IV inotropic agents: dopamine, dobutamine, and amrinone.

Three commonly used IV inotropic agents--dopamine, dobutamine, and amrinone--have been discussed in this article. Knowledge of their common mechanisms of action, individual actions, dosages, methods of preparation, precautions, and side effects can only help critical care nurses maintain or improve the quality of care given to patients receiving these medications.

[Introduction of a new technique of partial pulp amputation in permanent teeth in children after mechanical injuries. Preliminary communication].

The study material comprised 56 permanent teeth with traumatic pulp exposure complicated in 15 cases by partial tooth luxation in 52 children aged from 7 to 15 years. In each case routine clinical and radiological examinations were done. The time between trauma and procedure ranged from several hours to 70 days. In all teeth partial pulp amputation was done cutting off by the gentle technique a part of the coronal pulp and covering of the remaining pulp with calcium hydroxide, zinc oxide with eugenol or with Dycal, carboxyl cement and permanent filling. The analysis of the studied material showed that the greatest group comprised children aged 8-11 years (40 cases), and fractures of one-half of the crown prevailed (32 cases), point pulp exposure (32 cases), developmental stage Rc and A 1/2 of root (40 cases). The short-lasting follow-up made possible clinical and radiological control of only 39 teeth after 6 months, and 7 teeth after 12 months. After 6 months good results were obtained in 84.6% of cases.

[Radiological estimation of interradicular space of primary molar teeth].

Radiological estimation of interradicular space of primary molar teeth has been performed. Radiograms taken from the department's archives on which primary molar teeth without dental caries were visible have been used for the purposes of the examinations. Roentgenograms had been made for exposition conditions required for endodontic therapy needs. Periodontium, lamina dura of the alveolus, bone texture of the interradicular space and disposition of permanent tooth buds have been estimated according to the radiograms. From the examination results it followed that proper estimation of interradicular space of primary molar teeth according to radiograms made for endodontic therapy needs is not possible. Radiological image of interradicular space of primary molar teeth in various both with reference to bone structure and disposition of permanent tooth buds. In most cases the interradicular space of primary molar teeth demonstrates thin bone structure on radiograms that should be considered to be physiological state when the continuity of lamina dura of the alveolus is preserved.

Vertebrobasilar insufficiency. Part 1: Microsurgical treatment of extracranial vertebrobasilar disease.

Extracranial vertebrobasilar artery thrombo-occlusive disease may cause repetitive transient ischemic episodes and, less frequently, brain-stem or cerebellar infarction. This report describes 40 patients who experienced repetitive vertebrobasilar ischemic symptoms despite maximal medical therapy. The natural history, pathogenesis, and treatment options for each causative lesion are reviewed. The operative approaches to symptomatic disease of the proximal vertebral arteries, arterial compression by cervical osteophytes, traumatic lesions of the vertebral arteries, and thrombo-occlusive pathology of the distal extracranial vertebral arteries are outlined. Specific anesthetic and surgical techniques that have proved successful while achieving zero operative mortality and low perioperative morbidity rates are reported.

Vertebrobasilar insufficiency. Part 2. Microsurgical treatment of intracranial vertebrobasilar disease.

Posterior circulation transient ischemic attacks have an associated risk of subsequent infarction of approximately 5% per year. Intracranial vertebrobasilar thrombo-occlusive lesions appear particularly likely to result in repetitive ischemic symptoms and in infarction due to hemodynamic insufficiency. The authors present their experience with 45 patients with symptomatic intracranial vertebrobasilar vascular disease despite maximal medical therapy. The specific operative approaches for intracranial vertebral artery endarterectomy and extracranial to intracranial posterior circulation revascularization procedures are outlined.

Caries increment in primary teeth after application of Duraphat fluoride varnish.

In order to evaluate the possible cariostatic effect of a fluoride varnish in the primary dentition, Duraphat varnish was applied twice yearly for 2 years to the teeth of 195 3 1/2-year-old children of both sexes attending nine preparatory schools in the Warsaw area. A control group of 127 children attending nine other preparatory schools was also examined. For each child a dmfs1 index (including "caries without macroscopic defect") and a dmfs2 index ("caries without macroscopic defect" excluded) were determined as well as a dmft1 and a dmft2 index. After 2 years the difference between the test and the control groups regarding the mean dmfs1 and dmft2 index increments was 9.39% (P greater than 0.1) and 24.09% (P less than 0.05), respectively. No significant differences were found, however, concerning the dmft2 and dmfs2 indices, the percentage children with new gingival caries, the number of teeth extracted on account of caries and the number of remaining teeth. A test group of 46 children treated with fluoride varnish in which a complete radiological examination could be carried out before and after the experimental period and which was treated with fluoride varnish was compared with a similar group of 27 children, which was not treated with fluoride varnish. The differences in caries increments were small, less than 15%, and were not significant. Taking into consideration the great variation between the schools it was concluded that the cariostatic effect of Duraphat application twice yearly in the primary dentition of these children was slight.

Frontal lobe perivascular schwannoma. Case report.

A 15-year-old youth presented with a history and computerized tomography picture of subarachnoid hemorrhage. Conservative management was undertaken after failure to demonstrate an aneurysm. Reevaluation and operation 15 months later disclosed a firm leathery schwannoma intimately related to the frontal lobe and extending into the cortex in the Virchow-Robin spaces.

A comparison of D and L-tryptophan on the cerebral metabolism of 5-hydroxytryptamine and dopamine in the dog.

1 After D and L-tryptophan (50 mg/kg) were given intravenously in the dog, the concentration of the amino acid was increased in ventricular cerebrospinal fluid (CSF) during the subsequent 4 h or sampling, although the concentrations were significantly lower following the administration of the D-isomer. 2 There was no evidence that D-tryptophan increased the synthesis of 5-hydroxytryptamine (5-HT) in dog brain as judged by the failure to cause a change in 5-hydroxyindoleacetic acid (5-HIAA) concentrations in ventricular CSF different from that seen with controls. 3 There was no appreciable conversion of D-tryptophan to L-tryptophan in the dog. 4 D-tryptophan was cleared more rapidly from plasma than L-tryptophan. 5 No difference in plasma binding between D and L-tryptophan was detected.
