Radiation dose to the lens in sequential and spiral CT of the facial bones and sinuses.

A tendency for over-utilization of the radiological examinations of facial bones and sinuses has been observed. The radiosensitive lens is exposed to radiation during such examination. Comparison of routine roentgenography and spiral axial and coronal CT in sequential and spiral acquisition modes with different mAs values was undertaken using Alderson Rando Phantom. The spiral-CT technique with pitch 1.5 performed in the coronal plane gave the lowest radiation dose to the lens as compared with other CT methods and only seven times higher than routine sinus roentgenography.

MR and other imaging methods in the investigation of mycetomas.

PURPOSE: To report features of mycetomas (actino- and eumycetoma infection), which belong to the so-called rare bone infections, as evaluated by MR and other imaging methods and to assess chemotherapy treatment. MATERIAL AND METHODS: Twenty patients (average age 57 years) were diagnosed by different modalities. Analyses of 57 plain films, 31 three-phase bone scans, 28 CT and 35 MR examinations were performed. The MR protocol included T1-weighting without and with contrast, T2-weighted, proton density and fat suppressing sequences. RESULTS: Signs of chronic osteomyelitis were seen in plain films of 10 patients (50%), but the healing process was difficult to assess. Bone scintigraphy was positive in 12 patients (60%). In 14 patients (70%), CT could demonstrate bone lesions, but the healing process was difficult to estimate in 4 patients. MR imaging provided accurate diagnostic information in 15 cases (75%). In 16 patients (80%), small lesions could be identified due to grains, which seemed to differentiate mycetoma from other infections and tumorous lesions. MR examination gave definitive answer about the healing process in 18 cases (90%). CONCLUSION: MR investigation was superior to the other imaging techniques in the evaluation of mycetoma and the assessment of therapy.

Cutaneous nocardiosis of the chest wall and pleura--10-year consequences of a hand actinomycetoma.

We report an unusual case of primary cutaneous nocardiosis due to Nocardia otitidiscaviarum presenting first as a mycetoma of the right hand and wrist. The patient refused treatment and was lost to follow-up until he showed up 10 years later with numerous discharging large sinuses and abscesses on the upper right quadrant of the chest wall and in the right armpit. Roentgenograms revealed pleural masses. Histology was in keeping with the diagnosis of mycetoma. Treatment with amikacin, rifampicin and co-trimaxole proved to be successful.

Intracardiac tumour and brain lesions in tuberous sclerosis. A case report of antenatal diagnosis by ultrasonography.

A case of antenatal tuberous sclerosis was diagnosed by ultrasonography. Intracardiac tumour (highly suspected rhabdomyoma) with transitory heart failure and multiple brain lesions were observed. After delivery, echocardiography, spiral CT and MR imaging diagnosis of tuberous sclerosis was confirmed by typical skin lesions (depigmented macules) and development of seizures.

Conventional radiography and ultrasonography in the diagnosis of small bowel obstruction and strangulation.

Ninety-six patients, 45 men and 51 women (mean age 39 years, range 13- 90 years), with clinically acute abdomen were examined by conventional abdominal radiography and ultrasonography during a period of one year. Ultrasonography was performed with a linear transducer, which permitted study of morphology and motility of small bowel loops: distention, paralysis, intramural thickening, and extraluminal fluid. Nineteen cases of mechanical obstruction (9 simple and 10 of strangulation type) were observed. In the strangulation group ultrasonographic findings were positive in 91% whereas conventional radiography solely was positive in 30%. In the simple obstruction, 89% and 78%, respectively, were correctly diagnosed by the 2 techniques. Ultrasonography is recommended as a routine examination beside conventional abdominal films in acute abdomen.

[Biocorundum--a new/type of poreless ceramic material of aluminum oxide for the manufacture of the elements of hip joint prosthesis. Biological and technological studies].

The Institute of Refractories Materials in Gliwice undertook researches in the field of technology of a new ceramic material of aluminium oxide for production of coxal joint endoprostheses. A poreless ceramic material of high chemical purity and density and required microstructure and good resistance to fracture, was achieved. The new ceramic material was called Biocorundum. By means of a similar technology also series of samples were produced for testing the biocompatibility of the material. Biological studies were carried out in the Department of Experimental Surgery and Biomaterials Research, Chair of Traumatologic Surgery, Medical Academy of Wroclaw. The studies comprised: 1. laboratory and toxicological tests in vitro of water extracts, 2. pathomorphological studies after intramuscular, peritoneal and intra-bone implantation of samples of the material in rats and piglets, 3. ultrastructural scanning studies of the implantation in rats and pigs and after 3- and 6-months long keeping in the Ringer solution. Preliminary studies and biological studies in vitro have shown a complete conformity of the material with the requirements. Tissue reactivity was studied on 36 Wistar rats. By pathomorphological studies a systematic macroscopic and microscopic estimation was possible. They have shown only minimal tissue reactivity to biocorundum implantation. In experimental studies on 14 piglets the biocorundum grafts were implanted into the tibia epiphysis for 3- and 6-months. Clinical, pathomorphological and X-ray studies were carried out. In all cases the Biocorundum grafts healed without any complications. On the basis of pathomorphological studies, the tissue reactivity around the grafts were estimated as minimal. This shows a high biocompatibility of the ceramic material. In X-ray studies were observed around the grafts small osteosclerotic changes and periosteum stratification. These changes resulted after surgical interventions in quickly growing piglets. Complementary studies of the surface of the material by scanning microscopy after 3 months long implantation and after 3 months long contact with the Ringer solution did not show any changes in comparison with the surface of the initial samples. After 6 months long implantation in individual samples rounding of the ceramic material grain edges were found, which could result from biological corrosive power. This agree with the findings of other studies on other ceramic aluminium oxide materials.
