[Primary choriocarcinoma of the lung. Three cases in men].

Choriocarcinoma of the lung was recorded from three males, twice by autopsy and once by biopsy. Its histogenetic aspects are discussed in some detail. High levels of human choriogonadotropin (HCG) were recorded from blood serum and urine. The presence of this hormone in the syncytiotrophoblast and that of carcinoembryonic antigen (CEA) in the cytotrophoblast was positively verified by means of immunofluorescence. Granules of HCG were recordable electron microscopically from cytoplasm of the syncytiotrophoblast.

[Fibrolamellar variant of hepatocellular carcinoma].

A fibrolamellar variant of hepatocellular carcinoma with metastases in 2 regional lymph nodes was surgically removed from a girl, 17 years of age. This type of tumor has a typical histological pattern. Prognosis is found to be better than that of the common hepatocellular carcinoma.

Lethal extrapulmonary mycobacteriosis.

A 60 yr old previously healthy man was treated for gradually elevating fever and rash followed by leucopenia and mycosis of the gastrointestinal tract; he died within 6 weeks of the first symptoms appearing. Histologic examination revealed disseminated tuberculosis of paratracheal lymph nodes, liver, spleen and bone marrow with the presence of acid fast bacilli by smear examination. Multiple colonies of the same strain of Mycobacterium kansasii were isolated by bacteriological examination of lymph node and spleen tissue. Neither macroscopical nor histological examination showed any evidence of lung tuberculosis. The final diagnosis of the described acute disease and rapid death was stated as generalized extrapulmonary mycobacteriosis, which is a rare observation.

[Frequency of carcinoma in thyroid cysts].

The view that cystic thyroid nodes are as a rule benign is disproved by reports from the recent literature which quote an incidence of 17-32% of malignancies in conjunction with thyroid cysts. In 66 patients, investigated by the authors, with thyroid lesions confirmed on biopsy four suffered from thyroid carcinoma (two follicular, one papillary and one solid to anaplastic), i.e. a malignancy in 6%. In the literature it is recommended to follow up on a long-term basis patients with cysts which after thin-needle biopsy do not recede adequately or refill. Biopsies must be repeated, as the cytological yield of biopsies in these instances is not as reliable as in solid nodes. A haemorrhagic appearance of the bioptic specimen and anamnestic data on previous irradiation in the head or neck area are also considered risk factors.

[Analysis of deaths of patients in a pediatric clinic--the role of infection].

At the Paediatric Clinic in Plzen in 1982-1984 257 hospitalized children died; 175 were infants under 7 days of age, their deaths were not analyzed. At the age under one year 54 infants died, 25 children were older than one year. Infections of different aetiology and localization were revealed on necropsy as the immediate cause of death in half the children who died. On necropsy no infection was detected in 17 infants under one year and in 8 of the older children. Among the infants who died there were none, with one exception who were born healthy by a normal delivery, were admitted later on account of disease not threatening life and died subsequently from a nosocomial infection. The older children were admitted with malignant or irreversible chronic disease and the infections were associated with the breakdown of the defence of the organism.

The fate of the graft after extracorneal keratoplasty.

Two histological examinations are described. The first presents a full-thickness graft after extracorneal keratoplasty 2 years after transplantation. The graft is localized in the host tissue, externally lined by the thickened episclera and internally by the sclera. The reaction of the host tissue is slight and the graft is well preserved. In the second, a lamellar graft, rare capillaries grow 1 year after transplantation, the reaction of the host tissue is very mild, and the graft slowly changes into the original tissue of the host.

The nosocomial and occupational hepatitis type B infection in a chest disease hospital. A prospective study.

The results of a prospective study of hepatitis B infections among the patients of a Chest Disease Hospital are reported. A thousand and sixteen patients admitted during the twelve months study period were screened for the Hepatitis B(Surface)-Antigen(HBsAg) on admission and during the stay and were kept under epidemiological surveillance for six months following the discharge. The HBsAg prevalence was 3.54% on admission and the cumulative infection rate during the follow up period was 12.64 per 100 patients. The HBsAg prevalence on admission was significantly higher in males than in females, the susceptibility to infection found in the course of the study was in both sexes similar. The majority of infections was inapparent with the ratio of 8.6:1 of inapparent to clinically manifested forms. The incidence of viral hepatitis among the personnel was comparable to the incidence among the personnel of other hospital departments.
