Transplantation of pediatric kidneys into adult patients.

The authors are dealing with the problems of transplantation of pediatric kidneys into adult recipients. Kidneys obtained from children who died 1-3 year old can be both transplanted into one adult recipient. The authors used this method in transplantation of kidneys from 2 - year old donor into adult recipient.

The ureter in renal transplantation.

The outcome of kidney transplantation depends on several factors. The quality of the ureter of the transplanted kidney and the technique of its connection with the recipient's urinary tract seem to be two of the most important ones. In this paper the authors analyse the incidence of ureteral complications of the grafted kidney in 99 ureterocystoneostomies performed in conjunction with kidney transplantation. Urinary fistulas occurred in 7, stenosis of the ureter in 3 cases. The diagnostic possibilities and treatment of the complications are discussed.

Allograft rupture after kidney transplantation.

Spontaneous rupture of the parenchyma of the transplanted kidney occurs in 0.8 to 9.6 per cent of transplantations. Mortality is reported to be up to 9 per cent. Every rupture requires surgical intervention. In most cases graftectomy has to be performed. The cause of the rupture is an immunological destruction of the graft, the histological finding being a massive rejection of an interstitial type. In a set of 51 transplantations of cadaver kidneys rupture occurred in 4 patients (7.8 per cent). It was necessary to carry out a graftectomy in all cases immediately or later on.

The evaluation of some biochemical parameters in pyridoxine-treated calcium oxalate renal stone formers.

Several serum and urinary constitutents were evaluated in 13 calcium oxalate kidney stone formers in whom according to urinary vitamin B6 excretion sufficient vitamin B6 intake before the study was suggested. The influence of 28-day Pyridoxine (Spofa) supplementation (20 mg three times daily) on these biochemical parameters was searched. Pyridoxine supplementation was followed by a significant increment in the mean values of serum uric acid; the remaining serum constitutents did not change. Urinary calcium, phosphare, magnesium, sodium, potassium, and uric acid slightly increased, urinary oxalate excretion slightly fell; there was a high variability in the changes between the individuals. No changes in the mean values of clearance of endogenous creatinine, percentage of tubular reabsorption of phosphate and urinary zinc excretion were found. It is suggested that long-term studies are necessary to search the factors influencing successful stone prevention or stone recurrency in pyridoxine-treatment patients.
