Immune responses to dialyzer contents by dialysis patients.

Serum antibodies to the contents of cuprophan hollow-fiber dialyzers were demonstrated by immunodiffusion in 36 of 68 hemodialysis patients. The antibodies were not absorbed by ethylene oxide or washed cuprophan hollow fibers. No cross-reactivity with Escherichia coli lipopolysaccharide and pneumococcal polysaccharides was noted. The antibodies were IgM and probably IgG. Washed cuprophan hollow fibers did not absorb IgE detected by radioallergosorbent test (RAST). Lymphocyte transformation to the contents of both cuprophan hollow fiber and polyacrylonitrile plate dialyzers was demonstrated. Materials added in manufacture of dialyzers are immunogenic. The long-term biologic effects are unknown.

Effect of hemodialysis on methylprednisolone plasma levels.

The effect of hemodialysis on methylprednisolone levels in uremia was investigated. Methylprednisolone 15 mg/kg was given intravenously over a period of 20 min to five patients undergoing routine maintenance hemodialysis. Dialysis began 5 min after the infusion ended and continued for 5 h. Pre- and post-coil plasma levels and the dialysate level of methylprednisolone were measured at intervals. Dialysance averaged 18.4 +/- 6.1 ml/min, and mean half-life 149 +/- 26 min. The volume of distribution was 0.80 of body weight. The significant dialysance of methylprednisolone may require adjustment of dosage in certain clinical situations when hemodialysis is necessary for patients receiving the steroid.

Interstitial nephritis in rats produced by E. coli in adjuvant: immunological findings.

An increased incidence and severity of interstitial nephritis was produced in F344/fmai rats immunized with E. coli 022 in pertussis vaccine for 12-15 months. Migration of peritoneal exudate cells from immunized animals was inhibited by syngeneic kidney antigens. One out of twenty-eight immunized animals developed anti-TBM antibodies. In this model, interstitial nephritis develops in association with cell-mediated immunity to kidney tissue.
