Biologic markers in blood reflecting thyroid hormone effect at peripheral tissue level in patients receiving levothyroxine replacement for hypothyroidism.

Plasma fibronectin, serum procollagen-III-peptide and sex-hormone binding globulin as not specific markers of thyroid hormone effect at peripheral tissue level were determined and their values were related with serum levels of TSH, free-thyroxine and triiodothyronine during levothyroxine sodium replacement therapy for hypothyroidism. Low levels of biologic markers characteristic of hypothyroidism were normalized in consequence of hormone replacement and a negative correlation between their serum levels and TSH concentration was demonstrated in most subjects. However, in some patients a discrepancy in the response to levothyroxine between the pituitary and other target organs could be revealed. Additional evidence was disclosed that the pituitary thyrotroph sensitizes a minor decrease in serum thyroxine level, which would not be recognized by other target organs. Furthermore, it was revealed that during L-T4 replacement therapy in a large fraction of patients with subnormal serum TSH concentration blood levels of the measured markers often exceeded the upper limit of the normal range indicating the possibility of "tissue"-thyrotoxicosis beside the pituitary in other target organs, too. According to the present study which takes into consideration markers reflecting end-organ responsiveness to thyroid hormones it is recommended to adjust the dose of levothyroxine to maintain serum TSH in the normal range. For patients with subnormal TSH concentration a close follow-up is obligatory and in case od concomitantly raised free-thyroxine level the reduction of the levothyroxine dosage is proposed.

Disinfection of regenerated dialyzers with ozone.

Regenerated, ozone-disinfected dialyzers were use for a total of 190 hemodialyses in 35 patients without the occurrence of any complication or pyrogen reactions; the efficiency of hemodialysis was identical with that of new or formalin-disinfected dialyzers. The priming fluid contained in both compartments of ozone-disinfected dialyzers proved to be sterile. The ozone did not damage the dialyzer membrane or the supporting structure of dialyzer. Ozone can effectively substitute formalin and other chemical sterilants as a disinfectant of regenerated capillary dialyzers. The process can also be automated.

Radioimmunoassay of thyrotropin-releasing hormone in euthyroid subjects and in patients with thyroid diseases.

An episodic pattern of fluctuation in serum TRH level was demonstrated in euthyroid males. It is suggested that these rises in serum TRH-IR are due to its pulsatile hypothalamic release, however, it must be remembered that a high proportion of circulating TRH is derived from extrahypothalamic sources. The conception that the TSH surge during late evening is secondary to an increased TRH release could not be proved in the present study. In the euthyroid and primary hypothyroid groups the mean values of serum TRH-IR did not differ, while its level was slightly, but not significantly lower in hyperthyroid patients. The evaluation of a single TRH determination is impeded by the short half-life, low serum level and its production in extrahypothalamic sites, too.

Effect of beta-blocking agents in hyperthyroidism.

Propranolol modifies peripheral thyroxine metabolism, in opposition to the beta blocking agent GYKI 41099 which has no such effect. Both beta-receptor blockers produce a significant fall in the serum cAMP level of hyperthyroid subjects. It is suggested that the beneficial effect of the beta-blocking agents in hyperthyroidism is partly due to their depressive effect on cAMP production. As far as the action of propranolol is concerned, it may well involve an additional effect on T4 metabolism.

[Analysis of penicillin and ampicillin resistance in bacteria i. resistance and beta-lactamase production in clinical isolates (author's transl)].

The penicillin beta-lactamase (PBL) activity of the clinical isolates was measured at the same time by colorimetric microtitration, the iodometric method and chromatographic analysis. The minimal inhibitory concentration (MIC) of ampicillin (Ap) was determined for each isolate, the character of the coupled resistances, their frequency and distribution in the PBL producing and PBL-negative strains were also investigated. It was found that 1. the MIC value was 100 microgram/ml in 87.5 per cent of the Penicillin (P) and Ap resistant clinical isolates, 2. PBL was produced by 55--75 per cent of the P and Ap resistant strains depending on the species of the bacteria, 3. less than 50 per cent of the E. coli isolates produced PBL; in the case of other species this value was higher, about 65--70 per cent, 4. resistance to Su, SM, Tc, Nm and Cm was most frequently coupled to Ap and P resistance, 5. the distribution curve representing the number of the coupled resistance to the Ap and P shows a peak with 6 coupled antibiotics in the PBL-negative strains and 2 peaks in the PBL-producing strains.

Effect of propranolol on the plasma cyclic AMP and cyclic GMP levels to massive doses of glucagon in euthyroid and hyperthyroid subjects.

The plasma c-AMP level increases significantly in response to pharmacologica doses (1 mg i. v.) of glucaton. Beta blocking agents somewhat inhibit this increase in both euthyroid and hyperthyroid subjects. Catecholamine release elicited by glucagon thus plays some part in the response. In euthyroid subjects 2 microgram/kg glucagon still produces a significant increase in the plasma c-AMP concentration and propranolol fails to counteract this response. The c-AMP response elicited by identical doses of glucagon is more marked in hyperthyroid than in euthyroid subjects and can be somewhat reduced by propranolol. This indicates that in hyperthyroid patients even low doses of glucagon stimulate the release of catecholamines or enhance the responsiveness of beta receptors. The plasma c-GMP level increases in response to glucagon loading but in a lesser degree than does the c-AMP concentration. The response of c-GMP to glucagon is more protracted than the response of c-AMP and propranolol fails to counteract it.

Radiocirculographic study of the effect of ethyl apovincaminate on the cerebral circulation.

The effect of ethyl apovincaminate (RGH-4405, Cavinton) on cerebral circulation has been studied with radiocirculography (rc) in 12 healthy individuals and 17 patients suffering from cerebrovascular diseases. It was shown that acute Cavinton effect produced prolongation of circulation time both in healthy individuals and in more than half of our patients. Shortening of circulation time was observed in one case. On long-term Cavinton treatment circulation time was prolonged in 7 cases, not changed in 7, while in 3 cases it was shortened. The circulation times measured by rc did not reflect changes in the clinical picture.

Euthyroid endocrine ophthalmopathy: a study of the pituitary-thyroid system.

The response of TSH to TRH is often abnormal in patients with euthyroid endocrine ophthalmopathy and, on the evidence of the present observations, falls into different patterns. The close relationship between the euthyroid and the hyperthyroid types of endocrine ophthalmopathy and the possibility of the two forms representing in reality successive phases of the same process are pointed out. Demonstration of LATS in the serum, slightly increased serum-triiodothyronine-levels, failure of normal response to TRH and an abnormal triiodothyronine-suppression test in euthyroid ophthalmopathy may herald the manifestation of hyperthyroidism at some later time.
