Structure of 6-fluoro-1 beta,2 beta-methylene-3,20-dioxo-9 beta,10 alpha-pregna-4,6-dien-17 alpha-yl acetate.

C24H29FO4, Mr = 400.5, orthorhombic, P2(1)2(1)2(1), a = 12.207 (1), b = 14.715 (1), c = 11.572 (1) A, V = 2078.7 (5) A3, Z = 4, Dx = 1.28 g cm-3, lambda (Cu K alpha) = 1.5418 A, mu = 7.62 cm-1, F(000) = 856, T = 291 K, final R = 0.041 for 2230 observed reflections. The A and B rings assume flat boat and 9 beta,10 alpha-half-chair conformations, with asymmetry parameters delta C3s = 1.5, delta C1,2s = 11.2 and delta C6,7(2) = 3.1, delta C7s = 15.0, respectively.

Morphological appearance of renal allografts in transplant failure.

Renal allograft dysfunction may be due to a variety of causes including acute and chronic rejection, acute tubular necrosis, cyclosporine A toxicity, glomerulonephritis, and surgical causes. It can be difficult to distinguish between these etiologies. Ultrasound is a relatively simple, noninvasive method for imaging the transplant kidney, and is excellent for the diagnosis of surgical complications. The features of rejection include an increase in renal volume, reduced echogenicity, indistinctness of the corticomedullary junction, and splaying of the medullary pyramids. These findings, although suggestive of rejection, cannot differentiate reliably between rejection and other causes of allograft hypofunction. Doppler studies may show an increased resistive index in a kidney undergoing rejection, but this finding may also be seen in other pathological processes and is not specific. A normal ultrasound examination does not exclude rejection. Ultrasound is useful as a baseline examination. Abnormal findings should be interpreted in the light of the clinical situation in each case. Where there is any doubt about the diagnosis, ultrasound-guided biopsy should be performed.

Graft Rejection

Generation and characterisation of murine monoclonal antibodies specific for cervine immunoglobulin light chain, IgM and IgG.

Monoclonal antibodies (mAb) which react with cervine immunoglobulin (Ig) light chain, IgM and IgG were produced using conventional cell fusion technology. Hybridoma supernatants were initially screened for specificity against cervine Ig using an enzyme-linked immunosorbent assay (ELISA). The specificity of supernatants against size-fractionated cervine Ig was further determined. Supernatants were characterised using western blotting and autoradiographic techniques. The mAb OU1G, OU2G and OU3G were specific for cervine gamma-chain of IgG, whereas OU1L was specific for light chain of Ig. A further mAb (OU1M) bound IgM and not IgG. These mAb were found to have varying cross-reactivity against Ig from other species.

Animals

The effect of adjuvants on active and passive immunity in pregnant deer and their offspring.

Experiments were carried out to determine the effects of a range of adjuvants on immunoglobulin M (IgM) and immunoglobulin G (IgG) serum concentrations to a protein antigen administered subcutaneously to farmed female deer following mating. The antibody responses of animals immunised with keyhole limpet hemocyanin (KLH) in Freund's Incomplete Adjuvant (FIA), diethylaminoethyl dextran (DEAE-dextran) and aluminium hydroxide (alum) were compared with the response to antigen administered in the absence of adjuvants. Animals were subsequently challenged with a subcutaneous immunisation of the antigen in saline. Following parturition, the concentration of passively transferred antigen-specific antibody was measured in the serum of the offspring. The polyionic adjuvant, DEAE-dextran, produced the greatest enhancement of both primary and secondary IgG responses to KLH. Offspring suckling from mothers immunised with antigen in DEAE-dextran consequently had higher concentrations of specific antibodies in their serum than other fawns in the experiment. The adjuvants FIA and alum were approximately 20-fold less effective in enhancing antigen-specific IgG than DEAE-dextran but induced greater amounts of antigen-specific IgM. From the results presented in this paper, there is evidence that immunisation of deer during pregnancy may be an effective way of reducing morbidity in both mothers and offspring.

Molecular conformation of estramustine and two analogues.

The crystal and molecular structures of estramustine and two of its analogues have been determined by X-ray crystallographic techniques (a total of three different compounds). The compounds studied are estramustine [1,3,5(10)-estratriene-3,17 beta-diol-3-N,N-bis(2'- chloroethyl)carbamate] and its monohydrate, estromustine [17-oxo-1,3,5(10)-estratriene-3-yl-N,N-bis(2'-chloroethyl)carbamate], and 17-oxo-5-androsten-3 beta-yl-N,N-bis(2'-chloroethyl)carbamate. Three views of estramustine were obtained from the study of its two crystal forms. The main structural features found are as follows: (a) the geometries of the steroid moieties are closely similar to those of the parent steroids, (b) the bonds around the nitrogen atom of the nitrogen mustard grouping lie approximately in a plane in each structure, (c) the plane through the carbon atoms of the steroid A-ring lies approximately perpendicular to the plane through the carbamate atoms in each structure, (d) the carbonyl C-O of the carbamate points to the alpha side of the steroid moiety in each structure, and (e) one chlorine atom of the nitrogen mustard grouping makes a close contact [3.13 A], in each structure, to the nitrogen atom. Hydrogen bonding to the carbamate appears to occur from the alpha side of the steroid; there is no hydrogen bonding to the nitrogen atom of the carbamate group. These structural data provide some steric explanations for the resistance of the carbamate to enzymatic hydrolysis. The long in vivo half-life of the intact estramustine molecule is a result of this stability. This is responsible for the absence of alkylating ability and the propensity of the drug to bind microtubule-associated proteins and express an antimitotic mechanism of action.

Estramustine

Tuberculosis in domesticated red deer: comparison of purified protein derivative and the specific protein MPB70 for in vitro diagnosis.

The use of a Mycobacterium bovis-specific protein, mycobacterial protein bovis 70 (MPB70), was compared with complex, M bovis-derived purified protein derivative (bovine PPD), for its ability to improve the diagnostic precision of in vitro assays for tuberculosis in farmed deer. A combination of lymphocyte transformation and enzyme-linked immunosorbent assay (ELISA) was used to differentiate between specific M bovis reactivity and crossreactivity due to sensitisation with saprophytic mycobacteria such as Mycobacterium avium. In the lymphocyte transformation assay the response of mononuclear cells, from red deer, to MPB70 was found to be more specific, but less sensitive, as an indicator of infection by M bovis when compared with the complex antigen bovine PPD. When used in conjunction with bovine PPD alone, MPB70 was found to increase the specificity of the ELISA in diagnosing animals with disease.

Idiopathic inflammatory pseudotumour of the orbit: CT features correlated with clinical outcome.

Idiopathic orbital pseudotumour is an inflammatory condition which usually presents as an orbital mass. The response to treatment is variable and often unpredictable. In this study we have assessed the spectrum of computed tomographic (CT) appearances in 17 patients. Four categories of orbital pseudotumour were identified by CT based on the location and extent of the inflammatory process within the orbit. Patients within each of the four categories had broadly similar clinical outcomes. CT was valuable in identifying certain prognostic features enabling better clinical and therapeutic planning.

Adolescent

Optic canal fractures after rhinologic surgery.

In two women, 26 and 24 years old, respectively, polytomography revealed optic canal fractures. The investigation of visual loss after orbital or otolaryngologic surgery included tomography of the optic canals to rule out compression of the intracanalicular optic nerve.

Adult

Cord blood lymphocyte subpopulations and mitogenic activity in whole blood microculture.

Comparative studies on cord and adult blood showed that cord blood contained at least twice as many lymphocytes as adult blood. Relatively, the percentage of T cells (E-RFC) was significantly lower in cord blood lymphocytes. The percentage of B cells (EAC-RFC and SmIg bearing cells), as well as the total number of T and B cells (mm(-3)), was significantly higher in cord blood. In vitro mitogen transformation of cord and adult lymphocytes in while blood, cultured for different times and diluted to contain equivalent numbers of lymphocytes per culture, showed significant qualitative and quantitative differences. Responses to the T cell mitogens phytohaemagglutinin (PHA) and concanavalin A (Con A) were examined from 3 to 6 days in culture. Cord blood lymphocytes were significantly more responsive when cultured for 3 to 4 days, similar to adult cells after 5 days, but significantly less responsive after 6 days in culture. The optimal levels of T cell mitogen responsiveness in cord cells (Day 4) were similar to adut cells (Day 6). Spontaneous transformation of unstimulated lymphocytes and B cell mitogen transformation with pokeweed mitogen (PWM) and staphylococcal protein A (SpA) were all significantly higher in cord blood than in adult whole blood cultured for 5 days.

Voluntary vertical nystagmus.

A patient with hypertropia caused by fourth-nerve palsy manifested primary position oscillations characteristic of voluntary nystagmus. This benign condition must be kept in mind even when dealing with primary position vertical nystagmus to avoid extensive and potentially dangerous investigations.

Conformation of [Leu5]enkephalin from X-ray diffraction: features important for recognition at opiate receptor.

The conformation of [Leu5]enkephalin is produced by a Tyr-Gly-Gly-Phe beta bend stabilized by antiparallel hydrogen bonding between tyrosine and phenylalanine. On the basis of a comparison of the observed structure with the structure of known opiate agonists, three hydrophilic and two hydrophobic regions have been identified as contributing to the recognition of the molecule at the opiate receptor site.

Acquired color vision defects in retrobulbar neuritis.

We used the Farnsworth-Munsell 100 Hue Test to study the incidence of acquired color vision defects in 16 patients with unilateral and and seven patients with bilateral retrobulbar optic neuritis whose visual acuity had recovered to 6/12 (20/40) or better. We recorded the visual-evoked response to a pattern stimulus on the same day. All 30 affected eyes showed an excessive number of errors on Farnsworth-Munsell 100 Hue testing. The number of errors increased with greater residual optic nerve damage. One false-positive and two false-negative visual-evoked responses were found.

Cortical blepharoptosis.

A 59-year-old man with a previous left frontal lobe infarction sustained an infarction of the right frontal lobe. The patient manifested a bilateral symmetrical blepharoptosis that resolved in two months. Eyelid movements seem to be bilaterally represented in the frontal lobes. Bilateral cortical disease can produce supranuclear eyelid abnormalities such as blepharoptosis, difficulty maintaining eyelid position, and difficulty initiating eyelid movement.

Misdiagnosis of spasm of the near reflex.

A bilateral sixth nerve palsy portends serious disease of the central nervous system and precipitates extensive patient studies. Spasm of the near reflex, characterized by intermittent convergence, accommodation, and miosis, is a functional disturbance. Five patients with hysterical spasm of the near reflex erroneously diagnosed as a bilateral sixth nerve palsy are reported. The pupillary sign, intense miosis on attempted lateral gaze, is emphasized as an important clue to the correct diagnosis. Despite extensive investigation, no disease of the central nervous system was found. Neurotic or hysterical features were evident in every patient.
