Acute intrahepatic portal vein thrombosis complicating cholangitis in biliary atresia.

UNLABELLED: A 2-year-old boy with Kasai portoenterostomy had systemic features suggestive of cholangitis and acute portal vein obstruction. The rapid rise in portal pressure caused a transient, severe decompensation of hepatic function. A superior mesenteric arterial angiogram confirmed the presence of diffuse intrahepatic portal vein thrombosis. CONCLUSION: This case provides clinical and radiological evidence supporting an association between ascending cholangitis and acute intrahepatic portal vein thrombosis.

An algorithmic approach to chronic cough.

OBJECTIVES: To evaluate a stepwise approach to chronic cough that emphasized initial treatment of all patients with an antihistamine-decongestant for postnasal drip and to determine the value of routine bronchoprovocation challenge in the evaluation of chronic cough. DESIGN: Prospective trial using an algorithm for chronic cough in immunocompetent nonsmoking out-patients. SETTING: University-based pulmonary practice. PATIENTS: Forty-five patients met the inclusion criteria. The mean duration of cough was 140 weeks (range, 3 to 2080 weeks), and the mean severity of cough as assessed by patients on a four-point scale was "severe." RESULTS: Marked improvement and resolution (mean, 3.1 and 7.1 weeks, respectively), with resolution in 96% of patients. Antihistamine-decongestant therapy was beneficial in 39 of 45 patients and was the only therapy needed for 16 patients. Bronchoprovocation challenge had a negative predictive value of 100% and a positive predictive value of 74% for cough caused by asthma. No significant relationship was found between the time to cough resolution and duration or severity of cough. Eighteen percent of patients experienced a recurrence of cough at a follow-up interval of 3 months. CONCLUSIONS: A sequential approach to chronic cough that emphasizes initial treatment with an antihistamine-decongestant is effective. Bronchoprovocation challenge is useful in evaluating patients with chronic cough but can be delayed until the initial response to antihistamine-decongestant therapy has been assessed. The 18% incidence of recurrence highlights the fact that cough often is the manifestation of a chronic or recurring process that requires chronic or episodic therapy.

Treatment of acute uncomplicated urinary tract infections with 3 days of lomefloxacin compared with treatment with 3 days of norfloxacin.

The bacteriologic and clinical efficacies of 3 days of lomefloxacin therapy were compared with those of 3 days of norfloxacin therapy for the treatment of acute uncomplicated urinary tract infections in a prospective, randomized, double-blind study. One hundred sixty-four subjects were enrolled at five Canadian centers; 84 received lomefloxacin, and 80 received norfloxacin. Escherichia coli (84%) and Staphylococcus saprophyticus (11%) were the most common organisms isolated. Forty subjects (24%) had low quantitative counts in their pretherapy urine specimens. In the intent-to-treat analysis, 76 lomefloxacin subjects (91%) and 76 norfloxacin subjects (95%) were cured or improved at follow-up 5 to 9 days posttreatment and 73 (87%) and 71 (89%) subjects from the lomefloxacin and norfloxacin groups, respectively, were cured or improved at 4 to 6 weeks posttreatment. Bacteriologic eradication occurred in 61 of 63 lomefloxacin subjects (97%) with > or = 10(8) CFU/liter in their pretherapy specimens and 56 of 59 norfloxacin subjects (95%) at 5 to 9 days and 55 (87%) and 53 (90%) subjects from the lomefloxacin and norfloxacin groups, respectively, at 4 to 6 weeks. There were no statistically significant differences in outcome. Adverse effects which were potentially related to the study medications were reported by 26% of the subjects who received lomefloxacin and 25% of the subjects who received norfloxacin. There were no severe adverse events, and only one subject discontinued therapy. These data suggest that 3 days of therapy with either lomefloxacin or norfloxacin is effective in the treatment of acute uncomplicated urinary tract infections.

Monitoring renal function in children with urological abnormalities.

In clinical practice glomerular filtration rate routinely is estimated by measuring serum creatinine and using the Schwartz formula for calculating an estimate of creatinine clearance or using the Gates formula for calculating the clearance of diethylenetriaminepentaacetic acid based on the uptake during radionuclide imaging. We compared these methods to the plasma clearance of iothalamate during extended constant infusion. All 3 tests were performed in 14 boys and 5 girls 9 months to 16 years old with urological abnormalities and moderate renal insufficiency. Using iothalamate as the reference, calculated creatinine clearance overestimated glomerular filtration rate by more than 20% in 12 of the 19 patients (63%). Diethylenetriaminepentaacetic acid uptake grossly overestimated glomerular filtration rate; in only 3 instances was the estimate within 20%. The accuracy of both formulas was better in older patients and in those with more normal renal function. We conclude that the Gates formula for measuring glomerular filtration rate is grossly inaccurate in children with diminished renal function and the Schwartz formula, although better, has a poor level of predictability.

Bilateral carotid body tumors managed with preoperative embolization: a case report and review.

A patient with large bilateral carotid body tumors had preoperative, superselective embolization of major arterial afferent vessels. After marked reduction in tumor vascularity, total surgical extirpation was then possible without significant morbidity or carotid sacrifice. The use of preoperative embolization in the treatment of large bilateral lesions is emphasized and discussed.
