Craniofacial identification by computer-mediated superimposition.

Mass disasters are associated with a large number of fatalities, with victims being visually unidentifiable in most cases. Dental identification, although being an important and valuable identification method, is subject to the availability and quality of antemortem and postmortem dental records. This paper presents a simple-to-use method of human identification using an antemortem photograph showing anterior teeth with superimposition onto a postmortem image using specific features of Adobe Photoshop. We present cases and discuss the benefits and difficulties of this method.

Variations in chemical mimicry by the ectoparasitic mite Varroa jacobsoni according to the developmental stage of the host honey-bee Apis mellifera.

The ectoparasitic mite Varroa jacobsoni poses a major threat to the survival of European honey-bee populations. Development of effective control methods is therefore much needed. Study of interspecific chemical communication between the parasite and host is a particularly promising avenue of research. Previous study has shown that the cuticular hydrocarbons of the parasite mite Varroa jacobsoni are qualitatively identical to those of its honey-bee host Apis mellifera (Nation J.L., Sanford M.T., Milne K., 1992. Cuticular hydrocarbons from Varroa jacobsoni. Experimental and Applied Acarology 16, 331-344). The purpose of the present study was to compare the cuticular hydrocarbon patterns of the two species at different stages of bee development. Cuticular components were identified by gas chromatography/mass spectrometry. The proportion of each component was calculated at three stages of bee development (larvae, pupa, emerging bee). The degree of chemical mimicry between the parasite and host was evaluated by multivariate analyses using the resulting proportions for each category of individuals. There were four main findings. The first was that the proportions of some components are different at the larval, pupal and imago stage of bee development. Second, Varroa profiles vary depending on the developmental stage of the host. Third, the cuticular profile of adult mites is more similar to that of the stage of the host than that of later and/or earlier stages except for parasites collected from emerging adult bees. Fourth, the degree of mimicry by Varroa is greater during larval and pupal stages than during the emerging adult bee stages. The role of chemical mimicry - although it is not perfect - in enabling parasites to infest bee colonies by the parasite is discussed.

Elevated activity and expression of Src-family kinases in human breast carcinoma tissue versus matched non-tumor tissue.

Src-family kinase expression was measured in 52 human mammary tumor (T) specimens compared with non-tumor (NT) tissue from the same patient by enzymatic assays employing a Src-kinase family-specific peptide substrate and by immunoblotting with an antibody recognizing the Src-family kinases Src, Fyn, and Yes. In the T specimens, the mean enzymatic activity was moderately elevated (T: 160 fmol ATP min-1 mg-1; NT: 115 fmol ATP min-1 mg-1) with 25 tumor samples having higher activity than the corresponding NT tissue, 17 having lower activity, and no activity detectable in ten T/NT pairs. Immunoblotting revealed clearly elevated expression in 25 tumor tissues and no differences or expression below the detection limit in the remaining T/NT pairs. The data are in agreement with a possible role of Src-family kinases for the biology of mammary carcinoma.

Managed care, vertical integration strategies and hospital performance.

OBJECTIVE: The purpose of this study is to examine the association of managed care with hospital vertical integration strategies, as well as to observe the relationships of different types of vertical integration with hospital efficiency and financial performance. DATA AND METHODS: The sample consists of 363 California short-term acute care hospitals in 1994. Linear structure equation modeling is used to test six hypotheses derived from the strategic adaptation model. Several organizational and market factors are controlled statistically. PRINCIPAL FINDINGS: Results suggest that managed care is a driving force for hospital vertical integration. In terms of performance, hospitals that are integrated with physician groups and provide outpatient services (backward integration) have better operating margins, returns on assets, and net cash flows (p < 0.01). These hospitals are not, however, likely to show greater productivity. Forward integration with a long-term-care facility, on the other hand, is positively and significantly related to hospital productivity (p < 0.001). Forward integration is negatively related to financial performance (p < 0.05), however, opposite to the direction hypothesized. CONCLUSIONS: Health executives should be responsive to the growth of managed care in their local market and should probably consider providing more backward integrated services rather than forward integrated services in order to improve the hospital's financial performance in today's competitive health care market.

Randomised controlled trial of nurse practitioner versus general practitioner care for patients requesting "same day" consultations in primary care.

OBJECTIVE: To ascertain any differences between care from nurse practitioners and that from general practitioners for patients seeking "same day" consultations in primary care. DESIGN: Randomised controlled trial with patients allocated by one of two randomisation schemes (by day or within day). SETTING: 10 general practices in south Wales and south west England. SUBJECTS: 1368 patients requesting same day consultations. MAIN OUTCOME MEASURES: Patient satisfaction, resolution of symptoms and concerns, care provided (prescriptions, investigations, referrals, recall, and length of consultation), information provided to patients, and patients' intentions for seeking care in the future. RESULTS: Generally patients consulting nurse practitioners were significantly more satisfied with their care, although for adults this difference was not observed in all practices. For children, the mean difference between general and nurse practitioner in percentage satisfaction score was -4.8 (95% confidence interval -6.8 to -2.8), and for adults the differences ranged from -8.8 (-13.6 to -3.9) to 3.8 (-3.3 to 10.8) across the practices. Resolution of symptoms and concerns did not differ between the two groups (odds ratio 1.2 (95% confidence interval 0.8 to 1.8) for symptoms and 1.03 (0.8 to 1.4) for concerns). The number of prescriptions issued, investigations ordered, referrals to secondary care, and reattendances were similar between the two groups. However, patients managed by nurse practitioners reported receiving significantly more information about their illnesses and, in all but one practice, their consultations were significantly longer. CONCLUSION: This study supports the wider acceptance of the role of nurse practitioners in providing care to patients requesting same day consultations.

Does the spatial distribution of the parasitic mite varroa jacobsoni oud. (Mesostigmata: varroidae) in worker brood of honey bee apis mellifera L. (Hymenoptera: apidae) rely on an aggregative process?

Varroa jacobsoni is an ectoparasite of honey bees which reproduces in capped brood cells. Multi-infestation is frequently observed in worker brood and can be interpreted as an aggregative phenomenon. The aim of this study was to determine whether the distribution of V. jacobsoni in worker brood cells relies on a random or an aggregative process. We studied the distribution of Varroa females in capped worker brood at similar age by comparing, by a Monte Carlo test, the observed frequency distribution of mites per cell to simulated distributions based on a random process. A complementary approach, using the "nearest neighbor distances" (NND) with Monte Carlo tests, was investigated to study the spatial distribution (a) between mites in different cells and (b) between infested cells in brood. The observed distributions did not differ significantly from that expected by a random process, and we conclude that there is no aggregation during invasion of V. jacobsoni in worker brood.

Trends in psychiatric mental health nursing education.

Eighty-five program coordinators responded to a survey concerning Psychiatric Mental Health (PMH) graduate-nursing education. Twelve of these 85 program coordinators reported their programs had closed. Data from the remaining 73 indicated that the number of students entering PMH graduate programs is small. In the past 2 years, most programs have undergone some redesign. A significant curricular trend is the inclusion of pharmacology, physical assessment, and pathophysiology. Another trend is the shift to a Nurse Practitioner (NP)-type curriculum. PMH curricula seem increasingly modeled on a primary mental health care model and training in a broad base of skills.

A case-control study after a hantavirus infection outbreak in the south of Belgium: who is at risk?

Puumala is the most common hantavirus serotype in Europe and is spread mainly by the red bank vole. Between 1 July 1992 and 31 January 1994, an outbreak of Puumala virus-induced nephropathia epidemica (NE) occurred in the Belgian Ardennes. Serologically confirmed cases (n = 41) were compared with two groups of asymptomatic seronegative controls. Risks identified included sighting of living rodents, exposure to rodent droppings, and trapping rodents during the 4 weeks preceding onset of symptoms. Activities during this 4-week period that presented the greatest risk were woodcutting, reopening of a nonaerated room, and strenuous physical effort. This is the first case-control study on risk factors for NE in Europe. In comparison with the American form of hantavirus pulmonary syndrome, which is spread by deer mice, professional activity appears to be a more important risk factor for acquisition of hantavirus in Europe.

Heterologous myb genes distinct from GL1 enhance trichome production when overexpressed in Nicotiana tabacum.

Myb-class transcription factors implicated in cell shape regulation were overexpressed in Arabidopsis thaliana and Nicotiana tabacum in an attempt to assess the extent to which cellular differentiation programs might be shared between these distantly related plants. GLABROUS 1, a myb gene required for trichome development in Arabidopsis, did not alter the trichome phenotype of the tobacco plants in which it was overexpressed. MIXTA, which in Antirrhinum majus is reported to regulate certain aspects of floral papillae development, did not complement the glabrous 1 mutant of Arabidopsis. However, 35S:MIXTA transformants of N. tabacum displayed various developmental abnormalities, most strikingly production of supernumerary trichomes on cotyledons, leaves and stems. In addition, floral papillae were converted to multicellular trichomes. CotMYBA, a myb gene which is expressed in Gossypium hirsutum ovules and has some homology to MIXTA, was also overexpressed in the two species. A similar but distinct syndrome of abnormalities, including the production of cotyledonary trichomes, was observed in 35S:CotMYBA tobacco transformants. However, CotMYBA did not alter trichome production in Arabidopsis. These results suggest that the trichomes of Arabidopsis and Nicotiana are merely analogous structures, and that the myb genes regulating their differentiation are specific and separate.

Low back pain.

An SOI-I low back pain PTE utilized 2.5 office visits with an adult medicine physician compared with 3.7 office visits with an orthopedic surgeon. Adult medicine physicians performed 1.1 tests and administered 4.6 prescriptions during an SOI-I low back pain PTE. Office visits, tests, and prescriptions were the major cost drivers for low back pain PTEs. Of all low back pain PTEs, 19% received one-drug group, and 17.2% received two-drug group treatments. Nonsteroidal anti-inflammatory drugs and central muscle relaxants were the most commonly used medication therapies for these patients.
