CHO) diets on recovery of endurance capacity following a treadmill run to exhaustion. Two high CHO diets were used, one in which the normal diet was supplemented with complex carbohydrates and the other in which supplementation was achieved with simple carbohydrates. The thirty recreational runners who took part in this study (fifteen men and fifteen women) completed weighed food intake diaries two to three weeks before the start of the study. From an analysis of this information each subject's 'normal diet' was prescribed before Trial 1 and then a supplemented diet before Trial 2. The aim was to achieve an increase in carbohydrate content to 70% in the diets of the two high CHO groups and an equivalent increase in energy intake by the Control group. The subjects were required to run to exhaustion on a treadmill at a speed equivalent to 70% VO2max on two occasions separated by 3 days. After Trial 1 the subjects were divided into three equal groups. The Complex CHO group (301 +/- 86 mg vs 507 +/- 120 mg) and Simple CHO group (265 +/- 45 mg vs 462 +/- 81 mg) increased their CHO intake by approximately 70% (p less than 0.05) during the 3 days before Trial 2 whereas the Control group increased their energy intake with additional protein and fat so as to match the energy intakes of the two CHO groups.(ABSTRACT TRUNCATED AT 250 WORDS)

A progressive shuttle run test to estimate maximal oxygen uptake.

The purpose of the present study was to examine the validity of using a 20 m progressive shuttle run test to estimate maximal oxygen uptake. Running ability was described as the final level attained on the shuttle run test and as time on a 5 km run. Maximal oxygen uptake (VO2 max) was determined directly for seventy-four volunteers (36 men, 38 women) who also completed the shuttle run test. Maximal oxygen uptake values were 58.5 +/- 7.0 and 47.4 +/- 6.1 ml.kg-1.min-1 for the men and women respectively (mean +/- SD, P less than 0.01). The levels attained on the shuttle run test were 12.6 +/- 1.5 (men) and 9.6 +/- 1.8 (women; P less than 0.01). The correlation between VO2 max and shuttle level was 0.92. The correlation between VO2 max and the 5 km run was -0.94 and the correlation between both field tests was -0.96. The results of this study suggest that a progressive shuttle run test provides a valid estimate of VO2 max and indicates 5 km running potential in active men and women.

Kingella denitrificans prosthetic endocarditis.

Kingella species are rarely reported as a cause of endocarditis. We report two cases of K. denitrificans endocarditis occurring in patients with prosthetic valves. K. denitrificans has not been previously reported to cause prosthetic valve endocarditis. Both patients may have developed bacteremia from altered upper respiratory or oral mucosa with subsequent prosthetic valve infection. Treatment with penicillin G and streptomycin was successful in the first patient, whereas ampicillin alone resulted in clinical cure in the second case. Neither patient required further surgery. In vitro antibiotic susceptibility testing indicated that both organisms were highly sensitive to cefotaxime and ampicillin.

In vitro effect of dopamine and pimozide on human chorionic gonadotropin secretion.

In human placental explants cultured in vitro, dopamine inhibited human chorionic gonadotropin (hCG) secretion into the culture media. In the control flasks, the level of hCG secretion was 751 +/- 215 mIU/gm of tissue. When 1 mM of dopamine was added, hCG levels decreased to 321 +/- 57.6 mIU/gm of tissue (n = 6, P less than 0.1)--5 and 10 mM of dopamine significantly inhibited hCG secretion. In contrast, 1 mM of pimozide enhanced hCG secretion by 1.8-fold compared to control levels (1,707 +/- 343 versus 3,117 +/- 0.005). This in vitro effect on hCG is similar to the effect of dopamine and pimozide on hCS secretion by placental explants.

Failure of somatostatin to affect human chorionic somatomammotropin and human chorionic gonadotropin secretion in vitro.

The effect of somatostatin (SRIF) on human chorionic somatomammotropin (hCS) secretion was studied in human placental explants cultured in vitro. In the experimental flasks, SRIF was added in a concentration of 10, 100, and 1000 ng/ml media; hCS levels measured by RIA were not different from those found in the control flasks. In separate experiments, we investigated the action of SRIF on hCG secretion by a human malignant choriocarcinoma cell line maintained in tissue culture. SRIF (1000 ng/ml) did not inhibit basal or dibutyryl cAMP-induced stimulation of hCG secretion. These results suggest that somatostatin does not suppress hCS or hCG release in vitro from normal or malignant trophoblast, respectively.
