[Results of surgical treatment of anaplastic cerebral gliomas].

In the years 1971 through 1977 247 patients with anaplastic cerebral gliomas were treated. Out of this material a group of 119 patients was completed who were treated only surgically and had follow-up examinations or responded to an inquiry form sent to them. In the light of this follow-up study it was established that the survival time was shortest, only up to 6 months, in the patients with preclinical symptoms lasting up to 3 months. In the remaining patients the survival time was longer, reaching over one year in 11 cases and over 2 years in 3 cases. In view of the mode of spread of recurrent neoplasm the authors differentiated exophytous expansion - with the infiltration filling the postoperative tissue defect and extending beyond its margins, infiltrative expansion - with the infiltration spreading on the walls of the postoperative tissue defect, destructive expansion - with the neoplasm infiltrating and destroying the brain tissue. Two latter modes of growth were most frequent and led to death of the patients without producing signs of intracranial hypertension.

[Ischemic complications of ruptured intracranial aneurysms].

Eighty patients are reported with ischaemic complications developing during subarachnoideal haemorrhage due to aneurysm rupture. Ischaemic complications developed in 12.82% of cases in the whole group of subarachnoideal haemorrhages caused by aneurysm rupture. The causes of ischaemia were: closure or stenosis of the afferent artery during the operation, arterial spasm, thrombosis at the site of the aneurysm and brain oedema. Ischaemia during intracranial aneurysm rupture caused in most cases pale infarcts. Ischaemic complications of aneurysm rupture had a serious prognosis, nearly half the patients with this complication died.

[Case of double aneurysm located at the bifurcation of the basilar artery].

The authors report a case of two aneurysms at the bifurcation of the basilar artery in a 47-year old housewife. The technique for the exclusion of the aneurysms from the cerebral circulation using the operating microscope is described. The course of the illness and the patient's condition several months after procedure are described.

[Early complications following intracranial surgery].

The authors discuss early non-inflammatory postoperative complications developing in the first week after operations. The reported material comprised 97 patients from among 1 815 cases treated surgically in the years 1971--1973 at the Departments of Neurosurgery and Neurotraumatology, Medical Academy in Cracow. The most frequent causes of reoperations were brain oedema, epidural haematoma and haematoma in the empty space left after evacuation of lesion. In the group of reoperated patients the percent of fatal cases increased to 56,7%.

[Subarachnoid hemorrhage from ruptured aneurysm in the course of pregnancy].

The authors report a case of subarachnoid haemorrhage from a ruptured aneurysm of the left internal carotid artery at the site of branching-off of the ophthalmic artery in a patient with advanced pregnancy. Indications and contraindications to surgical treatment in pregnancy are discussed. The case and follow-up are described.

[Results of combined treatment (by surgery, ionizing radiation and the cytostatic drug CCNU) of patients with anaplastic glial neoplasms of the brain].

For estimating the effectiveness of combined treatment of cases of anaplastic gliomas of the cerebral hemispheres retrospective analysis was undertaken of 83 patients treated only surgically and prospective analysis of 81 patients treated by the combined method. In the group treated only surgically 47 patients had glioblastoma multiforme, and 36 had anaplastic oligodendroglioma. The general condition and the neurological status were evaluated using the score of Karnofsky. In the group of combined treatment the patients were given telegammatherapy from Co-60 cobalt source by the method of Skołyszewski and CCNU (Belustine) every 6-8 weeks for 1 year in doses of 100 mg/m2. In about 25% of patients transient leucopenia was observed, and 20% had transient thrombocytopenia. In about 30% of cases transient increase of intracranial pressure occurred. In the compared groups it was possible to establish, using the statistical method of representation probability of Kaplan-Meier and statistical analysis of Gehan as modified by Wilcoxon, that the mean survival time in the surgically treated patients was 6.25 months, while in the patients treated by combined method it was 13.97 months.

[Topographical and radiological correlations in aneurysms of the middle cerebral artery].

The authors demonstrated topographic-radiological correlations in cases of middle cerebral artery aneurysms verified surgically. In an own material of 91 cases treated in the Department of Neurosurgery, Medical Academy in Cracow in the years 1980-1983 the relationship of the aneurysm to the neighbouring vessels and the direction of the dome of the aneurysm were analysed. In the presented material aneurysms of the middle cerebral artery accounted for 18% of all intracranial aneurysms. Most frequently they were situated at the main bifurcation of the artery (86.8%). The dome of the aneurysms of the main bifurcation was lying usually in the line prolonging the main trunk of the artery cephalad and laterally. The results of surgical treatment are presented in 81 patients with aneurysms of the middle cerebral artery treated surgically.
