Tumour of the juxtaoral organ.

The juxtaoral organ is a normal and constant structure of the oral cavity. It consists of benign epithelial nests. We describe an intraoral tumour of the juxtaoral organ in a child. The tumour was not diagnosed after clinical and radiological examinations because it is extremely rare. A histological examination revealed a tumour of the juxtaoral organ, presumed to be neuroid hamartoma. This is only the second time that a tumour of the juxtaoral organ has been described in a child. We also describe the location, the embryology, the histology and the function of this organ. This is important because this structure can be confused with carcinomas of the oral cavity when examining frozen sections.

[Radial forearm or Chinese flap].

The Chinese or radial forearm flap, first described by Yang et al. (1981), is useful both as a free or a pedicle flap. This flap has proved to be one of the safest and most versatile flap in widely differing clinical situations. It is generally classified as a fasciocutaneous flap, but can also be used together with a segment of radius as a composite flap. However, the most common objection to this flap is the donor-site morbidity. The donor defect is significant, with a poor graft take over the exposed tendons. Inclusion of bone in the flap leads to a greater contour defect and to a risk of fracture. Many suggestions are given to minimize the complications on the donor-site defect. The surgical technique, the advantages, the disadvantages, and at least the indications of this flap are detailed.

[Contribution of the semi-free radial forearm flap for head and neck reconstruction].

Flap failure is mostly caused by venous thrombosis of the anastomosis. The failure rate is higher in post-irradiated neck. To reduce the risk of venous thrombosis, the authors propose to harvest the radial forearm flap using the cephalic vein as the drainage vein and as a pedicle. Only a single arterial anastomosis is carried out. This technique has been described already in patients who underwent head and neck ablative surgery with radical neck dissection. The authors report 2 cases in patients with post-irradiated neck. The flap transfers were successful in the 2 cases. The operative time of harvesting this hybrid version is the same with the conventional free flap technique. This version of the radial forearm flap can be a method of choice for some complex microvascular reconstruction in the head and neck region.

[The three-dimensional spiral scanner and volume rendering technique: importance in craniofacial traumatology and reconstructive surgery].

There are two reconstruction methods for 3D imaging with a helical scanner: a surface rendering process providing a 3D-SSD (Surface Shaded Display) image that looks like a plaster cast skull and 3D-VRD (Volume Rendering Technique) which is more precise and gives 3D images similar to radiographs. We review briefly the techniques used to acquire these images and present 3D volume-rendering scanners, pointing out its usefulness in craniomaxillofacial traumatology and reconstructive surgery.

[Extra-oral implants: indications and contraindications].

Osseointegrated facial prostheses are an interesting solution for maxillofacial rehabilitation when reconstructive surgery cannot be proposed. Progress in epithesis materials and in implantation techniques have renewed interest in maxillofacial prostheses. The best indications are for rehabilitation of nasal, auricular and orbitopalpebral defects. Another indication is complex loss of facial tissue. Endo-osseous epitheses are recommended mainly when there is no possibility of reasonable and satisfactory surgical reconstruction, when local carcinologic prognosis is uncertain, and to respond to the patient's demands. We detail here the precautions necessary when irradiation has been performed previously and finally present the contraindications against epitheses on osseointegrated implants.

[Diagnostic difficulties of chondrosarcoma of the jaw. Apropos of a case and review of the literature].

Chondrosarcoma is an uncommon malignant mesenchymal tumor characterized by the production of cartilaginous tissue and the absence of production of bone tissue. Maxillary and mandibular localizations are extremely rare and have a poor prognosis. The clinical and radiographic findings are similar to those seen in other tumors of the jaw, often delaying diagnosis and treatment and subsequently jeopardizing prognosis. In the literature, the mean delay from first clinical signs to diagnosis is about 8 months. As in all tumoral diseases, pathology confirms the diagnosis. We report a case of chondrosarcoma of the maxillary bone and review the literature, focusing on the difficulty in establishing the diagnosis, even at the pathology examination. We propose wide surgical excision, which, in agreement with other reports in the literature, is the only therapeutic possibility. We observed no sign of recurrence at 5-year follow-up. Radiotherapy and chemotherapy may be useful in certain cases.

[Mandibular reconstruction of gunshot wounds by progressive bone distraction. Report of five cases].

Five adult patients with gunshot wound defect underwent bilateral mandibular lengthening by an extraoral device. All patients presented with interrupting mandibular defect of 50 to 100 mm. Distraction of bone fragments led to mandibular reconstruction without bone grafting and simultaneous expansion of soft tissues, avoiding free or pedicled myocutaneous flaps for soft tissue reconstruction of the lower third of the face. Mandibular distraction also recreated the alveolar ridge with its attached mucosa which is equivalent to the gingiva. It can be used for dental rehabilitation by osseointegrated implants. Avoiding the morbidity and functional problems of classical flaps, mandibular distraction accelerates facial reconstruction, and social reinsertion of these patients.

[Implant-supported prosthesis. The significance of endosseous and peri-osseous implants].

The new extra-oral of Farmand (EPITEC system) is a plate-implant. This concept is really different from the Brånemark's implant and the use of EPITEC system give the authors opportunity of a first assessment. It seems that if, of course, Brånemark system is still the reference, the EPITEC system certainly has a real interest in thin osseous implantation areas.

[The management of extra-oral implants. Anachronisms and paradoxes].

Used in France for ten years and in Sweden since 1977. Extra-Oral Implants (EOI) represent one of the new therapeutic approach of facial defects treatment by maxillo-facial prostheses fixed on implants and of some transmission deafnesses. Audiologic applications are free of charges for the patient but not the prosthetic applications; the authors note this paradox and some other anachronisms. Finally the authors wonder why Extra-Oral implantology is a "correct" science and why Intra-Oral Implantology remains absent from the Administrative texts as if it was a sort of doubtful therapy without scientific principles.

[Intramuscular capillary-venous angioma extending into the infratemporal fossa. Apropos of a case].

A rapidly growing tumor in the temporal area extended into the infratemporal fossa. Imaging findings favored a hypervascularized meningo-sarcoma type extracranial tumor, but the pathology report on the surgical specimen confirmed the diagnosis of capillaro-venous angioma. The therapeutic approach was preoperative hyperselective embolization then access via the temporal and infra-temporal fossa for simple tumor exeresis. Mature angiomas and access via the infra-temporal fossa are reviewed.

[Psychological profile of suicide using firearms to the face. Review of the literature. Our experiences at the university hospital center at Caen].

We studied the psychological feature of subjects who attempted suicide with firearms. This retrospective study included 36 cases of "facial suicide" treated at the Caen University Hospital since 1981 and data in the literature. An analysis of the different parameters showed in several cases, that this population is different from the others, particularly hospitalized drug overdose suicide patients. Two notions were particularly remarkable in these patients: history of patients psychiatric disorders were very rare and recurrent suicide attempts.

[Metatypical carcinoma. Apropos of 4 cases].

The authors describe the anatomopathological, clinical and evolutive characteristics of the metatypical carcinoma, based on four cases and on the existing literature. Evolution of these tumors, with a maxillo-facial preferential location, is usually longer than for other cutaneous carcinomas. Cases presented in this article illustrate the severity of such lesions, whose treatment requires extensive amputations and rather unusual reconstruction procedures.

[Maxillofacial prostheses on endosseous implants. Various modes of fixation].

Both mechanical and magnetic types of fixation can be used with extra-oral implants, with the possibility of positioning maxillofacial prostheses by either mechanical, magnetic or mixed fixation, the latter by a combination of the two methods. These different modes of fixation allow precise positioning, avoid the use of adhesives, limit maintenance problems and, in addition, facilitate oncologic surveillance, providing a non-negligible complement to reconstructive surgery.

[Cystic chondromalacia of the auricle. A case report. Review of the literature].

Cystic chondromalacia is a clinical and histopathological entity which can be clearly distinguish from all other cystic lesions of the auricle. We report a case which was clinically asymptomatic and involved the scaphoid fossa of the anterior surface of the pinna. The pathological process consisted of degenerative changes of the auricular cartilage which produced a cavity containing a serous fluid. No etiologic factor was found, in particular no trauma. The posterior wall of the cyst was excised under local anesthesia. This case allowed us to review the clinical, histopathological features and surgical difficulties of this rare lesion.
