Endoscopic surgery for nasal and sinusal vascular tumours: about two cases of nasopharyngeal angiofibromas and one case of turbinate angioma.

Incomplete resection, technical difficulties and important bleeding are feared in endoscopic sinus surgery (E.S.S.). The authors report three cases of E.S.S. for vascular tumours: two angiofibromas and one angioma. Adjuvant treatments were sometimes used : embolization by angiography (one case), clip on the sphenopalatine artery (one case). However, in one case, E.S.S. alone was sufficient. Average blood loss was 500 cc. Total resection, good clinical evolution and no recurrence at 9 months are good arguments in favour of this technique.

Clinical efficacy of N-acetyl-aspartyl-glutamic acid nasal spray in children suffering from pollinosis: a double-blind multicentre study.

A double-blind, four-centre study was carried out in 66 children to compare the clinical efficacy and safety of N-acetyl-aspartyl-glutamic acid (NAAGA) and disodium cromoglycate (DSCG) nasal sprays. At similar dosage conditions (one puff per nostril, four times daily, for 3 weeks), no significant differences between the treatments were discernible in the primary efficacy parameters (scores, patients' and physicians' opinion). Both products induced statistically significant improvements in the nasal, ocular and total scores, but not in the respiratory (breathing) score. The hay-fever symptoms improved clinically in > 50% of the children after only two weeks with both treatments. The exact figures depended on the parameters considered. Thirteen out of 25 patients (52%) in the NAAGA group found the efficacy "good" or "excellent" at the end of the treatment period; the corresponding opinion in the DSCG group was expressed by 10 out of 24 patients (41.7%). Of the 28 patients that used the rescue medication, 12 (42.8%) were from the NAAGA group and 16 (57.1%) from the DSCG group. One patient on NAAGA treatment reported side effects, i.e. pruritus in the nose and sneezing.

[Lymphoma of the nasal sinus].

Three cases of malignant lymphoma involving the nose and/or the sinusal cavities are reported and discussed. An overview of the literature is made and some facts of theoretical and practical interest are pointed out, about diagnostic classification, staging, prognosis and treatment.

[Chronic atrophying polychondritis].

A case of chronic atrophic polychondritis in a 75 year old female patient is reported. The disease started with an asymptomatic lesion of the nasal septum. It was diagnosed after involvement of the auricular cartilages, which was followed by an ocular and then by a tracheal symptomatology. A histological study by means of optical microscopy and of electronic microscopy is presented. The general characteristics of chronic atrophic polychondritis are given. It is a systemic, inflammatory disease, and progresses by successive outbreaks. Its gravity lies in the frequency of laryngeal and tracheo-bronchial lesions. Its pathogeny is unknown. Two hypotheses are considered: the first, that it is due to a disturbance in the metabolism of mucopolysaccharides ; the second is based on an immunological mechanism.

[Exploration of eustachian tube permeability by salpingomanometry].

In order to appreciate the permeability of the Eustachian tube the authors measure the rhino-pharyngal pressure necessary to induce the opening of the tube. This opening is obtained by Valsalva method or by a deglutition associated to an overpressure. The measures are realized on divers, the majority of them never suffered before from any barotraumatic trouble. The modifications of pressure produced in the middle ear during the tubal opening are demonstrated by recording the correspondant variations of the tympano-ossicular compliance. By performing the Valsalva, we have better results than those obtained by actively clearing of the ears associated to a rhino-pharyngal overpressure. The method is of easy and quick apply. We consider as normal the levels of the forcing pressure ranging below 90 cm H2O.
