The medical futility controversy: bioethical implications for the critical care nurse.

Medical futility is a recent, complex bioethical issue. There is disagreement about how futility should be defined and who should be involved in futility decisions when an impasse exists between the patient/family and the physician. Bioethical discussions about Quinlan and Cruzan of the past have been replaced with the Wanglie, Baby K, and Linares cases--all of which involved critical care settings. Nurses often are involved in the debate and encounter ethical conflicts. Cost-containment, managed care, scarce resource allocation, and care due the elderly have fueled the debate. Key issues and their importance for critical care nurses will be reviewed.

The nurse's role in patient advocacy.

Advocacy is a broad and complex concept. The advocate's role may be conceptualized in various ways such as that of a rescuer, information giver, spokesperson, agent of quality control or guardian. Although nurses strongly subscribe to the concept of patient advocacy, they need to be supported in delaying advocacy actions when additional data and assessment are indicated or in avoiding advocacy actions when job security is perceived as threatened. Supervisors and managers need to assist nurses in becoming better patient advocates and in supporting them when they legitimately decide not to act as advocates.

Environmental rounds: the pursuit of trivia?

Details abound in the environment and many of them significantly influence the quality of patient care. Supervisors and the staff themselves periodically need to conduct environmental rounds. The pursuit of trivia is to be encouraged and demanded for both practical and philosophical reasons.
