Hepatic ascaridiasis.

Adult Ascaris, which are usually found in the intestine, can enter the ampulla of Vater if the sphincter of Oddi is not efficient. This situation seems common in children; in adults it occurs after endoscopic or surgical sphincterotomy. The worms can then migrate up into the intrahepatic bile ducts and provoke biliary stasis which may become infected and complicated by abscesses. It is inside such abscesses that we may find fragments of adult Ascaris. In the 4 cases reported in the literature during the last 20 years, we note in particular that the clinical signs vary considerably, according to the age of the patient. In children, the parasitic infestation is often massive and the clinical features are extremely serious, namely shock and acute abdominal pain, which lead to a laparotomy being done. In adults, the clinical feature is that of a biliary infection. Here we describe a fifth case of hepatic ascaridiasis which differs from the others in that the diagnosis was established by ERCP and CT scan and that treatment was medical.

Atrophies 'like flows' of the retinal pigment epithelium: a neuroepithelium-draining method of the posterior pole.

17 cases of eyes affected jointly by diseases of the posterior pole (central serous chorioretinopathy, serous detachment of pigmented epithelium, melanoma, osteoma, hemangioma) and flow-like atrophy of the pigmented epithelium, originating from the lesion and always directed downwards, are presented. It is likely that serous detachment of the neuroepithelium constituting the original wound (central serous chorioretinopathy) or the secondary one (serous detachment of the pigmented epithelium) of the posterior pole prolongs itself downwards, leaving a localized atrophy as a mark on the pigmented epithelium. The eventual draining role of serous detachment of the neuroepithelium by this 'flow' is disputed.

[Effectiveness of various hormonal preparations in adults with hypothyroidism (author's transl)].

Among more than 7000 adult patients with suspected dysthyroidism in who radioimmune assays were performed, the authors have selected 21 patients with high TSH hypothyroidism who had assays before and after replacement therapy with various preparations prescribed by their G.P. The results in this group of 61 treatments of at least 3 weeks duration were as follows: (1) under thyroid extract or thyroglobulin (21 treatments), the condition persisted in the majority of patients; (2) under thyroxine (31 treatments), the thyroid function returned to normal with a once-a-day dose of 150 mcg/-T4; (3) under triiodothyronine (7 treatments), none of the patients reverted to euthyroidism. Levothyroxine therapy therefore appears to be the most effective and cheapest way of providing optimal replacement therapy in patients with hypothyroidism. The daily dose of about 150 mcg can easily be adjusted according to the results of occasional hormonal assays.

[Treatment of diabetic retinopathy by centripetal pan-retinal photocoagulation with the argon laser].

The argon laser is such an effective method of treating diabetic retinopathy that it is used in the early stages. The indications for treatment are related either to ophthalmoscopically visible lesion, or to lesions revealed by angiography, which is necessary each year in all diabetics. As for as technique goes, it is desirable to begin pan-retinal photocoagulation in the periphery so as to take advantage of the drying out of the related oedema secondary to the destruction of the retina.
