Yeasts at different mucosal sites in patients with carcinoma cervix before and following radiotherapy.

The yeasts isolated from four superficial sites and blood were studied before and during radiotherapy in patients with carcinoma of the cervix. Significant yeast growth increased markedly in all four superficial sites during the 1st and 2nd week following radiotherapy. By the 4th week the count had decreased in all samples except in stool of 4 patients. Candida albicans was the most frequent isolate (54.3%) followed by C. tropicalis (28.3%).

Erythrocytic glucose-6-phosphate dehydrogenase deficiency in the Bania community of Punjab.

Five hundred members belonging to the Bania community of Punjab were screened for erythrocytic glucose-6-phosphate dehydrogenase (G-6-PD) deficiency. The incidence of enzyme deficiency in males was 2.84 per cent and in females 2.75 per cent, with an overall incidence of 2.80 per cent. No correlation between age and G-6-PD deficiency was found. The mean values for haemoglobin and haematocrit did not differ significantly in the normal and deficient subjects. Study of the deficiency pattern amongst family members of the enzyme deficient subjects confirmed the X-linked inheritance of G-6-PD deficiency.

Prevalence of R-plasmids and serotypes in intestinal Escherichia coli.

A total of 150 strains of E. coli isolated from faecal samples of normal population (Group I), hospital personnel (Group II) and hospitalised patients (Group III) were studied for the transfer of R-plasmids. Auto-transferable R-plasmids were observed in 72.0, 82.0 and 80.0 per cent of groups I, II and III respectively. 77.0 per cent strains were typable. 04 was the most common serotype found in groups I and II, 0134 in group III.

99mTc-labelled trimethylmonoiodo-IDA: a new radiopharmaceutical for hepatobiliary imaging.

A new radiopharmaceutical, 99mTc-3-iodo-2,4,6-trimethylphenylcarbamoylmethyl iminodiacetic acid (99mTc-trimethylmonoiodo-IDA) was prepared and evaluated for hepatobiliary scintigraphy. The new agent has less plasma protein binding capacity and greater lipophilicity as compared to that of di-isopropyl-IDA. Hepatobiliary specificity, a short transit time and a high gall bladder-to-liver ratio of the new agent were evident from tissue distribution data in mice. Blood tmax and clearance t1/2 of the radiopharmaceutical were observed at 2 and 3.5 min respectively. The hepatic peak-time tmax (5 min) and release time t1/2 (12.75 min) and release t1/2 (25.00 min) suggest the suitability of this agent for hepatobiliary imaging. Results of hepatobiliary scintigraphy in rabbits have further confirmed the above view.

Outbreak of Salmonella worthington meningitis & septicaemia in a hospital at Chandigarh (north India).

During August 1988 an outbreak of hospital acquired infection due to S. worthington has been reported at the Nehru Hospital, Postgraduate Institute of Medical Education and Research, Chandigarh. A total of seven neonates presented with the clinical features of meningitis and septicaemia during this outbreak and six babies died. S. worthington was isolated from blood and cerebrospinal fluid respectively. The same strains were isolated from the baby warmer mattress, baby cot, suction machine bottle and wall of the fridge. Samples from doctors, nurses and apparently healthy babies born during this period did not grow the above organism. This appears to be the first report on S. worthington in human beings from India. The outbreak was controlled by thorough cleaning and fumigation. The organisms were also mostly sensitive to antibiotics used, in contrast to the multiple drug resistant pattern reported from elsewhere.

99mTc(Cu)-mannitol complex: a new agent for dynamic renal function studies.

Mannitol has been labeled with 99mTc by using cuprous chloride as a reducing agent. Blood and kidney clearance of 99mTc(Cu)-mannitol was slightly faster than that of 99mTc(Sn)-DPTA in rat and maximum radioactivity ratio of kidneys to blood was 84.6 at 5 min. A comparative study of 99mTc(Cu)-mannitol, 99mTc(Sn)-DTPA was made in rabbits by taking serial images of kidneys and bladder with a gamma camera. Results show superiority of 99mTc(Cu)-mannitol over other agents for dynamic renal function studies.

Benign disease of the common bile duct.

The incidence of common bile duct (CBD) pathology in a group of patients with benign biliary disease (n = 505) was found to be 23.2 per cent. The spectrum included 111 patients (90.2 per cent) with CBD stones, 37 of whom (33.3 per cent) had no symptoms or findings pre-operatively indicating CBD involvement. Five patients had papillary stenosis, three had postoperative CBD strictures, one had a choledochal cyst and one had an external biliary fistula. Of the 100 CBDs measuring more than 10 mm in diameter, 90 harboured calculi. In the remaining 23 CBDs measuring less than 10 mm, calculi were present in 21. The presence of CBD calculi was demonstrated by intra-operative cholangiography in 49 patients. In the remaining patients (n = 74), the diagnosis of CBD pathology was made either by percutaneous transhepatic cholangiography, endoscopic retrograde cholangio-pancreatography, T-tube cholangiography or peroperative palpation. The surgical procedures performed included choledochotomy and T-tube drainage (n = 74), transduodenal sphincteroplasty (n = 27) and choledochoduodenostomy (n = 18). The overall mortality and morbidity of CBD exploration was 3.3 per cent and 24.4 per cent respectively, which was significantly greater than that for cholecystectomy alone (0.3 per cent and 8.6 per cent respectively). Transduodenal sphincteroplasty carried a much higher mortality (11 per cent) and morbidity (52 per cent) when compared with other procedures.

gamma-Glutamyl transpeptidase: a novel biochemical marker in inflammation.

gamma-Glutamyl transpeptidase (gamma-GT) plays an important role in the turnover of glutathione and protein biosynthesis. Because in inflammation both catabolic and anabolic steps are activated together with migration of cells, an alteration in gamma-GT activity was postulated to occur at the site of inflammation with the development of the inflammatory process. We discovered that gamma-GT activity was increased markedly at sites of inflammation produced in several ways in rats. A significant increase in enzyme activity appeared soon after the induction of inflammation. In carrageenin-induced acute inflammation, the paw tissue attained a 3- to 4-fold increase in enzyme activity within 4 hr; in established adjuvant arthritis, a 20- to 24-fold increase over its basal activity occurred in the rat paw tissue. The specific enzyme activity was 20-22 nmoles/min/mg protein in the cellular sediment of carrageenin-induced pleural exudate. In cotton granulomatous tissue it was 5- to 6-fold higher compared to the enzyme activity of the skeletal muscles. The in vivo increase in gamma-GT activity was prevented from occurring in proportion to the anti-inflammatory potencies of the test drugs given orally. The prevention of enzyme activity was observed with indomethacin in carrageenin-induced edematous paw tissue and with phenylbutazone in both adjuvant arthritis and carrageenin-induced pleural exudate. Prednisolone was observed to be the most potent drug against cotton granuloma. Nonsteroidal anti-inflammatory drugs (NSAIDs) were not found to affect enzyme activity in vitro when incubated with cellular infiltrate from a cotton pellet granuloma. Differences in certain physico-chemical characteristics, viz. stability at 50 degrees, pH dependency and effects of solvents, were not discernible in between the enzyme activities of the untreated and edematous paw tissues. The studies thus suggest that measurement of gamma-GT in inflammation may prove to be a valuable biochemical marker for the assessment of anti-inflammatory activity of drugs in vivo.

99mTc-Sn-acetylcysteine: a new renal scanning agent.

A new radiopharmaceutical, 99mTc-Sn-acetylcysteine, for renal scanning is reported. The preparation is simple and quick. It can also be adopted in the kit form. The complex has the advantage of great stability and fast clearance over earlier reported renal scanning radioagnostic agents.

Candidaemia: a 10-year study in an Indian teaching hospital.

Retrospective evaluation of candidaemia patients was performed in an Indian teaching hospital over a 10-year period. The incidence of patients with candidaemia increased eleven-fold in the second half of the study period (55 patients) compared with the first half (5 patients). Haematological malignancies (11 patients), neonatal septicaemia (9), cardiac abnormalities and cardiac surgery (9) were the commonest underlying diseases in these patients. Candida albicans (50%), C. guilliermondii (17%), C. tropicalis (15%) and C. parapsilosis (8%) were the most common fungal pathogens isolated from blood culture. Therapy with two or more antibiotics (92%), corticosteroid administration (25%), intravascular catheter use for over 24 h (78%) and neutropenia (48%) were the accountable predisposing factors. Prolonged hospitalization (mean average 22.2 days as compared with 11.2 days in other patients) was an added risk factor in these patients.
