Thrombosed aneurysmal nonpatent ductus arteriosus: a case report.

We report our observations made clinically and by conventional radiography, ultrasound, computed tomography and MRI on a newborn full-term infant with a thrombosed patent aneurysmal ductus arteriosus of Botallo. The baby had no heart murmurs, but from the beginning of the 2nd day of life developed sinus tachycardia, especially when in a left-sided position, and dyspnoea mainly when drinking. From his 1st day of life we observed generalized muscular hypotonia and a tendency for hyperextension at neurological examination.

Massive bleeding into the upper gastro-intestinal tract in hereditary chronic calcifying pancreatitis in the child.

This report deals with a young girl affected with chronic hereditary calcifying pancreatitis, which gave rise to acute life-threatening arterial bleeding into the upper gastro-intestinal tract. A pancreatic pseudocyst forming in the chronically affected pancreas spontaneously perforated into the stomach, thereby eroding a gastric wall artery. We think it is conceivable that in the future this type of complication in children with pancreatitis and pseudocyst formation might occur more frequently.

Fractures of the sacrum and disk herniation: rare lesions in the pediatric surgical patient?

Fractures of the sacrum and lesions of the intervertebral disks are seldom reported in childhood and adolescence. In our report, illustrated by three representative cases that we treated during the past six months, we show that sacrum fractures, not an easy diagnosis, may be more frequent than currently assumed, and that in quite a number of children and adolescents an anterior pelvic fracture may in fact be an unrecognized Malgaigne type pelvic fracture with a posterior fracture plane cutting through the sacrum. They may be accompanied by herniations and/or lacerations of intervertebral disks.

[Femoral fractures in school age (6th-12th year)].

The femoral fracture is a paradigme for problems arising in fracture of the growing skeleton: length recovering growth and overgrowth, axial and rotational correction during physiological variation of the anteversion of the femoral neck will be discussed and their impact considered. Different types of overgrowth are accounted for. Our own patients, treated conservatively, showed a mean initial shortening of 0.85 cm. Their mean total overgrowth was 0.6 cm, so that their final grown-up leg length was -0.25 cm on the fractured side. Our small series did not indicate, that a consolidation with a modest angulation of less than 20 degrees and/or an initial shortening of less than 2 cm gave rise to a final over-length of some importance. The anteversion of the femoral neck of the broken leg compared with the other leg showed a mean difference of 13 degrees. None of the patients showed an abnormal gait. We discuss the unanswered question, if torsion deformities will be spontaneously corrected by growth-dependent modification of the anteversion of the femoral neck alone or by detorsion of the diaphysial femur too. We present the modalities of conservative treatment and the indications of operative treatment. We finally will raise the problem about the nature of the bone-remodeling mechanisms: role of the vascular distribution, of tissue hormones and of the streaming potentials. The hypothesis of streaming potentials forming the short-time memory of bone, and their transformation into a pattern of bipolar collagen fibres is fascinating.

[Arthrography of the ankle joint in injuries of the fibular ligament system in children and adolescents].

Arthrography of the ankle is an optimal diagnostic tool in children and adolescents facilitating differential diagnosis between a simple ankle sprain and rupture of the fibular ligaments. It can moreover help to differentiate between rupture of a single and of two ligaments. 35 young patients with surgically confirmed ligamentous rupture were evaluated according to conventional radiographic criteria, and part of them by arthrography. The technique of the latter and the quite frequent normal variants in arthrograms of the ankle in childhood are described. The comparison of the pathologic anatomy disclosed by surgery and the arthrogram reveals a very good correlation. Its main value resides in giving fair information in clinically and radiographically controversial cases.

[Conservative management of burns].

The first chapter is dedicated to the frequency and the prognosis of burn trauma, and to the general management of the patient. The second part deals with the pathophysiological basis of the problem and with some controversial facts. The third chapter is a survey of inhalation injury and of electrical burns.
