[A case of chronic calcifying pancreatitis in a 19 year old man].

We present an advanced case form of chronic calcifying pancreatitis (ch.c.p.) of 19's age man, whose first clinical symptoms appeared when he was 5 years old. For the rest 14 years his complains were diagnosed as vegetative neurosis. The patient was admitted in to the Department in 4th day of severe abdominal pain. High increase of serum and urine amylase activity, changes in USG and CT examinations (20 mm wide pancreatic duct with numerous calcified stones) were find. Acute pancreatitis in the course of ch.c.p. was diagnosed. After typical treatment abdominal pain disappeared and results of laboratory tests achieved a normal range. During 1.5 year follow up (diet, pancreatic enzymes) substitution abdominal pain did not appear and the body weight increased 8 kg.

Efficacy and safety of ebrotidine compared with ranitidine in patients with duodenal ulcer.

OBJECTIVE: To compare the efficacy and safety of ebrotidine and ranitidine administered once daily in equimolar doses of 800 and 300 mg, respectively. PATIENTS: A total of 298 duodenal ulcer patients were studied. DESIGN: A multicentre, parallel, randomized clinical trial. METHODS: Of the 298 patients studied, 150 were randomly assigned to ebrotidine and 148 to ranitidine treatment. Digestive endoscopy was performed at enrolment and at weeks 4, 6 and 8 unless the ulcer had healed before. Endoscopic findings were the main parameter for the assessment of treatment efficacy. Plasma gastrin and pancreatic polypeptide concentrations were also measured before and after termination of the therapy. RESULTS: Ebrotidine achieved a duodenal ulcer healing rate comparable to that of ranitidine, and no statistically significant difference was found between the two drugs. The drugs were equally effective in improving ulcerous dyspeptic symptoms and in relieving gastric pain. Both tobacco and ethanol consumption influenced ulcer healing adversely, but healing in smokers was more pronounced in patients treated with ebrotidine, possibly because of its cytoprotective activity. CONCLUSIONS: Ebrotidine 800 mg is as effective and safe as ranitidine 300 mg in healing duodenal ulcer, but ebrotidine appears to be superior in promoting the healing of duodenal ulceration in smokers.

[Cholecystolithiasis in the urban population of Poland].

Multicentre study was carried out in three regions of Poland aimed to assess cholecystolithiasis incidence in urban population and some potential risk factors of the disease. 10133 persons in five age groups, from 16 to 70 (6071 women and 4062 men) were examined. The examination consisted of questionnaire considered sex, age, weight, dietary habits, complaints, chronic drugs use, family history and in women number of pregnancies and deliveries as well as hormone therapy. The results obtained were analysed statistically with Chi 2 test. Cholecystolithiasis was found in 1411 persons (10.7%), among them 18% women (1083 women) and 8.2% men (328 men). Incidence rate was 180.5/1000 women and 82.0/1000 men. The obtained increase in percentage of cholecystolithiasis cases with age was statistically significant. No relation were found between the number of stones and sex of the examined persons. In 1480 persons (43.5%0 the disease was asymptotic. The studies did not prove the correlation between cholecystolithiasis in women and sex of the first child or oral contraception. Positive correlation was found between cholecystolithiasis in women, obesity, number of pregnancies and family history. In men age was most significant risk factor. Incidence of cholecystolithiasis in Poland, Western Europe and USA is similar.

[Liver steatosis assessed by ultrasonographic examination in patients with hyperlipoproteinemia].

Ultrasonographic examinations were carried out in 174 patients with various types of hyperlipoproteinaemia, determining the incidence of the features of liver steatosis. In the group of 77 patients with hypercholesterolaemia (TCH > 200 mg%) the features of liver steatosis occurred in 13 cases (16.9%). In 90 patients with mixed hyperlipidaemia (TCH > 200 mg%, TG > 2.3 mmol/l) the features of liver steatosis were found in 50% of cases. Both in the groups of patients with hypercholesterolaemia and with mixed HLP, the patients with liver steatosis had significantly higher body mass index. In the group of patients with hypercholesterolaemia (without and with the features of liver steatosis) no differences were found in the concentrations of total cholesterol (TCH), triglycerides (TG) and high density lipoprotein fraction (HDL). In the patients with mixed HLP and the features of liver steatosis in USG examination, the mean serum triglyceride concentration was 6.2 mmol/l and was almost twice higher than that in the group without steatosis. The serum HDL concentration in the patients with mixed HLP was 39.2 mg% and was significantly lower than that in the patients with the same type of lipid concentration disturbances but without liver steatosis (48.2 mg%). The total serum cholesterol concentration was not differing significantly between the patients with liver steatosis and those without this pathological condition.

[Fatty liver assessed by histologic examination in patients with hyperlipoproteinemia].

Out of 665 patients after liver biopsy performed during 14 years of the work of the Clinic, 35 cases of patients with non-alcoholic liver steatosis and hyperlipoproteinaemia were selected and analysed retrospectively. The cases of patients with the presence of HBV or HCV infection markers were excluded. The histological material was divided according to the intensity of steatosis expressed as the per cent of hepatocytes with the features of fatty degeneration and also according to the following classification: I--steatosis, II--steatosis with hepatitis, III--steatosis with portal fibrosis, IV--steatotic cirrhosis. In the patients in whom the per cent of hepatocytes with fatty degeneration in biopsy examination exceeded 60%, the mean serum triglyceride concentration was 5.53 mmol/l and was over twice higher than that in the group with steatosis not exceeding 30% of hepatocytes. Similarly, in the patients with steatosis and accompanying hepatitis (steatohepatitis), the mean triglyceride concentration was 5.28 mmol/l and was over twice higher than that in the patients with steatosis with accompanying portal fibrosis. The patients with steatohepatitis had significantly lower HDL concentrations than those with the remaining types of histological changes.

[Evaluation of the effectiveness of the preparation Panzytrat 20,000 U in the substitute treatment of chronic pancreatitis].

Panzytrat 20,000 U produced by Knoll is a modern galenic form of active pancreatic enzymes in the form of microgranules with uniform enzymatic composition. The studies of the preparation effectiveness were carried out in 88 patients with diagnosed exocrine failure in the course of chronic pancreatitis. The patients were taking Panzytrat 20,000 for 4 weeks in doses 3 x 1 capsule with main meals. Control examinations were performed two and four weeks after starting the treatment, assessing the patients' condition on the basis of history taking and medical examination; taking into account weight gain, duration and intensity of pain and dyspeptic symptoms, frequency and character of defeacations and possible undesirable effects. In a group of 37 patients, BNT-PABA test was also performed before the treatment and after one capsule of Panzytrat. The treatment with Panzytrat 20,000 caused a statistically significant reduction of the number of defecations, significant reduction of pain and dyspeptic symptoms. A statistically significant increase of PABA recovery in urine was also obtained after one capsule of the preparation. Practically, no unfavourable effects of the treatment were observed. The studies carried out point to the high effectiveness of Panzytrat in dose 3 x 20,000. U daily in the treatment of exocrine pancreatic failure in the course of chronic pancreatitis.

[Role of nutrition in development of chronic alcoholic pancreatitis].

The diet of 45 men with alcoholic chronic pancreatitis in the period before the onset of the symptoms of the disease was studied. The dietary history was used as a tool in research of the dietary data. The diet of studied group was compared with the diet of 23 alcoholics without the symptoms of pancreatic injury and with the diet of 30 healthy men with similar age and social status. The energy intake and main nutrients content (especially protein and fats) was statistically higher in the studied group. High protein and fats consumption with high ethanol intake may promote the development of chronic pancreatitis.

[Evaluation of the influence of extrapancreatic factors (undernutrition, cirrhosis) on utilization of amino acids by the pancreas after its stimulation with secretin and cerulein].

We studied the utilization of amino acids by the pancreas after its hormonal stimulation in diseases in which disturbances occur of amino acid and protein metabolism. After 45 minutes of intravenous infusion of secretin (1 CU/kg) and cerulein (75 ng/kg) we found a significantly lower (p 0.05) decrease of amino acid concentration than in healthy individuals, both in chronic pancreatitis and in undernutrition and post-inflammatory cirrhosis with decompensation of liver function. Greatest disturbances in amino acid utilization by the pancreas were found in the case of valine, leucine and isoleucine. The results of studies show certain limitations in the use of this method for the assessment of the exocrine function of the pancreas.

Loperamide (imodium)--clinical trial.

1. Imodium (loperamide) produced by Chemical Works of Gedeon Richter, Budapest, is a useful preparation for the symptomatic treatment of both acute and chronic diarrhoea of various etiologies. 2. In chronic diarrhoea cases the best results were obtained in the treatment of functional disorders of the guts. 3. Side effects occurred only in few patients and with low intensity, the treatment had to be interrupted in none of the cases.

[Modification of the test for determining bacterial capacity for nitrate reduction].

For facilitation of the detection of bacteria able to reduce nitrates the presently used method of 24-hour broth culture with KNO3 and the method of growing on slant agar cultures with KNO3 (standard method) were compared, with certain modifications introduced. It was found that shortening of the incubation period from 24 to 6 hours and reduction of the volume of broth with KNO3 from 10 to 0.5 ml made possible obtaining of a greater number of correct results as compared to the standard method. In the 24-hour broth cultures with KNO3 done as yet in case of negative results it is indispensable to confirm them in the test with zinc dust. The placement of Griess reagents on points onto colonies growing on KNO3 agar may lead to false positive results.

[Chronic pancreatitis in the Bródno district of Warsaw].

A retrospective epidemiological-clinical analysis of patients with chronic pancreatitis living in the Bródno District of Warsaw is reported. It was found that the approximated coefficients of morbidity and incidence per 100 thousand of the population in 1987 were 17.0 and 5.0 respectively, and they were similar to those reported from Rochester (USA) and Copenhagen (Denmark). However, the incidence has risen from 2 in 1982 to 5 in 1987 per 100 thousand, which might be connected with increasing alcohol consumption by the population or increased detectability of chronic pancreatitis in this city district.
