[Treatment of calcium kidney lithiasis. ESWL, NLP, open surgery].

OBJECTIVE: To review current management of renal calcium stones. METHODS: Data from our experience are presented and the literature is reviewed. The results and indications of treatment are analyzed. RESULTS/CONCLUSIONS: Pyelic calculi are the best indication for ESWL. The success rate ranges from 33%-90% and is influenced particulary by stone size and composition. Similar results are achieved for calyceal calculi, although the overall complete resolution rate is 50%-75% for stones in the lower calyces versus 90% for PNL. Furthermore, the resolution rate is even lower if the infundibulo-pyelic angle is less than 90 degrees. The resolution rate ranges from 4%-58% for intradiverticular calculi and the best results are obtained when the calculus is small and the neck of diverticulum can be visualized at urography. The staghorn stone is the most complex and offers more difficulties to treatment by ESWL. Good results are achieved only in the staghorn type I (72% resolution; mean 3.5 sessions per patient).

Renal cell carcinoma: late recurrence in 2 cases.

We present 2 patients with late recurrence 16 and 11 years after nephrectomy for renal cell carcinoma. Metastases were solitary and their location very uncommon (vastus externo muscle and colon). In the reviewed series of 5,758 patients with renal cell carcinoma only a temporal muscle metastasis and another in the colon were observed but none of them was solitary.

Appearance of gas in renal tissue after therapeutic embolization.

We present a case of therapeutic embolization in a patient with adenocarcinoma who demonstrated the presence of gas in the renal infarcted tissue on the second day. The pattern of air distribution was intravascular. There was no sign of abscess clinically or radiographically. To the syndrome of postembolization we add a new sign--the presence of postembolization air.

Artificial spermatocele.

Herein is presented a new technique to construct an artificial spermatocele, using monofilament knitted polypropylene wrapped with tunica vaginalis and anastomosed end-to-end to the head or the body of the epididymis. The material was tested in 10 dogs. It did not harm the spermatozoa, the function of the reservoir or the tolerance of the surrounding tissues. Therefore, the material was used in a patient with bilateral congenital absence of the vasa deferentia, while wife became pregnant following cervical insemination. The spermatocele is still functional after 15 months.

[Paraneoplastic syndromes in carcinoma of the kidney (author's transl)].

The authors present a group of patients with renal carcinoma in whom there were paraneoplastic signs such as fever, liver failure, hypertension and polycythemia. They noted, moreover, the most important clinical aspects, evaluating the incidence of these signs in relation to the course and diagnosis. They discuss also the frequency of presentation of each of these paraneoplastic manifestations, commenting on the various theories which have been suggested to explain them.
