[Phenotype of the mononuclear cells of the cornea in ocular herpes].

There is now a great deal of evidence for cell-mediated immunopathogenesis of herpetic stromal keratitis. However, this has never been unequivocally demonstrated, so far. Accordingly, we established a new technical approach in which lymphocytes were cornea-eluted, further purified and characterized by means of monoclonal antibodies. Four corneal specimens were obtained after penetrating keratoplasty. The patients suffered from herpetic interstitial keratitis with perforation of the cornea. The most relevant point to emerge from this experiment is the marked infiltration of corneal buttons by suppressor/cytotoxic T cells. This finding strongly supports the recent suggestion, by Ahonen and coworkers that lymphocytes might be cytotoxic for keratocytes.

[Early treatment of ocular herpes simplex. Apropos of 6 cases of herpetic conjunctivitis].

Six patients with previous history of herpetic keratitis were seen during the 48 first hours after the out set of a conjunctivitis. Herpes simplex virus type 1 was isolated in all cases. Treated immediately with trifluorothymidine, they healed quickly without developing any corneal lesions. These data suggest the use of early antiviral treatment in patients with recurrent herpetic keratitis, even preventive therapy in at risk patients in well known predisposing conditions.

Delayed cerebral pseudo-tumoral radionecrosis following scalp-tumour irradiation. Case report and review of literature.

A case of delayed radionecrosis of the brain is reported, following irradiation of a scalp dermatofibrosarcoma. Signs and symptoms were those of an intracranial neoplasm and appeared 5 years after an irradiation dosage of 1890 rets. Twenty-seven similar cases were gathered from a survey of the literature. Surgical excision was the treatment of choice and led to cure or improvement in 20 cases. Analysis of doses and fractionation revealed excessive irradiation in all but one of the cases.

[Serologic survey on arbovirus infections in patients presenting with uveitis].

Viruses have been demonstrated or suspected as the causative agents of various types of uveitis. Some arboviruses can be responsible for ocular manifestations. In Brittany, both serosurveys and virus isolations were positive for arboviruses. These results led us to look for the possible role of arboviruses in the aetiology of uveitis. For this, we performed a serosurvey on 131 patients with uveitis of unknown aetiology. Patients were examined in the Ophthalmologic Department of the Brest University Hospital from May 1979 to May 1983. Serum samples were studied: by haemagglutination inhibition (HI) tests against thirteen antigens; by complement fixation (CF) tests against six antigens. The HI and CF tests were performed using micromethods. HI tests revealed antibodies against one or more flaviviruses antigens in thirteen patients: nine monospecific reactions against Yellow Fever virus (as a consequence of a vaccination) and four reactions against several flaviviruses. The four positive reactions are easily explainable by previous trips in countries where several arboviruses are endemic. By CF test we found antibodies at a low titer in two patients: the first one had antibodies against the Zaliv Terpeniya virus and the second against the Avalon virus. Signification of these results is discussed. Our study shows that in Brittany uveitis of unknown aetiology do not seem to be due to arboviruses, at least those tested in this survey or antigenically related.

[Trifluorothymidine therapy of corticoid-treated herpetic keratitis: experimental study in the rabbit].

The combination therapy of corticosteroids and antiviral agent is often necessary in the treatment of different forms of ocular herpes. The association of superficial ulceration with severe stromal inflammation indicates the possible use of ths therapy. An experimental study was performed in rabbits to evaluate if Trifluorothymidine (TFT) used in combination with topical corticosteroids can heal herpetic superficial ulcerations. Biomicroscopic and histopathologic lesions were compared with those of herpetic control eyes, TFT treated eyes and corticosteroid treated eyes. TFT plus corticosteroid treated lesions showed a clinically favourable evolution comparable to that of TFT treated rabbits without corticosteroid. However anatomopathologic examination revealed stromal alterations with herpetic capsids in keratocytes.

[Needles used in ocular microsurgery: comparative study using scanning electron microscopy].

The use of microsurgical sutures of various types reveals noticeable differences in the ease of penetration of needles into ocular tissues. In order to try to specify these variations, we have examined different needles used in ophthalmology with a scanning electron microscope: CU 1, CU 5 ( Alcon ), LE 1 (Davis Geck), GS 9, GS 17 (Ethicon), SSC Dm 122 (T ubinger material). The examination was performed on new, unused needles and after having been passed through a cornea five times. Studies on new needles showed certain structural variations. Although the tips had a more or less sharp stem, the edge varied from completely regular to notched. The needle surfaces varied from a very smooth appearance to fluted . After use, the examination reveals the presence of cellular debris. No irregularity was caused by contact with surgical instruments. Comparisons between the imprint and the electron micrographic appearance showed that penetration is clearly better with needles having fluted edges and surfaces.

[Fate of artificial lenses: a scanning electron microscopy study].

Thirty one intraocular lenses removed from human eyes 2 months to 30 years after implantation were examined by scanning electron microscopy. Binkhorst 4 loop lenses (12) and 2 loop lenses (6), and Choyce lenses (6) were the most prevalent styles examined. Most of the lenses produced before 1978 were found to have manufacturing defects (mold disalignment, unfinished gate areas, etc.). More recent lenses showed an improvement in the quality of the surface. The lenticular, nonimpinging portion of the lenses made with polymethylmethacrylate did not demonstrate signs of degradation. However the holes in the edges of the lenses were rough and poorly finished with obvious irregularities. On the other hand, the lens foot of a Choyce lens revealed severe unusual abnormalities: the plastic surface changes may be consistent with degradation. Examination of explanted iris clip lenses showed degradation of the polypropylene loops in two cases: the loops were found to have fractured surfaces.
