Endometriosis of the colon with mixed "germ cell" tumor.

A 42-year-old woman was found to have endometriosis of the oviducts. Five years later she had a hysterectomy and oophorectomy, the abnormal findings at this time being endometriosis with a chocolate cyst of one ovary. At a third operation, seven years after the first, she was found to have endometriosis of the colon, associated with malignant tumor in the serosa of the intestine and in the pelvis. Histologically, there was direct transition from benign epithelium in the ectopic endometrial glands, to dysplastic and neoplastic tissue. In different areas, the tumor included elements of yolk sac type (endodermal sinus tumor), choriocarcinoma, and teratoma. The implications of this unique transformation and combination are discussed briefly.

Myelin basic protein interacts with the myelin-specific ganglioside GM4.

Demyelinated plaques in multiple sclerosis have decreased amounts of both myelin basic protein and sialosylgalactosylceramide (GM4), a ganglioside specifically localized in myelin and oligodendroglia of the central nervous system. We have found that myelin basic protein is capable of releasing large quantities of entrapped [14C]glucose from multilamellar liposomes containing GM4. If the conformation of GM4 in liposomal membranes resembles that of GM4 in its natural environment, basic protein and GM4 may be associated within the myelin sheath of the central nervous system and their interaction altered in demyelinating diseases such as multiple sclerosis.

Carcinoma of the colon with undifferentiated, carcinoid, and squamous cell features.

An undifferentiated small cell tumor of the colon that was found by light- and electron-microscopic study to possess features of a malignant carcinoid merged into keratinizing squamous cell carcinoma. Single cells contained both dense-core secretory granules and tonofilaments. The implications of this combination in relation to theories of tumor origin and differentiation are briefly discussed.

Chitinase-overproducing mutant of Serratia marcescens.

Genetic modification of Serratia marcescens QMB1466 was undertaken to isolated mutants which produce increased levels of chitinolytic activity. After mutagenesis with ultraviolet light, ethyl methane sulfonate or N-methyl-N'-nitro-N-nitrosoguanidine, 19,940 colonies were screened for production of enlarged zones of clearing (indicative of chitinase activity) on chitin-containing agar plates. Forty-four chitinase high producers were tested further in shake flask cultures. Mutant IMR-1E1 was isolated which, depending on medium composition, produced two to three times more than the wild type of the other components of the chitinolytic enzyme system--a factor involved in the hydrolysis of crystalline chitin and chitobiase. After induction by chitin, endochitinase and chitobiase activity appeared at similar times for both IMR-1E1 and QMB1466, suggesting possible coordinate control of these enzymes. The results are consistent with IMR-1E1 containing a regulatory mutation which increased production of the components of the chitinolytic enzyme system and/or with IMR-1E1 containing a tandem duplication of the chitinase genes. The high rate of reversion of IMR-1E1 to decreased levels of chitinase production suggests that the overproduction of chitinase by IMR-1E1 is due to a tandem gene duplication.

The Brenner tumor: a report of multiple tumors.

A case of multiple Brenner tumors was studied with light and electron microscopy. Among the epithelial elements, argyrophil cells were found in addition to the typical "transitional" and mucus-secreting cells. In some areas, it was not possible to distinguish epithelial from tumor stromal cells. By electron microscopy, dense-core secretory granules were identified, as well as previously undescribed cytoplasmic tubular bodies. Some epithelial cells were intimately associated with reticulin and collagen. Some stromal cells had desmosomes, dense bodies, incomplete basal lamina, and, occasionally, multivesicular bodies, which suggests epithelial differentiation. Previous authors have documented morphologic similarities and continuity between Brenner-tumor epithelium and ovarian cells of almost every type. The inference is that there are multiple possible sources for the Brenner tumor. Classification must therefore be based on realize differentiation rather than presumed cell of origin.

Chronic expanding hematomas. A clinicopathologic entity.

Chronic exapnding hematomas occur in many locations, often simulating neoplasms. All have the same structure with a central mass of blood, a wall of granulation tissue, and dense, fibrous tissue at the periphery. The self-perpetuating exapnding nature of the lesion appears to be due to the irritant effects of blood and its breakdown products, causing repeated exudation or bleeding from capillaries in the granulation tissue.

Adenocarcinoma of the colon with choriocarcinoma in its metastases.

A 49-year-old woman died from metastases from a colon carcinoma. This was predominantly an ordinary tubular adenocarcinoma but it included arease of much less differentiated appearance with giant cells, while the hepatic and pulmonary metastase were poorly differentiated and contained choriocarcinoma, as established by means of morphologic and immunocytochemical examination. The choriocarcinoma appeared to be due to metaplasia within the carcinomatous tissue. This and other similar cases of mixed tumours suggest that unexpected trophoblastic differentiation may result from aberrant differentiation of locally proliferating cells, rather than originating in ectopic germ cells or in foci of embryonic totipotent cells. Implications for possible origin and for classification of tumors are briefly discussed.

Interaction of multiple sclerosis serum with liposomes containing ganglioside GM1.

Multiple sclerosis serum contains a factor or factors capable of releasing sequestered [14C]glucose from liposomes containing GM1, the major ganglioside present in human central nervous system myelin. Although some control sera can also release large quantities of liposomal glucose, multiple sclerosis and control sera differ in the mechanism of this release. Liposomal damage by multiple sclerosis serum does not appear to be complement mediated, but shares certain releasing characteristics of cholera toxin.

Rupture of a renal cell carcinoma in a child: five-year tumor-free survival and literature review.

The third case of presumably spontaneous rupture of a hypernephroma in a child is reported. This patient remains free of disease more than 5 years following radical nephrectomy, radiation and chemotherapy. Although renal adenocarcinoma in children remains a rare disease, more than 80 cases have appeared in the literature, making it necessary now to consider this tumor in the differential diagnosis of all childhood abdominal masses. Equally significant is the observation that presentation as an acute abdominal crisis in a child can occur, making it important that diagnostic considerations in such situations include hypernephroma, as well as Wilms' tumor. Review of the literature shows that occasional cures from spontaneous retroperitoneal rupture of hypernephromas have been reported in adults, but not in children, although this child has survived in spite of tumor spillage in the free peritoneal cavity.

Mediastinal histoplasmosis with abscess.

A mediastinal mass of clinically undetermined nature was found at autopsy to be an enormous abcess due to secondary infection in granulamatous caseating mediastinal nodes caused by Hitosplasma capsulatum. Other complications of mediastinal histoplasmosis are briefly reviewed.

Single coronary artery with myocardial infarction and AV block.

A case of single coronary artery with myocardial infarction was found in a 77-year-old woman. Study of the conductive system in this case did not show any abnormalities that could be correlated with the terminal clinical episode of heart block. In seven anatomically proven cases reported, the single coronary artery arose from the right aortic sinus, which suggests a possible causal relationship to infarction.

Sarcoid granulomas in CBA/J mice. Histologic response after inoculation with sarcoid and nonsarcoid tissue homogenates.

Homogenates of lymph node and skin biopsy specimens and lymphocyte suspensions from patients with sarcoidosis and control subjects were injected into the footpads of CBA/J mice; histologic response was examined at intervals of 15 to 360 days. Kveim tests and complete autopsies were performed on animals of each group. Granulomas or equivocal granulomas were observed in 14.8% of biopsy specimens from animals that received sarcoid tissue homogenates and in 14.8% of control animals. No granulomas were observed after injection of lymphocytes from patients with sarcoidosis or from control subjects. Kveim tests were negative in all animals and autopsies were unremarkable. These studies fail to confirm previous reports of a "transmissible agent" in sarcoidosis and demonstrate that by the use of these techniques, such an agent is not invariably present in sarcoid granulomas.

Non-ketotic hyperosmolar coma with insulin resistance in a 12-year-old girl.

Although the frequency of recognition of the syndrome of non-ketotic hyperosmolar coma is increasing in all age groups the mortality rate remains high. This paper reports the occurrence of this syndrome in a 12-year-old girl. The pathogenesis is discussed and a planned approach to the management of circulatory collapse, hypokalemia, hyperglycemia and hyperosmolarity over the initial 24 hours is proposed. The phenomenon of insulin resistance which was observed in this case is a rare complication of this syndrome. Awareness of the occurrence of this syndrome in all age groups, early recognition of its features and a planned approach to management are mandatory to reduce the very high mortality rate.
