[Induction of labor in the scarred uterus. 50 cases].

Fifty patients with a scarred uterus had labour induced with Oxytocics in the Maternity Hospital of Montfermeil, between 1987 and 1988. The criteria for selecting patients for vaginal delivery were those usually adopted and the induction of labour depended on the Bishop score. Fourty-two patients delivered vaginally with a low maternal morbidity (1 ruptured uterus without serious consequences, and no perinatal morbidity). Induction of labour makes it possible to watch the labour at a time when the obstetric and anaesthetic supervision can be at its best. This leads to a reduction in morbidity. However, labour in this way, even though it is increasingly possible to conduct vaginally, is still a high risk procedure.

[Endometrial ossification. Apropos of 5 recent cases].

Five cases of ossification of the endometrium that have been seen in the course of five years are reported. Special attention is paid to a case that occurred in the post-partum period. Several different hypotheses as to the aetiology of the pathology of this condition are discussed yet again; the only one that seems likely to be possible is that these ossifications start as metaplasias. The clinical, paraclinical and therapeutic angles are also dealt with.

Neuroendocrinology of opioid peptides and their role in the control of gonadotropin and prolactin secretion.

Substantial evidence now exists to indicate that the endogenous hypothalamic opioidergic mechanism(s) represents one of the important controlling systems for release of gonadotropin-releasing hormone. Modulations of frequency and amplitude of the secretory activity of gonadotropin-releasing hormone appears to be mediated through an inhibitory action of endogenous opioids, and the functional coupling of the opioidergic and gonadotropin-releasing hormone systems is an ovarian steroid-dependent event. There is also evidence to implicate suprahypothalamic mechanism(s) that enhance endogenous opioid inhibition of secretion of gonadotropin-releasing hormone. Although exogenous opioid peptides and their synthetic analogs consistently induce the secretion of prolactin, blockade of opioid receptors in humans by naloxone failed to elicit a decrement in the levels of prolactin under a variety of conditions. On the contrary, naloxone induced a remarkable increment in the secretion of prolactin via an increased frequency of pulsatile release which is synchronized with pulses of luteinizing hormone. These observations suggest that a common neuroendocrine mechanism is involved in the opioidergic control of the secretion of both luteinizing hormone and prolactin in women.

The dopaminergic inhibition of LH secretion during the menstrual cycle.

To assess the potential inhibitory role of hypothalamic dopaminergic input on the LRF-LH system, the gonadotropin response to a dopamine receptor antagonist, metoclopramid (MCP, 10 mg iv bolus) was examined during different phases of the menstrual cycle in 12 women. In addition, the role of dopamine infusion on naloxone (opiate receptor antagonist) induced LH increments was examined. MCP induced an abrupt increase in circulating LH levels in the mid-luteal phases but not in the early and late follicular phase subjects. No significant changes in serum FSH levels were observed. Dopamine, when infused concomitantly with naloxone, completely suppressed the naloxone induced pulsatile increments of LH in mid-luteal subjects. These findings support the contention that an increased dopaminergic inhibition of LRF-LH system occurs during the high estrogen-progesterone phase of the menstrual cycle, and provide preliminary evidence that the inhibitory role of endogenous opioids on LRF release may involve the dopaminergic system.

[Stage 3 intra-epithelial cervical neoplasms. Conservative treatment using CO2 lasers or conization: analysis of 2 years' experience].

Sixty four patients with stage III intra-epithelial neoplasia were seen in colposcopic clinic during a two years period. All were followed up for a minimum of 1 year (ranging 1 to 3 years). 28 patients 43.8%) were treated using the carbon dioxide laser in an ambulatory setting. In 36 patients (56.2%) a cone biopsy was first performed for therapy. Following laser treatment the failure rate is 7%, the failure and recurrence rate 14%. The CO2 laser appears to be an acceptable procedure for C.I.N. III lesions in young women. Selection of lesions is of paramount importance and should be able to reduce the number of therapeutic conizations.

[Study of fertility following celioscopic treatment of unruptured tubal pregnancy].

54 patients out of a series of 200 with ectopic pregnancies that were treated by laparoscopic means were followed up for more than 18 months so that post-operative fertility could be assessed. 35 have had intra-uterine pregnancy (64.8%), 3 of them after a second ectopic pregnancy treated by laparoscopy. 16 patients (29.7%) failed to conceive, four of them after a second ectopic pregnancy. 8 patients have a "solitary tube", 4 of them have had an intra-uterine pregnancy and one a second ectopic pregnancy. This results compare favorably with those obtained after a conservative management by laparotomy. History of infertility or previous ectopic pregnancy decreases the intra-uterine pregnancy rate (45.8% versus 80%, p less than 0.01).

The food-entrained prolactin and cortisol release in late pregnancy and prolactinoma patients.

Abrupt and concomitant increments in circulating cortisol and PRL levels in association with food intake at noon were observed in hyperprolactinemic pregnant women. During late pregnancy, the quantitative release was at least 12-fold greater for PRL and substantially more for cortisol compared to values in nonpregnant women. In contrast, patients with hyperprolactinemia due to prolactinoma exhibited an absence of the PRL response to the noon meal and a significantly smaller rise in cortisol, suggesting an independent mechanism for the release of these two hormones due to food ingestion. These observations may be important for future elucidation of neuroendocrine links between feeding and metabolic homoeostasis in pregnancy.

[Induction of ovulation and pregnancy resulting from chronic and pulsatile administration of gonadotropin-releasing-hormone (Gn-RH)].

Amenorrhoea of high origin, namely hypothalamic, reflects inhibition or a drop in endogenous secretion of Gn-RH. Two patients with this trouble were treated with synthetic Gn-RH in a pulsatile way and by a continuous method. We have been using a portable reprogrammable mini-pump (Zyklomat-Ferring) connected up to an intravenous catheter which has delivered 15 to 20 micrograms of Gn-RH for one minute every 90 minutes. This treatment brought about progressive rises in serum concentrations of oestradiol and of LH. The ovulation was obtained in both women, and checked by the demonstration by ultra-sound of rupture of the dominant follicle and by estimation of plasma and progesterone levels. One of them became pregnant following this treatment. This kind of therapy seems to us to be an attractive physiological alternative for induction of ovulation.

[Intraepithelial cervical neoplasia: incidence of associated condylomatous lesions. Results of CO2 laser treatment].

The present study reports our results using the CO2 Laser destruction of cervical intra-epithelial neoplasia (CIN) and points out the relationship with condylomatous lesions. 97 patients were selected and evaluated by cytology, colposcopy and directed punch biopsy. The cytologic criteria used for identification of virus infection are now well recognized. 12 CIN were overdiagnosed and lesions reclassified as condyloma. 85 CIN were studied. Epithelial changes suggestive of a condylomatous lesion were found in 27 cases (31.7%) and respectively 11.5% of CIN I, 37.8% of CIN II and 45.5% of CIN III. Further studies are required to evaluate the significance of condylomatous-CIN association. 70 women were treated by CO2 Laser including 24 CIN I, 32 CIN II and 14 CIN III. The primary failure rate for CIN I was 9.1%, CIN II 12.2%, CIN III 30.7%. However after a second treatment the total success rate was 93.8%. These data may give impetus for active management of precancerous lesions of the cervix.

Endogenous opiates modulate pulsatile luteinizing hormone release in humans.

To test the postulate that endogenous opioid peptides may be involved in the neuroendocrine mechanisms controlling the frequency and amplitude of LH pulses, saline and an opioid receptor antagonist, naloxone, were infused sequentially, each for 6-h intervals, in six normal cycling women during the luteal phase of the menstrual cycle. During naloxone infusion (1.6 mg/h), there was a significant (P less than 0.01) increase in both the frequency and amplitude of LH pulses compared to those in saline controls. FSH pulses were not discernible in individual subjects; however, a significant increment in FSH levels occurred concomitantly with the increase in LH. These data strongly suggest that endogenous opiates, through an inhibition of hypothalamic LRF, participate in the endocrine events leading to the low frequency of episodic LH secretion characteristic of the luteal phase of the human menstrual cycle.

Acute prolactin release triggered by feeding.

An abrupt increment in serum prolactin (PRL) levels was demonstrated within 45 minutes of initiation of lunch at 1200 or 1400 h and supper at 1700 h. These observations establish that feeding triggers an acute release of PRL by the hypothalamic-pituitary unit.

[Carbon dioxide laser and gynaecological surgery (author's transl)].

The authors describe their one year's experience with a CO2 Laser in gynaecology. They emphasise the ease of use of the equipment. Three major divisions of gynaecology can benefit from the CO2 Laser: benign or precancerous lesions of the vulva and of the vagina, dysplasia of the cervix, surgery of the fallopian tubes. In the future, technical progress will enable the CO2 Laser to be used in laparoscopy and its indications will certainly be extended to laparoscopic surgery.

Laparoscopy procedures to promote fertility ovariolysis and salpingolysis. Results of 93 selected cases.

In 93 selected infertile patients removal of periadnexal adhesions was attempted by laparoscopic procedure. Adhesiolysis was assessed according to Palmer's classification reviewed in Madrid (July 1980). Success was defined in terms of pregnancies, therefore patients with tubal obstruction were excluded. The rate of intra-uterine pregnancies was high (51,6%) and appeared to correlate with the quality of adhesiolysis. The present data strongly suggest that laparoscopic procedures represent a satisfactory method of adhesiolysis and emphasize the critical importance of case selection.
