Autointoxication revisited.

The pustular dermatitis associated with small bowel bypass surgery and the cutaneous manifestations of inflammatory bowel disease are well known and generally assumed to be due to the absorption of microbial antigens from the bowel. Monomeric serum IgA is assumed to originate in the gastrointestinal tract, and circulating IgA immune complexes, as seen in dermatitis herpetiformis, should make us suspicious of a gastrointestinal tract source. These circulating immune complexes and perhaps polyclonal increases in serum IgA may be the result of minor perturbations of mucosal permeability or the failure of locally produced dimeric serum IgA to inactivate bacterial or dietary antigens. Such disparate entities as Reiter's syndrome, psoriasis, pyoderma gangrenosum, and ankylosing spondylitis, as well as the pustular eruptions of Behçet's syndrome, pustular psoriasis, and lithium therapy, may share this common pathogenesis.

Ultraviolet A phototherapy in the prophylaxis of solar urticaria.

Five patients with solar urticaria were treated with ultraviolet A (UVA) radiation. Three experienced marked increases in sunlight tolerance, but two patients failed to improve. Subjective clinical improvement was documented objectively by marked increases in the minimal urtication doses obtained by provocative light testing. These results indicate that some patients with solar urticaria may respond to treatment with UVA radiation alone.

Actinic reticuloid. Response to combination treatment with azathioprine, hydroxychloroquine, and prednisone.

We treated an intractable case of actinic reticuloid with azathioprine, hydroxychloroquine, prednisone, and solar protection with E-Z film and obtained a clinically worthwhile effect. The patient was initially sensitive to less than 2 mjoules/cm2 ultraviolet B (UVB) and less than 1 joule/cm UVA, which caused erythematous and infiltrated scaly plaques that persisted for more than 6 weeks after testing. The case is also instructive in the difficulty presented in differentiating it from mycosis fungoides, and for the presence of Sézary-type cells in skin, blood, and lymph nodes.

Inflammatory linear verrucous epidermal nevus. Epidermal protein analysis in four patients.

Inflammatory linear verrucous epidermal nevus (ILVEN) may be difficult to differentiate clinically from psoriasis. We report four cases of ILVEN that confirm the observation that the analysis of epidermal proteins by sodium lauryl sulfate-polyacrylamide gel electrophoresis (SLS-PAGE) discloses a pattern different from that seen in psoriasis. We have also found, however, that the SLS-PAGE epidermal protein pattern is not identical in each case of ILVEN.

The Koebner (isomorphic) response in psoriasis. Associations with early age at onset and multiple previous therapies.

Among 1,253 patients with psoriasis who were enrolled in a study of photochemotherapy, 33% had a history of a Koebner response (KR) at the site of physical trauma. The early onset of psoriasis and previous treatment with multiple types of therapy were each independently associated with the KR. Among patients with both onset before age 15 years and previous treatment with four or more types of therapy, 75% reported the KR, compared with only 5% of patients with onset after age 30 years and no previous therapy. At the time of enrollment in the photochemotherapy study, the history of the KR was not associated with worsening, the duration of psoriasis, or the extent of skin surface involved. Patients with the KR may manifest their disease earlier in life or may be more prone to flare after any type of treatment.

Papular polymorphic light eruption: an immunoperoxidase study using monoclonal antibodies.

Biopsy specimens of papules taken from eight patients with polymorphic light eruption were examined by immunoperoxidase techniques employing monoclonal antibodies. In each case, most infiltrating mononuclear cells were T cells. The majority of T cells were T8-positive (cytotoxic/suppressor) in four cases and T4-positive (helper/inducer) in two. In two cases, approximately equal numbers of both T cell subsets were present. In only three cases were rare B cells identified by their reactivity with anti-IgM antibody. MI-positive mononuclear cells (macrophages) represented less than 5% of cells infiltrating the dermis. In five subjects, anti-T6 antibody stained increased numbers of dermal mononuclear cells considered to be Langerhans/indeterminate cells. The pathogenesis of papular polymorphic light eruption may involve injury to upper dermal venules mediated by T cells and Langerhans/indeterminate cells.
