[Thoracic radiography in detecting heart diseases].

Within the scope of a physical examination, more than 147,000 patients from the Eastern part of France, were examined and had a chest X-ray, between 1982 and 1987. Seven physicians read those radiographs and reported their findings. After discussion of an interindividual variation noted between the seven physicians, the authors present the major findings of this study: relation between a cardiovascular anomaly discovered on the radiographs and a high blood pressure; relation between a cardiovascular anomaly discovered on the radiographs and one or several electrocardiographic anomalies; relation between anomaly of the cardiovascular silhouette as seen on the radiographs and increased levels of triglycerides and cholesterol in young adults.

[Neuro-endocrine carcinoma of a Bartholin's gland].

This is a case of a 38 year old woman in whom a small basophil cell tumour occurred with inguinal and iliac lymph node metastases followed by pulmonary metastases and extension of the tumour into the vagina and the bladder. The patient died eight months after having started multiple chemotherapy and radiotherapy with telecobalt. Extra studies were carried out ten years later on the cells. This made it possible to diagnose the condition as a neuro-endocrine carcinoma rich in serotonin.

Subcutaneous granuloma caused by Phialophora richardsiae: case report and review of the literature.

A phaeomycotic cyst in a 47-year-old man, caused by Phialophora richardsiae, was treated successfully by excision. A critical review of the literature indicates that the pathological course of P. richardsiae infections usually follows a similar pattern to that of the present case, viz. generation of a well-defined and limited nodule following traumatic subcutaneous introduction of the fungus. The nodule typically accumulates a viscous yellow fluid. The infection remains localized, often being encapsulated by a fibrous layer; adenopathy is not observed. Concomitant bacterial infections are also common; in the present case Pseudomonas stutzeri was identified. Neither bacterial nor fungal infection recurred after surgery.

Patterns of healing on replanted baboon incisors coated with an allogeneic fibrin-fibronectin protein concentrate.

This study investigated the use of an allogeneic fibrin-fibronectin protein concentrate (AFFP) in preventing ankylosis and root resorption on replanted teeth during healing. In 2 adult male baboons (Papio ursinus) after extraction of the mandibular incisors, the coronal two-thirds of the exposed roots were planed to remove the remnant of the periodontal ligament and the cementum. The exposed dentinal surfaces were demineralized with citric acid at pH 1 for 3 min and notched at the junction between the planed and non-planed surfaces. Before replantation, the experimental roots and alveoli were coated with the AFFP prepared from pooled fresh-frozen baboon plasma. The animals were killed 55 days after operation and histometrical analysis was performed on serial sections cut in the buccolingual direction parallel to the long axis of the recovered specimens. An analysis of variance demonstrated significant differences between the two treatments with regard to the magnitude of ankylosis (greater for citric acid-demineralized plus AFFP-treated surfaces). There were variations in the magnitude of the different patterns of healing both between and within the experimental and control surfaces. Within the limits of the study, in this primate replantation model, the biochemical treatment of surgically exposed and demineralized root surfaces with AFFP did not enhance connective tissue attachment regeneration nor prevent dentoalveolar ankylosis and root resorption.

[Blood pressure variations from clinostatism to orthostatism].

The reactivity of blood pressure (BP) and heart rate (HR) to change from supine to standing position was examined in 257 healthy adults, adolescents and children divided into equivalent group of both sexes. BP and HR were measured every minute during 14 minutes in a supine position then during 5 minutes in a standing position with an automatic device using the oscillometry method. A second measurement session was repeated two weeks later. On most patients an increase of diastolic BP (7.73 +/- 4.34 mmHg) and HR (16.40 +/- 7.47/mn) was observed after standing whereas systolic BP modifications were inconstant (2.19 +/- 5.55 mmHg). A comparison of variability indices according to postural state showed a significant higher variability index in a standing position only in female subjects. There was no difference according to age groups. Reproducibility of diastolic BP and HR increase is good two weeks later in respectively 65 p. 100 and 80 p. 100 subjects but it appears very poor for systolic BP. The results in healthy subjects would be compared to those observed in hypertensives to determinate if a postural change test may provide a clinical simple way to discriminate abnormalities in blood pressure regulation.

[Primary or secondary malignant tumors of tonsillar cysts. Apropos of a case of occult papillary cancer of the thyroid disclosed by intracystic metastasis].

The development of a malignant process within a tonsillar cyst is controversial. This phenomenon is exceptionally rare, at present only three cases which have been compatible with anatomo-clinical diagnostic criteria have been clearly established. Based on an original case showing a secondary papillary thyroid cancer in a tonsillar cyst, the authors analyse, in the light of recent data from worldwide literature, the various stages in the diagnostic process in both primary and secondary malignant tumours. The therapeutic and prognostic aspects of the two tumour varieties are also discussed.

[Leukoplakia of the oral cavity].

The definition of oral leukoplakia is presently under revision. Most authors do not include anymore the clinical conditions known as cheek and lip biting, frictional keratosis, actinic cheilitis, smoker's palate and other lesions attributed to tobacco, e.g., snuff dipper's lesion. An epidemiological review reveals a prevalence ranging from 0.2 to 11.7% among various population groups. As malignant transformation rate of leukoplakia increases up to 17.5% with time, there is a need for continued observations no matter how long the lesions have been present. Among the aetiological factors, smoking is probably the most important, but industrial hazards in certain factories are responsible for a high prevalence of oral leukoplakia. The tongue and the floor of the mouth are predilection sites for carcinomatous transformation. A histological examination is essential to the diagnosis or confirmation of this premalignant lesion.

Kinetics of interleukin-1 production by macrophages during infection with Listeria monocytogenes.

Production of interleukin-1 (IL-1) by peritoneal macrophages from mice inoculated intravenously with Listeria monocytogenes was measured at increasing intervals of infection. IL-1 activity in the 24 h macrophage supernatants was determined by using the thymocyte PHA co-mitogenesis assay. IL-1 production increased as the infection progressed, reached a peak on the 9th or 10th day and then declined progressively to approach normal values by the 20th day. Our data on the kinetics of IL-1 levels during an acute infection with L.monocytogenes are discussed in relationship to the development of cell-mediated immunity and its regulation by macrophages.

Effect of imipenem (N-formimidoyl-thienamycin) on the in-vitro lymphocyte proliferation.

Imipenem, a new carbapenem, was tested for its inhibitory or stimulatory effects on concanavalin A- and phytohaemagglutinin-stimulated and unstimulated proliferation (measured by 3H-thymidine uptake) of murine splenocytes and thymocytes. Addition of imipenem at concentrations of 10-50 mg/l induced lymphocyte transformation in unstimulated and mitogen-stimulated lymphocytes. Since imipenem stimulated blastogenesis at concentrations achievable in serum when used therapeutically, these results may have potential clinical significance.

[Variability of arterial pressure and heart rate measured at two periods of 15 minutes to 15 days intervals].

Intra individual blood pressure (BP) and heart rate (HR) variations and their possible correlation with sex and age were evaluated in 271 healthy adults, adolescents and children divided into equivalent groups of both sexes. BP and HR were measured every minute during 14 minutes with an automatic device using the oscillometry method. A second measurement session was repeated two weeks later. On most patients, a decrease of BP was observed during the first minutes towards a point of relative stability, which is reached after the eighth measurement. Mean differences between the first and the fourteenth minutes is about 12 mmHg for systolic BP and 18 mmHg for diastolic BP. The mean values of the 14 determinations on day 1 and day 15 showed a significant correlation from 65 to 75 according to different groups. The variability index within one subject was almost similar in all groups. A comparison of the variability indices observed on day 1 and day 15 showed a significant correlation only for systolic BP in male and for HR in female subjects. Reproducibility of variability appears very inconstant. The results suggest that this procedure cannot characterize subjects with a high BP variability. This study suggests that fourteen repeated measurements of BP values over a fortnight period allows a better estimation of BP level at rest. It cannot be equivalent to 24 hour BP record to estimate variability.

Progressive changes of Kaposi's sarcoma of the gingiva and palate. Case report in an AIDS patient.

As the incidence of acquired immunodeficiency syndrome (AIDS) cases is rapidly increasing, it is expected that the oral lesions of Kaposi's sarcoma (KS) related to AIDS will be encountered more frequently. The progressive changes of KS of the gingiva and palate are described in an AIDS patient who has been monitored for 7 months. In the initial stage, the oral manifestations of KS can be confused with various other lesions. The histopathological appearance includes proliferating slit-like vascular channels and scattered macrophages containing numerous hyaline, eosinophilic granules. Rapid neoplastic proliferation seems to be related to cyclical episodes of impaired immunity.

Periodontal manifestations of acute autoimmune thrombocytopenic purpura. A case report.

Purpuric hemorrhages are usually the result of thrombocytopenia, but are also seen in vascular disorders such as vitamin C deficiency, Henoch-Schonlein purpura, and von Willebrand's disease. The progressive purpuric hemorrhages in the oral mucosae and skin in a case of acute autoimmune thrombocytopenic purpura (AITP) in a young adult are described. Localized hemorrhagic periodontal lesions of doubtful interpretation proved to be the first clinical signs of the underlying thrombocytopenia. The diagnosis is confirmed by finding thrombocytopenia with normal or increased numbers of megakaryocytes in the bone marrow. Treatment must be started before the onset of serious complications such as cerebral hemorrhages.
