Drug policies and harms: a conceptual framework.

Existing indicators of drug-related policies and harms - official statistics, treatment data, and so on - are difficult to interpret and to compare across jurisdictions. We propose a conceptual framework including key dimensions of policy and harm. The framework is designed to guide creation of a new dataset in which existing empirical data can be used to assign city-specific scores on policy and harm dimensions. By an interpretive process involving local experts and a coordinating body, we believe that existing data can be evaluated and synthesized to derive these scores. We also propose a cumulative case-study method of querying the dataset to test hypotheses regarding relationships between policies and harms.

Advanced management facilities for clinical laboratories.

As part of the OpenLabs (AIM 2028) programme a decision support system (DSS) for laboratory capacity management has been developed. This DSS contains a simulation module for determining the performance of planning rules given the equipment and staffing of the clinical laboratory and the demand for laboratory services. User requirements are discussed and a method is developed to (re-)define rules concerning various planning decisions. To show the functionality of the simulation module some simulation experiments are described.

Decision support for clinical laboratory capacity planning.

The design of a decision support system for capacity planning in clinical laboratories is discussed. The DSS supports decisions concerning the following questions: how should the laboratory be divided into job shops (departments/sections), how should staff be assigned to workstations and how should samples be assigned to workstations for testing. The decision support system contains modules for supporting decisions at the overall laboratory level (concerning the division of the laboratory into job shops) and for supporting decisions at the job shop level (assignment of staff to workstations and sample scheduling). Experiments with these modules are described showing both the functionality and the validity.

The Amsterdam morphine-dispensing experiment.

While the debate concerning the dispensing of hard drugs continues around the world, much can be learned from the Amsterdam morphine-dispensing experiment described here. The modus operandi is explained and results are represented diagrammatically.

A monoclonal antibody to VIII:C produced by a mouse hybridoma.

Spleen cells of a BALB/c mouse immunized with factor VIII procoagulant activity (VIII:C) (isolated by affinity chromatography) were fused with mouse myeloma cells (P3 x 63 Ag8). After the fusion 12/32 wells produced an inhibitor to VIII:C. Cells from one well (1B3) were subcloned four times in order to isolate the hybridoma that produces the anti-VIII:C antibody. Injection of hybridoma cells in pristane pretreated BALB/c mice results in anti-VIII:C titers of 5000-10,000 Bethesda U/ml. Analysis of the produced immunoglobulin demonstrated heavy chains of IgG1 (produced by the myeloma cell line) and IgG2b subclass. The 1B3 antibody neutralizes VIII:C in LMW FVIII, crysosupernatant, cryoprecipitate, and normal plasma. It was found that binding of the IgG to FVIII results in a delay in its activation and not in an inhibition of its cofactor activity. The antibody removes VIII:C from pooled normal plasma when coupled to Sepharose; when coupled to plastic tubes, it binds VIIICAG from isolated VIII:C, purified FVIII, and pooled normal plasma; it does not bind VIIIR:AG, fibrogen, or serum VIIICAG. The 1B3 antibody can be used successfully in an IRMA for VIIICAG.
