[Speed of intubation using a new neuromuscular blocker. Rocuronium bromide (ORG 9426)].

OBJECTIVES: To compare intubation conditions and neuromuscular parameters, patency time, maximum level of block, time and clinical duration of effect for rocuronium bromide 0.6 mg/kg (ORG 9426) versus equally potent doses of vecuronium and atracurium under similar experimental conditions. PATIENTS AND METHODS: Sixty patients were divided into three groups of twenty. Intubation conditions were scored on a scale of 3-12 points based on relaxation of the vocal cords, presence of cough and ease of laryngoscopy after 60 or 90 sec. Neuromuscular parameters were obtained by integrated electromyography of the thenar and hypothenar muscle structure, evoked by supramaximal stimuli of the cubital nerve in train of four (2 Hz in 2 sec). The shape of the electrocardiographic curve, heart rate and mean arterial pressure measured non-invasively were recorded throughout the surgical procedure. RESULTS: Means of onset times and times of effect for rocuronium (33 and 135 sec) were significantly lower than those obtained with vecuronium and atracurium. Clinical duration, free of hemodynamic changes, was similar in the three groups. Rocuronium produces excellent intubation conditions 60 sec after administration, although at this point peripheral muscle block was only partial. Rocuronium may be superior to other neuromuscular blockers available today as a result of the speed with which it affords excellent intubation conditions.

[A comparative study of hepatic cholestasis after infusion of long chain triglycerides and a mixture of medium and long chain triglycerides].

A randomized, double blind prospective study made on surgical patients who required parenteral nutrition during a 10-day period, with complete fasting. The patients were required to show a normal hepatic function measured by gamma-GT, alkaline phosphatase (FA), normal bilirubin and ALT. The evolution of the cholestasis parameters was observed on days 0, 1, 3, 8 and 10. An increase in gamma-GT was observed in the groups. This was much greater in the group with LCT (p<0.005) on the tenth day than in the MCT/LCT group. FA increased only in the LCT group, and was statistically significant (p<0.001) on the tenth day compared with the MCT/LCT group.

DNA ploidy and S + G2M fraction (proliferative index) as prognostic determinants in human periampullary cancer.

BACKGROUND/AIMS: The aim of the present study was to investigate the relationship between pathological factors, DNA ploidy patterns, proliferative index and survival in a population of patients undergoing resection for adenocarcinoma of periampullary region. MATERIALS AND METHODS: DNA ploidy and proliferative index of periampullary cancer tissue was measured by flow cytometry in 73 patients who had undergone pancreatoduodenectomy. RESULTS: Seventy-four percent (n = 54) of cancers were diploid and 26% (n = 19) were aneuploid. The median survival time of patients with diploid cancers was 30.1 (s.d. 5.1) months and with aneuploid cancers was 16.0 (s.d. 2.9) months (p = 0.02). Kaplan-Meier survival curves showed that the patients with aneuploid cancers succumbed to recurrent disease within 40 months after surgery whereas 20% of patients with diploid cancers survived longer than 40 months. In patients with long survival (3 years) there was a significant difference between aneuploid and diploid cancers (p < 0.001). In the multivariate analysis, tumor location and tumor size are independent prognostic variables for the length of survival after resection of periampullary cancer. CONCLUSIONS: DNA ploidy pattern appears to be one important prognostic predictor in patients with "long term" survival after successful pancreatoduodenectomy for periampullary cancers.
