[Binding of complement by anti-native desoxyribonucleic acid antibodies in systemic lupus erythematosus].

Antibodies directed against double-stranded (native) desoxyribonucleic acid (dsDNA) were detected by the immunofluorescence technique on Crithidia luciliae in 38 out of 58 patients with systemic lupus erythematosus (SLE) and in 22 out of 296 controls with autoimmune diseases. The SLE patients had high titers of anti-dsDNA antibodies which belonged at least to the IgG class and fixed the complement in 20 out of 38 cases. These complement-fixing antibodies differed from the others in their greater activity, as evaluated by C3 levels, and in their association with a high incidence of renal lesions.

Immunological effect of silica dust analyzed by monoclonal antibodies.

We found a significant decrease of OKT8 + ve cells in silicosis patients (18.1%), but also in unaffected exposed workers (19.0%), when compared with sex- and age-matched controls (22.8%). The proportion of OKT8 + ve cells was significantly lower in subjects with antinuclear antibodies (15.7%) and in those with IgG-rheumatoid factors (16.3%).

[Pure hypertensive nephropathy associated with malignant arterial hypertension in disseminated lupus erythematosus. Case with a favorable outcome].

The authors report the association of a clinically florid form of lupus with malignant hypertension and renal failure. The renal histology revealed lesions of nephroangiosclerosis, with virtually no signs of lupus proliferative glomerulonephritis. With a follow-up period of 42 months, the renal function has returned to normal, although steroid treatment has been interrupted for 33 months. However, intensive antihypertensive treatment is still required. This unusual case is discussed in the light of previous reports in the literature of the association of disseminated lupus erythematosus and malignant hypertension, which is nevertheless a rare entity.

Diminished bactericidal activity in megalocytic interstitial nephritis.

The first case of favorable evolution under conservative treatment for acute renal failure caused by megalocytic interstitial nephritis and the ongoing transitory impairment of the polymorphonuclear bactericidal ability are described. This case report has been included in a critical overview of the cases previously reported.

Ventilatory and occlusion pressure responses to hypercapnia in patients with chronic renal failure.

Little is known of the effect of chronic renal failure (CRF) on ventilatory regulation. In 38 subjects (19 healthy, 19 with CRF before and after dialysis), we performed measurements of ventilation (VE) and occlusion pressure (P0.1) while the subjects were breathing air and hypercapnic gas mixtures. The results have shown that (1) during air ventilation, CRF patients exhibited lower values of VE and P0.1, which returned to normal after dialysis; (2) during hypercapnic ventilation, CRF patients had the same response as healthy subjects for VE but higher P0.1; hemodialysis induced an upward shift of the CO2 response curve in CRF patients. A twofold mechanism is probably involved: pulmonary edema, which reduces lung elasticity, and neuromuscular hypoexcitability, both implying a stronger central command.

Lowered Fc IgG receptor-bearing T lymphocytes correlate with non-organ-specific autoantibodies in silicosis.

A survey of silicosis patients and people exposed to silica dust was set up in an effort to look for any relationship between humoral and cell-mediated autoimmune phenomena. It was found that in both sets of subjects, the level of Fc IgG receptor-bearing T lymphocytes was significantly reduced, there was also an inverse correlation between these cell concentrations and IgG- and IgM-circulating immune complex levels. In addition, raised levels of various autoantibodies were found in both groups, however, none of the exposed subjects has developed silicosis to date.
