Pharmacokinetics of fenoterol in pregnant women.

The beta 2-sympathomimetic drug fenoterol (fenoterol hydrobromide, CAS 1944-12-3, Partusisten) is routinely used to inhibit uterine contractions (tocolysis). Investigations of plasma concentrations of those receiving i.v. or oral tocolysis often show different results, both within particular groups of pregnant women and in comparison with non-pregnant persons. The aim of this study was to determine the pharmacokinetics of fenoterol in pregnant women, an important factor which so far had not been known. Four healthy pregnant women with similar weight and gestational age and all with premature labor were administered a continuous intravenous infusion of 4 micrograms fenoterol/min. During and up to 24 hours after the end of the infusion, venous blood samples were taken in order to determine the fenoterol plasma concentrations by radioimmunoassay. From a steady state concentration (css) of 2242 +/- 391 pg/ml (x +/- S.E.), a non-linear two-phased plasma elimination was seen with half-lives t1/2 of 11.40 min and 4.87 h. The area under the plasma concentration-time curve (AUC0-12h) was 6.27 ng/ml x h. The total clearance (Cltot) was 114.8 l/h. These data are nearly the same as the data already known for healthy non-pregnant (male) volunteers. The deviations which are seen in the plasma concentrations in pregnant women in comparison to non-pregnant persons during or after continuous i.v. infusion can therefore not be caused by differences in the pharmacokinetics. Other factors, however, such as body weight and/or gestational age, might influence the results.

[Cushing's syndrome in pregnancy].

Active Cushing's syndrome during pregnancy is a rare phenomenon which was so far described in the literature only in 85 women. The authors give an account of a patient who developed Cushing's syndrome during pregnancy and who was delivered during the eighth month of a healthy foetus by caesarean section, afterwards the hypercortisolism associated with hyperaldosteronism was treated by dextrolateral adrenalectomy. In the removed adrenal gland was a cortical adenoma made up of clear cells and cells reminding of the zona glomerulosa. Subsequently the authors summarize data from the literature on the prevalence of different pathogenetic forms of Cushing's syndrome in pregnant women, on the influence of hypercortisolism on mother and foetus, on the optimal diagnosis and therapy of this syndrome during pregnancy.

[Dynamics of digoxin-like immunoactivity during the oral glucose tolerance test in pregnant women].

The changes of serum digoxin-like immunoactivity (DLIA) during 75g oral glucose tolerance test (OGTT), were investigated in 25 pregnant women with various glucose tolerance. 13 women had a normal glucose tolerance (NGT), 12 women had an impaired glucose tolerance (IGT) or gestational diabetes mellitus (GDM). Test was performed after 25 week of pregnancy. The results were compared with 9 nonpregnant healthy volunteers (NHV). After an oral glucose challenge, there was observed a significant decrease in serum DLIA (p < 0.01 in 1st hour), in both groups of the pregnant women. In the group of NHV these findings were not found. Our findings suggest a possible relationships between DLIA and glucose tolerance in pregnant women. However, it needs further investigation. [Diagnosis of malignant ovarian tumors using color Doppler sonography].

In a prospective investigation the authors followed the circulation in 32 patients with ovarian tumours before operation. They examined the RI using colour Doppler sonography. Based on histological examination the authors divided the patients into groups with malignant and benign tumours resp. The peripheral resistance and RI in the group of patients with malignant tumours was 0.35 +/- 0.03, i.e. significantly lower than in the group with benign tumours, 0.65 +/- 0.08 (p < 0.001). In none of the patients with malignant tumours the RI was higher than 0.50. The authors recommend this method as a screening examination for early detection of malignant ovarian tumours.

[Does transabdominal amniocentesis in the 2d trimester affect fetoplacental circulation?].

In a prospective study the authors focused attention on Doppler sonographic examination of the foetoplacental circulation and whether it is influenced by transabdominal amniocentesis. They examined 124 pregnant women subjected during the 17th or 18th week of pregnancy to amniocentesis in the out-patient department. Before amniocentesis and soon after it they examined the flow in the uterine artery on the right and in the umbilical artery. From five consecutive cycles they evaluated the S/D ratio and RI in both vessels and compared the results. They did not record any complications during amniocentesis. Evaluation of the results of both investigated parameters before and after amniocentesis did not reveal any statistically significant differences in the investigated vessels. Based on the assembled results, the authors assume that transabdominal amniocentesis, despite its invasive character, does not influence the foetoplacental circulation and does not have a negative impact on the course of pregnancy.

Recombinant human erythropoietin in the treatment of postpartum anemia.

Postpartum maternal anemia (hemoglobin concentration below 10 g/dL) is a common problem in obstetrics. Human recombinant erythropoietin, which has been shown to correct the anemia of end-stage renal disease and eliminate the need for transfusions, was used in a comparative study of women with postpartum hemoglobin concentrations below 10 g/dL. Five daily doses of 4000 IU were given. Hematologic and clinical data were compared on days 5, 14, and 42 after therapy in the treated women and in untreated women. Both groups received the same iron and folic acid supplements. Significantly greater increases in reticulocytes, hemoglobin, and hematocrit were seen by day 5 for the treated subjects compared with controls. Ferritin levels were significantly lower in the therapy group than in controls. No differences were seen between the groups in the platelet counts or clinical characteristics. No negative side effects were observed. As in other studies in populations without renal disease, recombinant human erythropoietin enhanced endogenous erythropoiesis over and above the normal physiologic recovery rate.

The development and formation of the sulci on the dorsal surface of the sheep's brain.

The authors studied development of the sulci on the dorsal surface of the brain of 150 merino sheep. The sulcus coronalis was the first to appear, at a CR length of 162 mm (72-75 days). Further sulci originated in the following sequence: the sulcus ansatus at a CR length of 178 mm (76-77 days), the sulcus cruciatus at 180-183 mm (77-78 days), the sulcus marginalis at 185 mm (78-79 days), the sulcus ectomarginalis at 221 mm (85-86 days) and the sulcus endomarginalis at 265-270 mm (96-97 days). After the sulci have been formed, their development continues with accentuation of secondary branching.

[Uterine artery flow in pregnancy with 1st trimester hemorrhage].

The authors examined in a prospective study in 25 pregnant women blood flow in the uterine artery during haemorrhage in early pregnancy with the diagnosis of imminent abortion. The control group was formed by a group of 25 women with intact pregnancy, in the first trimester. For examinations an ultrasound apparatus ATL MK 600 was used, the authors evaluated the S/D ratio and RI from five consecutive cycles. The mean values of the S/D ratio in the investigated group were 4.85 +/- 0.22, in the control group 4.41 +/- 0.24 which is a statistically significant difference. Similarly the mean RI values in the investigated group were 0.79 +/- 0.01, in the control group 0.77 +/- 0.02--the difference is statistically significant. The results confirmed the assumption that by monitoring of the blood flow in the uterine artery in women with bleeding during the first trimester of pregnancy it is possible to detect relatively early risk patients.

[Decortication: indication and morbidity--a retrospective analysis].

Decortication, that is excision of both the visceral and parietal pleura, has become a rarely performed operation. It is used for control of empyema of the pleura or recurrent malignant pleural effusion. During the last ten years at the University Hospital in Zurich 50 decortications were performed upon 49 patients. Mortality was 4%, however, death was due to the primary disease and not related to the operation. 38 of 50 decortications were straight forwards. The main complication was bleeding, needing re-thoracotomy in three patients and leading in five patients to a pneumothorax due to occlusion of the suction drains. We conclude, that if conservative treatment fails to result in a rapid progress, decortication should be performed immediately.

[Morphogenesis of the cerebral fissures in sheep].

The author studies the development and formation of the boundary fissures in the brains of the foetuses of sheep during their embryonal development from a vertex-to-coccyx length (VC length) of 38 mm (39th to 40th day) to birth. The marginal fissures of the cerebral hemispheres were found to develop first. The sulcus hippocami develops from them at a VC length of 46 to 48 mm (42nd to 45th day) and is one of the fissures with a constant course of formation. Sulcus rhinalis lateralis develops from a VC length of 70 to 72 mm (49th to 50th day). The caudal part of this fissure is deeper during the whole prenatal development than the rostral part.

[Carcinoma of the oral cavity--analysis of local metastases and results of therapy].

A series of 453 patients admitted to the 2nd Clinic of Stomatology in Bratislava over the ten-year period from 1977-1986 was statistically evaluated. The series comprised patients with the diagnosis of epidermoid carcinoma of the oral cavity, with the exception of carcinoma of the lip. The condition of the regional lymphatic apparatus at the first examination and over the succeeding period was studied. The relationship between the treatment used, clinical stage and prognosis of tumorous disease in the orofacial region was evaluated. The possibilities of different types of cervical dissection are discussed along with the histological findings recorded. In the light of the survival period a direct relationship between the clinical stage of the disease at the onset of treatment and prognosis has been confirmed.

[The development of myocardial infarct in a patient treated with 5-fluorouracil].

The authors describe the first case of myocardial infarction in their department which occurred during 5-fluorouracil treatment. In a 37 year-old patient two years before the infarction an epidermoid carcinoma of the lower lip was found. The patient was subjected to operation and radiotherapy. Because of a relapse of the process after one year the patient was re-operated with subsequent chemotherapy (vincristine, methotrexate, cis-platinum). Because the effect was not satisfactory, after another year 5-fluorouracil treatment was started (i.v. infusion of 3000 mg in two days). In the course of the infusion the patient developed the clinical picture of a non-transmural infarction of the anterior wall. The course was uncomplicated. The ECG returned to normal on the 9th day after discontinuation of 5-fluorouracil treatment. The effect of 5-fluorouracil on the heart is explained by a coronary vascular spasm. The authors recommend therefore administration of calcium blockers.
