Exfoliative dermatitis due to nifedipine.

We report the case of a 63-year-old Caucasian female, with reno-vascular hypertension, who developed a generalized skin eruption with lichenoid histological features, induced by nifedipine. A repeat challenge with nifedipine confirmed that it was the causative agent.

Darkening of white hair in Parkinson's disease.

We describe a patient with long-standing Parkinson's disease who noted that his white hair turned grey and darkened 8 months after the addition of carbidopa to his established levodopa (L-dopa) therapy and 4 months after the introduction of bromocriptine.

Cutaneous reaction to diltiazem resulting in an exacerbation of angina.

A 77-year-old woman developed a widespread erythematous rash after treatment with the calcium antagonist, diltiazem, which resulted in an exacerbation of the angina for which the drug was originally described. Drugs which may give widespread cutaneous side-effects should be used with caution in patients with compromised cardiac function.

The effect of prostaglandins F2 alpha and E2 on the adrenergic activity of the uterus of laying hens (Gallus domesticus) in vitro.

The effect of PGF2 alpha and PGE2 before or after administration of isoproterenol or norepinephrine was studied on uterine strips of White Leghorn laying hens. Prostaglandins (0.2 ng/ml of bath solution) always produced an increase in tension and area under the tracing, the effect of PGF2 alpha being significantly marked especially when administered after norepinephrine. Norepinephrine and isoproterenol (0.1 microgram/ml of bath solution) produced a marked decrease in area under the tracing, the effect of isoproterenol being more potent especially when administered before prostaglandins.

Safety and effectiveness of a portable external automatic defibrillator-pacemaker.

Sudden cardiac death in the community can be reduced by early resuscitation. The commonest arrhythmia encountered in this setting is ventricular fibrillation. A portable external automatic defibrillator-pacing machine for use by emergency services and laymen after basic training, has recently been introduced. This device has a tongue-abdominal pathway for sensing the electrocardiogram and respiration, as well as delivery of current. In its automatic mode, the machine cannot defibrillate unless the patient's breathing and gag reflex are virtually absent. The decision-making characteristics of the machine were defined in isolation from the patient using simulated ECG signals and recordings of arrhythmias obtained during cardiac surgery and during electrophysiological studies. The pathway was evaluated separately by attaching it to a conventional defibrillator and using it in the elective cardioversion of 15 patients. Electrode sites were examined and creatine phosphokinase (CPK) and creatinine phosphokinase isoenzyme (CPKMB) release determined following conversion. Repeated playback of the same 34 minutes of selected recordings showed that decisions were consistent 95% of the time. Automatic pacing always occurred with asystole. ECG signals less than 0.35 mV in amplitude were not recognized, but a manual override switch could be operated in those circumstances where there was failure to act upon fine ventricular fibrillation. Defibrillation of 50% of the recordings of supraventricular and ventricular tachycardia did occur, but this could not have happened in practice unless the patient was unconscious, and in addition, the operator failed to countermand this decision. Prevention of epinephrine and stress-induced egg laying delay by feeding propranolol to the laying hen.

White Leghorn laying hens fed propranolol (2 to 8 mg/100 g food) were injected subcutaneously with epinephrine (.5 to 4.0 mg) or saline (4.0 ml). The time interval between first and second ovipositions was delayed about 9 to 14 hr in the epinephrine-injected hens and about 4 hr in the saline-injected hens as compared to control hens. When propranolol was added to the food about 24 hr prior to epinephrine or saline injections, delayed ovipositions were prevented. Ovulation occurred in hens having an egg in the shell gland (uterus) after epinephrine injections. These results suggest that propranolol may be added to the food to prevent a fall in laying during stress.

Distribution of adrenergic receptors in the domestic fowl oviduct.

The effect of epinephrine, phenoxybenzamine (alpha-receptor blocker), and propranolol (beta-receptor blocker) on the oviductal motility in an in vitro system was investigated in strips from infundibulum, magnum, isthmus, and uterus from laying hens. Epinephrine resulted in a significant (P less than .05) contraction of both infundibulum (+166.6%) and magnum (+70.3%) and relaxation of the uterus (-56.9%), while the response of the isthmus was inconsistent. Epinephrine induced contractions in propranolol blocked strips in all four oviductal segments studied (infundibulum, +138.1; magnum, +70.4; isthmus, +35.9; and uterus, +30.1%). Epinephrine, in phenoxybenzamine-blocked strips, resulted in relaxation of all segments (infundibulum, -27.5; magnum, -40.5; isthmus, -63.4; and uterus -53.6%). These results suggest that the activity of alpha-excitatory adrenergic receptors is greater in the anterior than in the posterior segment of the oviduct. conversely, greater activity of beta-inhibitory receptors seems to be present in the posterior than in the anterior segment of the oviduct.

The fractionation of adenosine-rich oligoribonucleotides on polyethyleneimine-cellulose.

After combined pancreatic and T(1) ribonuclease treatment of RNA, a characteristic series of products of the type A(n)N are obtained, where N can be any of the four ribonucleosides. Depending on whether phosphatase treatment is used before the addition of labelled phosphate at the 5'-terminus with bacteriophage phosphokinase, the labelled oligonucleotides obtained may or may not possess a phosphate group at the 3'- as well as the 5'-end. The behaviour of these characteristic products after electrophoresis on cellulose acetate strips followed by chromatography on polyethyleneimine-cellulose thin-layer plates was examined.

A nursing management minimum data set. Significance and development.

Building on Werley's nursing minimum data set, the authors identified a nursing management minimum data set (NMMDS). This collection of core data elements is needed by nurse administrators to make management decisions and compare effectiveness across institutions.
