A technique for control of the condylar head during open reduction of the fractured mandibular condyle.

Open reduction of fractures of the neck of the mandibular condyle may be technically difficult. Control of the proximal fragment may be achieved by insertion of a Moule pin into the condylar neck. This facilitates retrieval of the condylar head and alignment of the bone fragments which are essential components of the technique. Accurate anatomical reduction and stabilisation of the fracture simplifies the technique of miniature plate fixation.

Gentamicin impregnated polymethylmethacrylate (PMMA) beads in the treatment of primary chronic osteomyelitis of the mandible.

Primary chronic osteomyelitis (chronic diffuse sclerosing osteomyelitis) of the mandible is an uncommon condition, probably arising as a consequence of infection with bacteria of low virulence, in which bone deposition rather than bone resorption occurs. These bacteria are most likely derived from skin or oral mucosa gaining access to bone from the periodontium or through the circulation. Furthermore, delayed hypersensitivity and ischaemic changes within bone may contribute to the inflammatory response, which once initiated is very difficult to eradicate. When the likely aetiological factors are considered a logical approach to management includes the surgical removal of affected bone and the topical application of a broad spectrum antibiotic to the resultant surgical bed.

Parapharyngeal neurilemmomas.

Parapharyngeal neurilemmomas are uncommon benign tumours. Two patients are reported with one tumour arising from the vagus and the other from the hypoglossal nerve. Access to the vagal tumour was accomplished with the aid of an osteotomy at the angle of the mandible. Theoretically these lesions can be enucleated with preservation of the nerve or resected and repaired with a nerve graft. However, these alternative procedures were impractical in both these patients and the explanations for this are discussed.

Oral nodular fasciitis.

Nodular fasciitis is a proliferative fibroblastic lesion which presents as a tumour-like mass. Accurate diagnosis is important to avoid unnecessary and often mutilating surgery. Although up to 20% of cases occur in the head and neck region, lesions of the oral cavity are extremely rare. A case of oral nodular fasciitis is described, together with a review of the literature.

A review of tumours of the deep lobe of the parotid salivary gland.

Tumours of the deep lobe of the parotid gland may present as a swelling in the oropharynx. They are uncommon when compared with those of the superficial lobe, are frequently misdiagnosed and subjected to per-oral biopsy which is hazardous and predisposes to seeding of the tumour. Investigation and diagnosis are discussed with particular reference to the role of computed tomography. The surgical approach to these inaccessible tumours is illustrated by reference to patients treated. Modifications to the technique of Cooke and Ranger (1969) for excision of parapharyngeal tumours are suggested, which are applicable to tumours of the deep lobe extending medial to the mandible and presenting in the side wall of the pharynx.
