Bladder cancer and smoking in males: types of cigarettes, age at start, effect of stopping and interaction with occupation.

A case-control study of 512 male cases of bladder cancer and 596 male hospital controls (all living in the province of Turin, Northern Italy) has been analyzed for cigarette smoking. Relative risks (RRs) decreased with years since stopping, whereas they seemed not to change with age starting to smoke. A multiplicative effect of RRs for smoking and high-risk occupations was suggested. Filters exerted a protective effect (RR = 0.3 for smokers of filter-tip cigarettes only vs. smokers of non-filter-tip cigarettes). A previously unreported difference was noticed between black types and blond types of tobacco (RR = 0.4 for smokers of the blond types only vs. smokers of the black types, adjusted for age, average daily amount, years since stopping, occupation and use of filter).

Relative potencies of four CEA preparations assayed by three commercial radioimmunoassay kits.

An attempt has been made to determine relative potencies of four Carcinoembryonic Antigen (CEA) reference preparations, including the World Health Organization CEA standard, assayed by three kits (Abbott, Cis, Roche). For some kits and reference CEAs, relative potencies varied as a function of the concentration at which CEAs were assayed; whether this was due to different CEA antigenicities or to inappropriate technical conditions (e.g. unequal adsorption of CEA onto surfaces, in connection either to the protein composition of buffers or to different degrees of purification of CEAs) requires further investigation. Even when the CEA concentration had no influence, relative potencies differed according to the origin of the kits. Since even when a single reference material is used, different relative potencies are obtained with different kits, valid comparisons can hardly be made, at least for the time being, unless the same technique and the same test system (antiserum and antigen) are used. Modalities of such a technique should be carefully determined and the reagents carefully selected so that lack of parallelism, if any, between CEA of different origins can only be ascribed to differences in antigenicity.

[An analysis of survival data in the epidemiological context].

The present work reviews the methodology of survival analysis in the epidemiological field, especially within the framework of cancer registries. The main concepts used in survival analysis are presented, and the problems of competing mortality, as well as variable and stage adjustment are also discussed. Although the estimation and comparison of survival data distributions obtained from epidemiologic observations is technically possible nowadays thanks to a variety of statistical methods which take into account multiple proGnostic factors, it is proposed that pilot studies should be undertaken to look at the main problems related with potential selection biases and the standardization of prognostic factors to be evaluated.
