[Results with multi-band appliances in the treatment of class II].

Corrections of Angle's Class II malocclusions constitute nearly 1/4 of the treatments performed by the orthodontist. The author studied the results achieved with treatment methods applicable to these malocclusions and compared them with those obtained by means of multiband appliances. On the basis of 2,110 cases, he establishes the indications for the use of multiband appliances in the framework of social dental treatment.

Mesenteric vascular occlusion in infants and children: report of two cases and review of the literature.

Mesenteric vascular occlusion is a rare disease in the paediatric age group. The pertaining literature is reviewed and two cases are reported. In the first one the vascular obstruction developed after a Wilms tumour on the right side had been removed. Following bowel resection and end-to-end anastomosis the patient died of uraemia. Post mortem examination revealed a diffuse membranous glomerulonephritis and pseudoxanthomatosis in the remaining left kidney, the function of which had probably been affected by the shock associated with mesenteric thrombosis. In the second case a mixed mesenteric vascular occlusion was found without any previous disease; after ileal resection and ileo-coecal anastomosis the baby made a smooth recovery.

Cholangiodysplastic pseudocirrhosis: light and electron microscopic examination.

The familial type of cholangiodysplastic pseudocirrhosis is presented. This chronic liver disease is caused by malformation of the intrahepatic bile ducts. The female infant was 5 months old when the diagnosis was established. The liver biopsy was studied by light and electron microscopy. Electron microscopic examination revealed active proliferation of ductual cells and progression of fibrogenesis, findings consistent with the rapid and fatal course of the disease. In the case presented an acute cholangitis occurred, but after healing the progression of the original process led to hepatic insufficiency. It is suggested that cholangiodysplastic pseudocirrhosis is a chronic, progressive liver disease the course of which might be hastened by the complication of cholangitis; the process itself causes liver cirrhosis without inflammation.

Electrostimulation of the neuropathic bladder.

Intravesical electrostimulation offers promise in management of neurogenic bladder in children with myelocele. In the past 2 yr, we carried out about 500 treatments in 25 children aged 2 wk to 13 yr. After repeated stimulation, the starting intravesical pressure increases considerably in all cases; however, this was not always accompanied by clinical improvement. Our early results are encouraging, especially after repeated 10-day courses of electrostimulation; bladder contractions increase slowly and micturition begins to appear. Seventeen of 25 patients showed significant improvement, in two patients no improvement was observed. The remaining six have not been treated long enough to evaluate.

Morphological characteristics of neonatal obstructive jaundice.

In cases of neonatal obstructive jaundice, liver biopsy material has been studied electronmicroscopically. In bile duct atresia, morphologic changes are extensive and they develop early. The severity of destruction is related to the quantity of accumulated bile deposits. Hypoxia due to the disturbance of microcirculation plays an important part in the damage. In cases of inspissated bile syndrome, the changes are less severe and they prove reversible.

Treatment of neonatal abdominal cysts.

Intraperitoneal or retroperitoneal cystic structures in the newborn appear with a variable clinical picture and in the case of intra-abdominal cysts, surgery is performed mostly on an emergency basis. In such cases the exact preoperative diagnosis is difficult and is seldom made. With early laparotomy, extensive small bowel resection can usually be avoided. Retroperitoneal lesions are mostly of renal origin; in such cases, preoperative diagnosis is easy, being based on specific examinations. In the year 1973, 5 newborns with an intraperitoneal or retroperitoneal cyst have been treated surgically. In this series, one infant died after resection of 90% of the small bowel.
